Vyvanse vs. Adderall. Top NYC Psychiatrist Dr Edward Fruitman, M.D. from Trifecta Health ( https://trifectahealthnyc.com) shares his opinion on this matter. Both amphetamine-based stimulants are used for ADD/ADHD in order to improve attention and concentration for those affected by the disorder. The difference, however, comes from how they are formulated and how they are taken.

Adderall, is more flexible as it can be taken multiple times during the day, depending on the individual's needs. Vyvanse, on the other hand, is commonly taken once a day. Furthermore, adderall has an immediate release; which can produce a "peak effect" which can be undesirable for those who do not want to feel "over-stimulated". Whereas, Vyvanse will not produce this "peak effect" as it its release is extended and produces consistent and balanced stimulation throughout the entire day. Furthermore, Vyvanse cannot be broken up or snorted; therefore one must swallow it whole which cuts down on the risk of over-use or abuse.

*What are ADHD meds?*
Most ADHD drugs are stimulants. They stimulate and activate your central nervous system within 15 or 20 minutes of popping a pill. Caffeine is a stimulant, but ADHD drugs are a LOT more powerful. Like Adderall, which is one of the most commonly prescribed stimulant medications in the U.S. The active ingredient is a version of amphetamine, which has been around for decades.

*What is ADHD?*
ADHD stands for attention deficit hyperactivity disorder, and it’s one of the most common mental health conditions in people under 18. It’s like having 100 tabs open on your browser, but in your brain, so you end up paying attention to the WRONG stuff instead of focusing on what you actually need to get done. In the U.S., Adderall is the most common drug prescribed to treat ADHD.

*How do ADHD drugs help people with ADHD?*
A lot of it has to do with a specific neurotransmitter in the brain that you’ve probably heard of before – dopamine. It’s key when it comes to your brain’s reward pathways. When you have a normal level of dopamine hanging out in the synapses of your brain, you’re able to maintain motivation when you’re doing something you’re not interested in. But for someone with ADHD, there’s less dopamine hanging out in the synapses. So, there’s less motivation to get things done. To get that dopamine level up, there’s more of a desire for short-term rewards. ADHD meds work by helping dopamine to stick around longer in the synapses. For people with ADHD, they can stay focused and avoid distractions, and that can be a total game-changer when it comes to getting stuff done at school or at work.

*So what happens when someone WITHOUT ADHD takes an ADHD drug like Adderall?*
Many studies in the lab don’t show that people without ADHD get any boost to their cognition when they take ADHD drugs, but real-life situations like exams and writing papers haven’t fully been tested. But many studies do show that these kinds of meds make you think you did better than you actually did. And there are worrisome side effects that can hurt you long term.

Dr. Pagnani of Rittenhouse Psychiatric Associates discusses two categories of controlled stimulant medications used to treat ADHD in adults: amphetamines and methylphenidates. Amphetamines (like adderall, vyvanse and dexedrine) and methylphenidates (like ritalin, concerta and focalin) both increase availability in the extraneuronal space of norepinephrine and dopamine by preventing their reuptake and also increasing their release. These medications have significant abuse potential and patients can develop physiological dependence and tolerance. They can also cause withdrawal, if taken regularly and decreased or stopped abruptly. There are various side effects, some potentially serious, and taking these medications must be under the direct supervision of a medical professional.

Dr. Pagnani discusses that in his clinical experience, he's found that the amphetamine stimulants seem to have a higher likelihood of causing anxiety, irritability, insomnia, restless, tiredness or "crash," between doses or when the medication is wearing off. With that said, these medications may work better for individuals with significant hyperactivity as part of their illness, than the methylphenidates, and there are certainly many patients who tolerate them well. Methylphenidates are often preferred (when medication management with a controlled medication is warranted) for patients who's primary symptom of ADHD is difficulty with focus (mental, as opposed to physical hyperactivity). He discusses that he believes there's a lower abuse potential (although there certainly IS abuse potential) with methylphenidates than amphetamines.

Do stimulants change your personality? If you look this question up on the Internet you will probably find that the short answer is no. But if you really do a deep dive into the research on how these medications affect the brain the longer answer is: probably. I think the reason for the no answer is because of how we define personality. Your personality is really something that develops in late adolescence or early adulthood and is based on a number of factors including genetics, environment, how you were nurtured etc. Probably the most popular personality disorder we hear about his narcissistic personality disorder. And I would agree that it's going too far to suggest that these medications can make someone a narcissist.

I discuss how stimulants work in the brain and how this can lead to blunting of your emotions. I also discuss research that suggests that stimulants can have long term effects on the brains of children and adolescents.

What is Adderall?
Adderall is a central nervous system stimulant used for ADHD and narcolepsy.

So, is Adderall bad for you? Well, here are some Adderall side effects:

• Insomnia 
• Dry mouth
• Loss of appetite 
• High/low blood pressure 
• Raynauds
• Increased heart rate
• Erectile dysfunction
• Abdominal pain
• Constipation
• Nose bleeds 
• Tics
• Loss of bladder control
 
High doses of Adderall can even put you in a coma or cause death.

Brand names: Adderall and Mydayis.
Available as a tablet and capsule.

How to use it?
- Tablet: swallow it whole with half a glass of water during a meal. This may lower the side effects
- Extended release tablets or capsule: swallow it whole with half a glass of water during a meal, but do not crush, chew, break, or open them. This can lower their effectivity.

When to use it?
Adderall contains amphetamine and dextroamphetamine. Those have a stimulating effect on the central nervous system, and impact the part of your brain which contributes to hyperactivity and impulse control.
Adderall can be used to treat attention deficit hyperactivity disorder (ADHD) and narcolepsy.

What is the correct dosage?
The precise dose will depend on your specific indication of use, the side effect you might experience and the severity of your symptoms.Therefore always use the precise amount and frequency your doctor prescribed you.

- However, when treating ADHD symptoms with regular tablets in adults: a usual starting dose would be 5 mg orally 1 or 2 times a day. This can be slowly increase up to a maximum dose of 40 mg per day.
- When treating narcolepsy with regular tablets in adults your doctor might prescribe an initial dose of 20mg per day divided over several dosages.

What are common the side-effects?
Adderall may cause nervousness, restlessness, excitability, irritability, agitation, dizziness, headache, fear, anxiety, agitation, tremor, weakness, blurred vision, sleep problems (insomnia), dry mouth or unpleasant taste in the mouth, diarrhea, constipation, stomach pain, nausea, vomiting, fever, hair loss, loss of appetite, weight loss, loss of interest in sex, impotence, difficulty having an orgasm, increase blood pressure, increased heart rate, and heart palpitations.

Safety:
First of all be careful to combine Adderall with alcohol. As this might increase its dazing effects.
In addition, it can be dangerous to drive while using this drug. As it can cause you to become drowsy, sleepy and/or dizzy. If you experiencing any of these side effects than please do not drive. This is especially true for the first few weeks you are using it.
Furthermore, adderall can have interactions with other drugs your are taking. always discuss this with your prescribing doctor.
And avoid drinking fruit juices or taking vitamin C at the same time you take Adderall. These can decrease the effectivity of adderall.

Dr. Pagnani of Rittenhouse Psychiatric Associates discusses Strattera (Atomoxetine) a norepinephrine reuptake inhibitor that can be used in the treatment of ADHD for adults. This medication does not have the abuse potential that you see with stimulants like Amphetamines (Adderall, Vyvanse) or Methylphenidates (Ritalin, Concerta) and it is not controlled. It does have various side effects (and monitoring vitals and seeing your primary care provider for regular visits is recommended). With that said, it may be a safer option than many ADHD medications, and can be surprisingly effective. Dr. Pagnani discusses that it's not uncommon that he evaluates adults who trialed this medication, but did not have it increased to typical doses that are effective for adults. He also discusses that some individuals who are poor metabolizers of P-450 2D6 may do well on very low doses (and become toxic on higher doses), and it's vital that individuals seek treatment from a medical professional, for these reasons.

In this episode, I explain how medications such as Adderall, Ritalin, Vyvanse and other stimulants work to increase focus and treat attention-deficit/hyperactivity disorder (ADHD). I explain the brain circuits involved in focus and the key roles dopamine and norepinephrine play in their regulation. Then I explain how stimulants such as Adderall and Vyvanse can increase focus and reduce hyperactivity in kids or adults with ADHD, and how and why Ritalin and other medications (e.g., Modafinil, Guanfacine) may work better for some. I explain the long-term effects of ADHD medications on height, cardiovascular health, hormones, predisposition to addiction and psychosis, and whether these treatments can be used and then stopped. I also discuss the immense individual variation in dosage sensitivity for these medications and the negative side-effects that occur from recreational use. This episode ought to benefit anyone with ADHD, their parents, those on ADHD medications or anyone curious about how these medications work.
