The FDA approved Buspar for anxiety in 1986, and it has been available as a generic busprione since 2001. Pharmacologists classify buspirone as a serotonin 1A partial agonist, as its primary action appears to be the activation of pre-synaptic serotonin receptors, which gradually decreases firing of serotonin neurons.

For years, buspirone has been considered a third or fourth tier option for treating ADHD. In addition to effectively treating anxiety associated with ADHD, it may also provide direct benefit for ADHD. 1-2-pyrimidinylpiperazine (1-PP), a major metabolite of buspirone, is an alpha-2 antagonist, which boosts levels of norepinephrine, dopamine and serotonin. Limited research indicates buspirone does reduce ADHD symptoms in humans.

A handful of studies, almost all in rodents, indicate that buspirone can successfully treat several side effects associated with Ritalin, including teeth grinding (bruxism), liver toxicity, and growth inhibition. Buspirone also appears to reduce the potential for substance abuse and cognitive dulling that are associated with long term Ritalin use in rodents.

In September 2023, the FDA approved gepirone, (Exxua) which is chemically similar to buspirone, for the treatment of depression. Gepirone is also metabolized to 1-PP, so you may well be hearing more about this class of drugs for depression, anxiety, and ADHD.

Your Medication Buspirone

Buspirone is also known by the brand name: Buspar.

Buspirone comes in 5, 7.5, 10, 15, and 30 mg tablets.

Buspirone is an anti-anxiety agent most commonly used to treat anxiety. No matter what your doctor has you taking it for, buspirone is taken by mouth twice daily. Buspirone may be taken with or without food, if upset stomach occurs, take with food. If you miss a dose, take the missed dose as soon as you think about it. If it is close to time for your next dose, skip the missed dose and resume your normal dosing schedule. Do not take 2 doses at the same time or extra doses.

Some common side effects with buspirone include dizziness, drowsiness, headache and nausea.
Some less common side effects include nervousness, confusion, outbursts of anger, blurry vision and weakness.

While taking buspirone, remember it is important to tell your doctor or pharmacist if you have:
• Signs of an allergic reaction (rash, itching, swelling, tightness of the chest, etc.)
• An allergy to buspirone.
• Taken certain drugs used for low mood (depression) like isocarboxazid, phenelzine, or tranylcypromine or drugs used for Parkinson's disease like selegiline or rasagiline in the last 14 days. Taking this drug within 14 days of those drugs can cause very bad high blood pressure.
• Any of these health problems: Kidney disease or liver disease.
Buspirone needs to be stored at room temperature in a dry place. Do not store in bathroom. Keep all drugs in a safe place and out of the reach of children and pets.

Dr. Pagnani of Rittenhouse Psychiatric Associates discusses Buspar or buspirone, which is a medication that is used to treat anxiety disorders. This medication does not have the abuse and dependency risk that many anxiolytics (like benzodiazepines have) and generally causes less risk of sexual dysfunction as your SSRIs and SNRIs. But with that said, it requires 2 - 3 time dosing throughout the day, isn't always tolerated, and may not be as effective as your SSRIs or SNRIs for some patients.
