Esomeprazole and Domperidone

Uses:

Esomeprazole and domperidone is a combination medication that is used to treat gastroesophageal reflux disease (GERD) and other conditions that involve excessive stomach acid production. Esomeprazole is a proton pump inhibitor (PPI) that works by reducing the amount of acid produced by the stomach. Domperidone is a dopamine antagonist that helps to improve gastric emptying, which can help to relieve symptoms of GERD such as heartburn and regurgitation.

Doses:

The usual dose of esomeprazole and domperidone is one capsule taken once daily. The capsule should be taken at least one hour before eating.

Missed Doses:

If you miss a dose of esomeprazole and domperidone, take it as soon as possible. However, if it is almost time for your next dose, skip the missed dose and take your next dose at the regular time. Do not take a double dose to make up for a missed dose.

Storage:

Esomeprazole and domperidone should be stored at room temperature away from light and moisture.

Domperidone is used short-term in breastfeeding mothers of children being treated with the aim of improving breast milk supply.
While using this medicine – Remember
• Domperidone is not recommended for use in mothers with cardiac arrhythmias or mothers taking any medication which may cause cardiac arrhythmias.
• Domperidone in very small amounts enters the breast milk but there have been no known cases of harmful cardiac effects on babies.
• If you are taking any other medication, including over the counter or complementary medicines (also known as traditional or alternative medicines including vitamins, minerals, herbs, aromatherapy and homeopathic products) from a health food store, discuss this with your doctor prior to commencing domperidone
When and how should I take domperidone?
• Domperidone is used together with frequent expressing. It will work best for you if you are expressing regularly with an electric breast pump – this is at least eight times per day with no breaks longer than five hours.
• Domperidone is normally taken by the breastfeeding mother every eight hours (three times per day). A suggested regime would be at 6am, 2pm and 10pm. Check with the lactation consultant or pharmacist on the best way to fit this medicine into your and your baby’s routine.
How much should I take?
• The usual starting dose is 10 mg (one tablet) every eight hours (three times per day e.g. 6 am, 2 pm and 10 pm). The starting dose at the hospital should never exceed 30mg (3 tablets) in each 24 hours.
• Once your milk supply is well established, keep taking the dose of one tablet (10mg) every eight hours (three times per day) for one more week; then it may be possible to gradually reduce the medicine slowly over a few days and then stop. It is advisable to contact the lactation consultant before you plan to stop this medication, to review your milk supply, and then talking to the doctor who prescribed it to give you a weaning plan.
• The dose may change, depending on each woman’s individual response. Rarely, doses of up to 60mg (6 tablets) are used in each 24 hours, prescribed by the GP.
Side effects you must do something about
• Dizziness, fainting or heart palpitations. Stop taking domperidone and seek medical attention
• Muscle spasms
• Restlessness

Domperidone, sold under the brand name Domstal among others, is a medication used as an antiemetic, gastric prokinetic agent, and galactagogue.

Domstal is a medicine that increases the movements or contractions of the stomach and bowel. Domstal is also used to treat nausea and vomiting caused by other drugs used to treat Parkinson's Disease.
Domperidone is to be given only by or under the immediate supervision of your doctor. Domstal should be used at the lowest effective dose for the shortest duration necessary to control nausea and vomiting.
It is recommended to take oral Domstal tablets before meals. If taken after meals, absorption of the drug is somewhat delayed.
Patients should try to take each dose at scheduled time. If a scheduled dose is missed, the missed dose should be omitted and the usual dosing schedule resumed. The dose should not be doubled to make up for a missed dose.
Usually, the maximum treatment duration should not exceed one week.
Side effects associated with domperidone include dry mouth, abdominal cramps, diarrhea, nausea, rash, itching, hives, and hyperprolactinemia (the symptoms of which may include breast enlargement, galactorrhea, breast pain/tenderness, gynecomastia, hypogonadism, and menstrual irregularities)

Domperidone is a dopamine D2 receptor antagonist that belongs to the classes of prokinetic agents and antiemetics.
It is primarily used to enhance gastrointestinal motility and relieve symptoms of nausea and vomiting.
Major Indications of domperidone include the following.
Nausea and vomiting associated with various conditions, including chemotherapy, surgery, and delayed gastric emptying.
In the treatment of gastroesophageal reflux disease, as an adjunct to proton pump inhibitors in refractory cases.
Gastroparesis, which causes delayed gastric emptying, especially in diabetic individuals.
In functional dyspepsia to improve symptoms of bloating and early satiety.
And to enhance lactation, by increasing prolactin levels.
Domperidone blocks dopamine D2 receptors in following areas to exert its pharmacological effects.
The chemoreceptor trigger zone, located outside the blood-brain barrier, reducing nausea and vomiting.
The gastric antrum and duodenum: This enhances gastrointestinal motility by:
Increasing esophageal peristalsis and sphincter tone.
Promoting gastric emptying.
And reducing pyloric sphincter tone.
Domperidone does not cross the blood-brain barrier significantly, resulting in fewer central nervous system side effects.
Domperidone is commonly administered as an oral tablet or a suspension.
In addition, intravenous preparations and suppositories are also available.
The drug has a poor oral bioavailability due to extensive first-pass metabolism in the liver and intestinal wall.
It is metabolized in the liver primarily by cytochrome P450 enzymes.
Domperidone is mainly excreted in feces via biliary excretion.
A smaller fraction is excreted in urine as metabolites.
And the half-life is approximately 7-9 hours.
Domperidone is generally a well-tolerated drug.
However, it might cause the following adverse effects in certain individuals.
Gastrointestinal effects such as dry mouth, diarrhea, or abdominal cramps.
Endocrine effects like elevated prolactin levels, leading to Galactorrhea, Gynecomastia, and Menstrual irregularities.
And cardiovascular effects such as QT interval prolongation, torsades de pointes, and sudden cardiac death, especially in older patients or with higher doses.
Contraindications for the use of domperidone include the following.
Known QT prolongation.
Congestive heart failure or significant bradycardia.
Concurrent use of drugs that prolong the QT interval, such as ketoconazole and erythromycin.
Severe hepatic impairment, as domperidone is extensively metabolized in the liver.
Prolactin-secreting tumors.
And known hypersensitivity to domperidone.
