Educational Pearls:

1. Bactrim (TMP-SMX) is a sulfa antibiotic used for a number of infections but can have untoward effects.
2. Hypoglycemia can be induced by the sulfa component of Bactrim, which binds receptors in the pancreas causing insulin to be secreted, dropping blood sugar levels.
- Oral diabetic medications, specifically sulfonylureas, when taken with Bactrim most often lead to these hypoglycemic episodes.
3. Hyperkalemia can be induced by Trimethoprim by blocking potassium excretion in the kidney, so Bactrim in combination with ACE inhibitors/ARBs/spironolactone can induce high potassium.
4. When prescribing Bactrim, remember to check home medications and if a patient is taking an above medication, check a baseline BGL and potassium.

In this video we will cover everything you need to know about Ceftriaxone, also known as Rocephin. We will discuss how and when to use it, we will cover its side effects, dosage and much, much more.

Generic name: Cotrimoxazole.
Brand names: Bactrimel.
Available as tablets, suspension and via drip.

How to use it?
- Regular tablet: just swallow it whole with half a glass of water.
- Suspension: shake the bottle before use. Measure the correct amount with the measuring cup or measuring spoon. Drink that amount. Rinse the measuring cup with some water afterwards and drink that too.
- Intravenous administration: your doctor or nurse will take care of this. You don’t need to arrange this yourself.

Furthermore, it is important your drink at least 1,5-2L of water, during your treatment with Cotrimoxazole. This will prevent this medicine from crystallizing in the kidneys and causing kidney stones.

When to use it?
Cotrimoxazole is part of a group of antibiotics called Sulfamethoxazole/Trimethoprim. They can be used to treat infectious with several types of bacteria. For example:
- Infections of the respiratory tract, like a pneumonia.
- Gastrointestinal infections, like a stomach infection.
- Urinary tract infections and a middle ear infection.

What is the correct dosage?
The precise dose will depend on your specific indication of use, the side effect you might experience and the severity of your symptoms. Therefore always use the precise amount and frequency your doctor prescribed you.
- However, when treating most infections in adults your doctor might prescribe a dose of 960 mg 2×/day. During serious infections this dose can be increased up to 960 mg 3x/day.

Cotrimoxazole is usually given up to 2 days after the symptoms have disappeared, but at least for 5 days. If there is no clear improvement of your symptoms after 7 days, than contact your doctor. Your treatment needs to be reconsidered. And for long-term treatment: the dose is usually halved after 14 days.

What are the side-effects?
- Common (1–10%), nausea, vomiting, skin rash and elevated blood values of several parameters.
- Uncommon (0.1–1%): Convulsions, diarrhea, impaired renal function, deafness and an inflammation of the liver.
- Rare (0.01–0.1%): hallucinations and inflammation of the throat, stomach, pancreas, nerves and blood vessels.
- Very rare (0.01%): allergic reaction, convulsion, dizziness, meningitis, shortness of breath, coughing, muscle pain and several serious side effects.

Safety:
First of all there are no limitations for this drug regarding the usage of alcohol or driving a car.
However, cotrimoxazole can have interactions with other drugs your are taking. always discuss this with your prescribing doctor.
Lastly, cotrimoxazole can be dangerous for pregnant women, women who are breastfeeding and their babies. Therefore always discuss this with your personal doctor.

Bactrim:

Generic name: sulfamethoxazole and trimethoprim.
Brand names: Bactrim, Bactrim DS.
Drug class: Sulfonamides.

What is Bactrim?

Bactrim contains a combination of sulfamethoxazole and trimethoprim. Sulfamethoxazole and trimethoprim are are both antibiotics that treat different types of infection caused by bacteria.

What is Bactrim used for?

Bactrim is a prescription medicine used to treat ear infections, urinary tract infections, bronchitis, traveler's diarrhea, shigellosis, and Pneumocystis jiroveci pneumonia.

To help reduce the development of drug-resistant bacteria and maintain the effectiveness of Bactrim, this medicine should be used only to treat infections that are proven, or strongly suspected, to be caused by susceptible bacteria.

Warnings:

You should not use Bactrim if you have severe liver disease, kidney disease that is not being monitored, anemia caused by folic acid deficiency, if you take dofetilide, or if you have had low platelets caused by using trimethoprim or a sulfa drug.

You should not take Bactrim if you are pregnant or breastfeeding.

Use Bactrim only as directed. Tell your doctor if you use other medicines or have other medical conditions or allergies.

Before taking this medicine:

You should not use Bactrim if you are allergic to sulfamethoxazole or trimethoprim, or if you have:

severe liver disease;

kidney disease that is not being treated or monitored;

anemia (low red blood cells) caused by folic acid deficiency;

a history of low blood platelets after taking trimethoprim or any sulfa drug; or

if you take dofetilide.

Sulfamethoxazole and trimethoprim may cause birth defects. Do not use Bactrim if you are pregnant. Tell your doctor if you become pregnant.

Do not breastfeed.

Bactrim should not be given to a child younger than 2 months old.

To make sure you can safely take Bactrim, tell your doctor if you have ever had:

kidney or liver disease;

a folate (folic acid) deficiency;

asthma or severe allergies;

HIV or AIDS;

a thyroid disorder;

malnourishment;

alcoholism;

an electrolyte imbalance (such as low blood sodium or high potassium);

porphyria, or glucose-6-phosphate dehydrogenase (G6PD) deficiency; or

if you use a blood thinner (such as warfarin) and you have routine "INR" or prothrombin time tests.


How should I use Bactrim?

Take Bactrim exactly as prescribed by your doctor. Follow all directions on your prescription label and read all medication guides or instruction sheets.

Drink plenty of fluids to prevent kidney stones.

Antibiotic medicines can cause diarrhea. Tell your doctor if you have diarrhea that is watery or bloody.

Keep using Bactrim even if your symptoms quickly improve. Skipping doses could make your infection resistant to medication. Bactrim will not treat a viral infection (flu or a common cold).

You may need blood and urine tests, and this medicine may be stopped based on the results.

Store Bactrim at room temperature away from moisture, heat, and light. Do not refrigerate.

What happens if I miss a dose?

Use the medicine as soon as you can, but skip the missed dose if it is almost time for your next dose. Do not use two doses at one time.

What happens if I overdose?

Seek emergency medical attention.

Overdose symptoms may include loss of appetite, vomiting, fever, blood in your urine, yellowing of your skin or eyes, confusion, or loss of consciousness.

What should I avoid while using Bactrim?

Bactrim could make you sunburn more easily. Avoid sunlight or tanning beds. Wear protective clothing and use sunscreen (SPF 30 or higher) when you are outdoors.

Bactrim side effects:

Get emergency medical help if you have signs of an allergic reaction to Bactrim (hives, cough, chest pain, shortness of breath, swelling in your face or throat) or a severe skin reaction (fever, sore throat, burning eyes, skin pain, red or purple skin rash with blistering and peeling).

Seek medical treatment if you have a serious drug reaction that can affect many parts of your body. Symptoms may include: skin rash, fever, swollen glands, joint pain, muscle aches, severe weakness, pale skin, unusual bruising, or yellowing of your skin or eyes.
