quinine is one of the dangerous drugs which need to be diluted before giving either intravenously (iv) or intramuscular(im) because the treatment quinine is very strong side effects. iv quinine is one of the effective treatment agents sever malaria and artesunate both iv and im artesunate.so diluting we use 18 mils of dextrose 5% and 2 mills of quinine .but toddy i will use 10 mills .so i will aspirate 9 mills of dextrose 5% and aspirate 1mill of iv quinine.so after diluting you calculate and give iv quinine in the dextrose
note never ever give iv quinine direct it is very dangerous.

ISSUE: Due to continued reports of serious side effects in patients using quinine (Qualaquin) ''off-label'' for night time leg cramps, FDA has approved a risk management plan to warn against the use of this drug for such unapproved uses. Quinine should not be used for night time leg cramps. Quinine use may result in serious and life-threatening hematological reactions, including serious bleeding due to thrombocytopenia, and hemolytic-uremic syndrome/ thrombotic thrombocytopenic purpura, which in some cases may result in permanent kidney damage. In some patients, adverse reactions result in hospitalization and death.

calculating the doses and the mils from prescribe mg of quinine
and how i give iv quinine .i always give in dextrose 5 %.
but before i give iv quinine ,i dilute the quinine with dextrose 5% because concentrated quinine is very dangerous. i used 18 mils of dextrose 5% and add 2mils of quinine to make 20 mills that's my diluent. How i calculate the dose is 10mg/kg. the dose of quinine is calculated per kilogram body weight.
then i calculate the mils from the the prescribe mg and then i put in dextrose 5% to run for four hours in fusion .don't run it very first.
the side effects are
headache, hypoglycemia.
some fever
that is how i give iv quinine and calculate the dose

What is quinine and what is quinine used for?


Quinine is used alone or with other medications to treat malaria (a serious or life-threatening illness that is spread by mosquitos in certain parts of the world). Quinine should not be used to prevent malaria. Quinine is in a class of medications called antimalarials. It works by killing the organisms that cause malaria.


How to use quinine?


Quinine comes as a capsule to take by mouth. It usually is taken with food three times a day (every 8 hours) for 3 to 7 days. Take quinine at around the same times every day. Follow the directions on your prescription label carefully, and ask your doctor or pharmacist to explain any part you do not understand. Take quinine exactly as directed. Do not take more or less of it or take it more often than prescribed by your doctor.

Swallow the capsules whole; do not open, chew, or crush them. Quinine has a bitter taste.

You should begin to feel better during the first 1-2 days of your treatment. Call your doctor if your symptoms do not improve or if they get worse. Also call your doctor if you have a fever soon after you finish your treatment. This could be a sign that you are experiencing a second episode of malaria.

Take quinine until you finish the prescription, even if you feel better. If you stop taking quinine too soon or if you skip doses, your infection may not be completely treated and the organisms may become resistant to antimalarials.

Other uses for this medicine.

Quinine is also sometimes used to treat babesiosis (a serious or life-threatening illness that is transmitted from animals to humans by ticks). Talk to your doctor about the risks of using this medication for your condition.


What special dietary instructions should I follow?


Unless your doctor tells you otherwise, continue your normal diet.


What should I do if I forget a dose?


Take the missed dose as soon as you remember it. However, if it has been more than 4 hours since the time you should have taken the missed dose, skip the missed dose and continue your regular dosing schedule. Do not take a double dose to make up for a missed one.


What side effects can quinine cause?


This medication may cause low blood sugar. You should know the symptoms of low blood sugar and what to do if you develop these symptoms.
Quinine may cause side effects. Tell your doctor if any of these symptoms are severe or do not go away:

nausea.
restlessness.
difficulty hearing or ringing in the ears.
confusion.
nervousness.

Treatment of falciparum (malignant tertian) malaria.
Treatment and prevention of nocturnal leg cramps in adults and the elderly, when cramps cause regular disruption of sleep.

1. Neuropsychiatric (anxiety, depression, psychosis)
