- In 3 minutes you'll learn:
- What is Furosemide ( Lasix 40 mg )?
- Furosemide is a loop diuretic. It inhibits sodium and chloride reabsorption in proximal part of ascending loop of Henle. It acts directly on the cells of the nephron and indirectly modifies the content of the renal filtrate.
- It is available as 20, 40, and 500 mg tablets, and also available as injection.
- Furosemide is used in the treatment of edema associated with CHF, hepatic cirrhosis, and renal disease; hypertension.
- Edema
- Adult: Initial: PO 20–80 mg/d as single dose. Increase by 20–40 mg q6–8h prn. Maximum: 600 mg/d.
- Hypertension
- Adults: Initial: PO 40 mg b.i.d.
- CHF, chronic renal failure
- Adults: PO 2–2.5 g/d.

Loop diuretics pharmacology nursing review that includes the mechanism of action, side effects, nursing interventions, and patient education.
Loop diuretics mechanism of action is on the thick ascending limb of the loop of Henle. These medications will inhibit the sodium-potassium-chloride (NKCC2) cotransporter. When this cotransporter is inhibited, it will decrease the ions (sodium, potassium, and chloride) reabsorbed into the blood.
In turn, this will decrease sodium reabsorption in the loop of Henle (which normally reabsorbs about 25% of sodium) and decrease the tonicity of the medulla, which is normally hypertonic. The end result will be that the collecting ducts and loop of Henle will reabsorb less water back into the blood and more water will leave the nephron as filtrate.
Side effects of loop diuretics include: hypokalemia, hypocalcemia, hypomagnesemia, ototoxicity, metabolic aklaosis, and dehydration.
Loop diuretics are used to treat heart failure, pulmonary edema, hypercalcemia, and liver impairment (due to ascites).

Generic Name: Furosemide
Trade Name: Lasix
Indication: edema, hypertension
Action: prevents reabsorption of sodium and chloride in the kidneys, increase excretion of water, sodium, chloride, magnesium, potassium
Therapeutic Class: diuretics
Pharmacologic Class: loop diuretics
Nursing Considerations:
use caution with liver disease
may cause hypotension, dry mouth, excessive urination, dehydration, electrolyte abnormalities, metabolic alkalosis
hypokalemia may lead to increase risk of digoxin toxicity
monitor renal panel
use caution with other anithypertensives
causes arthritic symptoms/do not administer with aminoglycosides due to ototoxicity
