MEDICAL ANIMATION TRANSCRIPT: A vaginal yeast infection is a common condition where too much yeast grows in a woman's vagina. Other names for it are vulvovaginal candidiasis and candida vulvovaginitis. Yeast is a type of fungus which is a plant-like organism. The type of yeast that usually causes the infection is called candida albicans. Your vagina is the passage from the uterus to the outside of your body. The surface inside your vagina normally contains small amounts of this yeast, which usually doesn't cause any problems. Other germs, such as the healthy bacterium Lactobacillus, also live in the vagina with Candida. They keep each other in balance. Sometimes conditions change in the vagina that make the yeast grow out of control. When this happens, yeast cells invade the lining of your vagina, called mucosal epithelium. As the yeast grows and damages your cells, it causes immune cells to enter the tissue. This inflames and damages the tissue as your immune cells try to clear the infection. Unfortunately, yeast cells can avoid immune destruction by killing immune cells. Or, yeast cells may grow too large for immune cells to destroy them. This allows the yeast to grow and spread, forming a layer called a biofilm in your vagina. It's not always clear why a person gets a yeast infection. But, you're more likely to get a yeast infection if you are pregnant, have diabetes. wash the inside of your vagina, also known as douching, have a weakened immune system, take birth control pills, or have taken antibiotics or steroid drugs. The most common symptom of a yeast infection is an extreme itchy feeling in your vagina and your vulva, which is the area around the entrance to your vagina. Other symptoms may include redness, burning and swelling in your vagina or vulva, pain during urination or sex, and a thick white odorless discharge from your vagina. You can treat a vaginal yeast infection with antifungal medicines. These medicines may be creams or suppositories that you apply in your vagina. Or, they may be pills you take by mouth. It's important to use an antifungal medication for as long as the directions tell you to. Don't stop using them early, even if your symptoms go away. Some of the ways you can help prevent a vaginal yeast infection include only washing your vulva, not inside your vagina, and not using douches or hygiene sprays. Keeping the genital area clean and dry. Not wearing tight underwear or pants. Wearing underwear with a cotton crotch. Always wiping front to back after using the toilet. Cleaning your vulva after sex. And, keeping your blood sugar under control if you have diabetes. Also, take antibiotics for other infections only as prescribed. What is Fluconazole? Fluconazole is used to treat serious fungal or yeast infections, including vaginal candidiasis, oropharyngeal candidiasis (thrush, oral thrush), esophageal candidiasis (candida esophagitis), other candida infections (including urinary tract infections, peritonitis [inflammation of the lining of the stomach], and infections that may occur in different parts of the body), or fungal (cryptococcal) meningitis. This medicine works by killing the fungus or yeast, or preventing its growth.

Thank you for tuning into my video today. Today I’m going to talk about Fluconazole. Fluconazole is a pharmaceutical antifungal drug. It’s also known by the name of Diflucan. Fluconazole is also present in a vaginal cream called Canesten that many women tend to use. I’ve seen many patients over the years who’ve taken Diflucan. Some with reasonably good success, but in most case, the drug has been taken recurrently with very, very poor long-term results.

The best result you’re going to get from any pharmaceutical drug usually is the very first time you take it. The body’s not used to the drug. It will have a powerful effect and give the desired result. But then what happens is the drug is discontinued, so then the symptoms come back, and the drug is used again and again and again. And eventually, the body builds resistance to Diflucan. The resistance is well documented.

I’ve seen some patients who take this drug weekly or monthly for years on end with bad results. And in many cases, they end up with significant side effects, particularly hepatic or liver side effects because Diflucan affects Phase II or the glucoronidation pathway of the liver and it can make a person quite nauseous or sick. They can get skin reactions. They can get headaches. They can get insomnia. They can get a whole range of different side effects from Diflucan.

I’ve got a pharmaceutical drug guide here and I’m going to read out some of the common side effects of Diflucan or fluconazole. Side effects include gastrointestinal upset, headaches, elevated liver function test, abnormal vision, skin odor, skin rashes, hot flashes, weakness, thirst, shaking in the body, polyuria, which means urinating a lot, renal pain or pain in the lower pain, kidney pain, dizziness, seizures, skin itching, sweating, fevers, drug eruptions, more skin rashes, sleeping disturbances and insomnia, nervousness, pain, female sexual dysfunction, menstrual disorders, respiratory disorders, deafness, heartburn, high blood pressure, liver damage. Why the hell would you want to take a drug that creates all these problems just to give you some temporary relief from a vaginal yeast infection or some kind of a yeast infection you might have? There are natural ways you can get on top of these infections without resorting to drugs.

Have a look at yeastinfection.org for some natural solutions. If you’re thinking about an antifungal drug and you want to get some results but don’t want the side effects with it, have a look at the product I developed called Canxida. It’s going to be more powerful than fluconazole. You don’t need to take it for years on end. You’re not going to get all of these side effects that I’ve just read out here. It’s certainly worth a shot.

Think about it. If you’re going to take fluconazole, take it once, but don’t take it recurrently. There’s no point in it. You’re wasting your time doing that. You’re only going to get drug resistance and also liver toxicity from taking it.

Today our video is regarding an oral medicine which is used to treat fungal infection. There are antifungal creams availaible in the market for fungal infection, but some time, creams are not effective so you have to take tablets in order to eliminate the infection
The medicine which we are talking about is Fluconazole, which comes under the brand name of diflucan, which is used to prevent and treat a variety of fungal and yeast infections. It belongs to a class of drugs called azole antifungals. It works by stopping the growth of certain types of fungus.

Fluconazole is used to treat many fungal infections caused by candida including:

Vaginal yeast infections caused by Candida, skin irritation on the head of the penis (balanitis), and fungal infection of the mouth (oral thrush)
infections in your blood or elsewhere in your body
Fluconazole is also used to treat a brain infection called cryptococcal meningitis. This is caused by a fungus called cryptococcus.

Fluconazole is available as capsules or a liquid .
for adults Fluconazole comes in 50mg, 150mg or 200mg. the doctor will prescribe you daily or on weekly basis based on your medical condition.
The liquid usually comes in 2 different strengths:

50mg fluconazole in a 5ml spoonful (50mg/5ml)
200mg fluconazole in a 5ml spoonful (200mg/5ml)

oral (mouth) thrush – 50mg a day, taken for 7 to 14 days
vaginal thrush or balanitis – 150mg, taken as a single dose
vaginal thrush that keeps coming back – 150mg, taken once every 72 hours for the first 3 doses, then take 150mg once a week for 6 months
candida infections (in your blood or elsewhere in your body) – 200mg to 800mg a day for several weeks
cryptococcal meningitis – 200mg to 800mg a day for several weeks
to stop cryptococcal meningitis coming back – 200mg a day, taken long term
to prevent fungal infections if you have a weakened immune system (a low white blood cell count) – 50mg to 400mg a day, until your white blood cell count improves

side effects usually include nausea, diarrhea.

Also remember Fluconazole is usually not recommended in pregnancy

Fluconazole is a triazole antifungal agent.
It is effective against a wide range of fungal infections and is known for its relatively low toxicity and excellent oral bioavailability.
Fluconazole is indicated for the treatment and prophylaxis of various fungal infections, including the following.
Systemic fungal infections, such as cryptococcal meningitis.
Mucosal candidiasis, including oropharyngeal, esophageal, and vaginal candidiasis.
Dermatomycoses, including tinea infections.
Fungal infections in immunocompromised patients, such as those with HIV AIDS or undergoing chemotherapy.
And for prophylaxis of fungal infections in patients undergoing bone marrow transplantation.
Fluconazole works by inhibiting lanosterol 14α-demethylase, an enzyme required for the synthesis of ergosterol, an essential component of fungal cell membranes.
This disruption leads to increased membrane permeability and ultimately fungal cell death. Importantly, fluconazole selectively targets fungal enzymes with minimal effects on human cholesterol synthesis.
Fluconazole is available as oral tablets, capsules, and oral suspensions.
Intravenous route is reserves for severe or systemic infections.
Fluconazole is rapidly and almost completely absorbed from the gastrointestinal tract, with a bioavailability exceeding 90%.
It is widely distributed, including excellent penetration into the cerebrospinal fluid.
Fluconazole undergoes limited hepatic metabolism, up to 10-20% of the dose administered.
It is primarily excreted unchanged in the urine, and minor metabolites are also excreted renally.
Fluconazole has a long elimination half-life of about 20 to 50 hours, allowing once-daily dosing.
Major Adverse Effects of fluconazole include the following.
Gastrointestinal effects such as Nausea, vomiting, abdominal pain, and diarrhea.
Elevated liver enzymes and, rarely, hepatotoxicity or liver failure.
Rash, which can progress to Stevens-Johnson syndrome or toxic epidermal necrolysis in rare cases.
QT prolongation and torsade’s de pointes, especially in patients with other cardiac risk factors.
Rarely, leukopenia or thrombocytopenia.
Contraindications for the use of fluconazole include the following.

Known allergy to fluconazole or other azole antifungals.
Concurrent use of certain medications, especially with drugs that prolong the QT interval such as cisapride and astemizole, due to the risk of life-threatening arrhythmias.
Pregnancy, especially in high doses, because of the teratogenicity associated with fluconazole.
And severe renal or hepatic impairment.
