Introduction to Bacitracin
Bacitracin is a polypeptide antibiotic that is primarily used topically to treat bacterial infections. It is known for its effectiveness against Gram-positive bacteria and is commonly found in over-the-counter antibiotic ointments.

Mechanism of Action
Cell Wall Synthesis Inhibition: Bacitracin works by inhibiting the synthesis of the bacterial cell wall. It interferes with the dephosphorylation of bactoprenol, a lipid carrier molecule that transports peptidoglycan precursors across the bacterial cell membrane. This inhibition prevents the incorporation of these precursors into the growing peptidoglycan chain, leading to a defective cell wall and ultimately causing bacterial cell lysis and death2.

Clinical Uses
Bacitracin is used to treat a variety of infections, including:

Minor skin infections

Superficial wounds

Eye infections (as an ophthalmic ointment)

Side Effects
Common side effects of bacitracin include:

Skin irritation

Allergic reactions (e.g., rash, itching)

Nephrotoxicity (when used systemically, which is rare)

Resistance
Bacterial resistance to bacitracin can occur through various mechanisms, including:

Efflux Pumps: Bacterial efflux pumps that expel the antibiotic from the cell.

Enzymatic Inactivation: Production of enzymes that inactivate bacitracin.

You can find more detailed information on bacitracin and its role as a cell wall synthesis inhibitor here and here.

NEOSPORIN Antibiotic Ointment/Cream | Uses, Directions, & Side Effects | Dr. Eilbra Younan Side Effects 1 Views • 5 months ago

In this video, we’ll walk you through everything you need to know about Neosporin, including what it is, how to use it, the differences between ointment and cream, its availability, and possible side effects:

• What is Neosporin?: Neosporin is a popular antibiotic ointment or cream designed to prevent infections in minor cuts, scrapes, and burns. It contains three active ingredients—bacitracin, neomycin, and polymyxin B—which work together to kill bacteria and speed up healing.

• How to use Neosporin:
1. Clean the wound: Gently wash the affected area with soap and water.
2. Apply a thin layer: Use a small amount of Neosporin and apply a thin layer over the wound.
3. Cover if needed: If the wound is in an area prone to irritation, cover it with a clean bandage.
4. Reapply 1-3 times daily: For best results, reapply Neosporin as directed, usually 1-3 times a day until the wound is fully healed.

• Ointment vs. Cream:
o Neosporin Ointment: Thick and greasy, great for deeper wounds, dry or cracked skin, or wounds that benefit from a moist healing environment, such as post-surgical wounds.
o Neosporin Cream: Lighter, non-greasy, and easily absorbed, ideal for smaller cuts, facial wounds, or areas that need quick absorption like your hands or face.

• Availability: Neosporin is available over-the-counter (OTC) in most drugstores. For more serious wounds, doctors may prescribe a stronger version, which is often covered by insurance.

• Side Effects: Side effects are generally mild but may include skin irritation, redness, or itching. Rare allergic reactions can occur, especially if you're sensitive to one of the active ingredients.
