Mirtazapine is used to treat depression and a few anxiety disorders. It is given at 15 mg dose to be taken in the evening. However, low dose of 7.5 mg mirtazapine is also used to treat insomnia and anxiety.
It can produce sedation due to blockage of histamine receptors. It can also increase appetite leading an increase in body weight. It can be combined with other antidepressants like venlafaxine. Mirtazapine is used in post traumatic stress disorder.

In this video, Dr Sanil Rege discusses in detail the mechanism of action and psychopharmacology of Mirtazapine. In addition, he provides insights in the use of Mirtazapine in clinical practice.
This information is not clinical or medical advice. Please discuss your medical needs with your doctor.
Discussion points:
• Mirtazapine belongs to the antidepressant class NaSSA (Noradrenergic and Specific Serotonergic Antidepressant).
• Mirtazapine works through α2 pre-synaptic antagonism and 5HT2A, 5HT2C, 5HT3 and H1 antagonism.
• Alpha 2 antagonism increases serotonin and noradrenaline release.
• Mirtazapine combined use with SNRIs is termed California rocket fuel due to its significant NA and DA increasing properties.
• Mirtazapine recommended dose range.
• Lower doses have a sedating property due to greater anti-histaminic effects, responsible for weight gaining properties.
• Clinical experience with Mirtazapine, managing side effects and principles of prescribing Mirtazapine.

Overview of the antidepressant Remeron (mirtazepine), which is not only free of sexual dysfunction, but actually quite effectively reverses anorgasmia caused by SSRIs, as well as blocking the nausea caused by those medications (and queasiness in general, from any cause). It is also an excellent, non-habit-forming antihistaminergic sleep aide and can be taken once a day, at bedtime, alone or in combination with other antidepressants, for depression, anxiety, insomnia, nausea, anorgasmia, and anorexia, given that it also greatly stimulates the appetite (usually considered an unwanted side effect). Because it increases both serotonin and norepinephrine by unique mechanisms of action, it combines well with reuptake inhibitors in a synergistic fashion that avoids redundancy in terms of mechanism of action. It is especially favored in the treatment of elderly patients.

Additional notes: Thanks to mirtazapine, I missed out on going to a gig Sunday night! I wasn't well enough in time and it wouldn't have been safe to go out not knowing if I'd doze off in a bad place. And sure, it's not a big deal, there'll be other gigs, but it was one of my goals to get me out of the house more, and I wasn't able to even try to achieve it! I should have waited until today to try this medication, and in future should be smarter about the timing of these things. And that's what's really frustrating about bad side effects, they essentially put my life on pause, including all the practical things I've been trying to do to get my life back on track. Even the extra sleep had no value, because I still woke up with no energy, and actually feeling a lot worse. I'd really rather get no sleep than get _that_ sorta sleep. Also one point where I _miiiight_ just care about weight gain... I have so many nice goth clothes in my current size!! Hahaha I mean I haven't been wearing them anyway, but it _would_ be rather inconvenient if I couldn't fit any of them all of a sudden...! 😅

Mirtazapine is the best antidepressant for sleep and anxiety/depression but that is my opinion.
My Mirtazapine review shows that Mirtazapine has less side effects compared to other antidepressants like amitriptyline and Trazodone used also for sleep and depression, as well as Diazepam or Valium.
Mirtazapine is fast-acting and works within 1 to 2 weeks of starting the antidepressant.
Mirtazapine or Remeron is not a traditional SSRI as it has both the serotonin and histamine receptor. The histamine receptor helps with sleep (insomnia) at 7.5mg and 15mg doses while the serotonin receptor helps with depression and anxiety at higher doses of 30mg and 45mg.
The main side effect of Mirtazapine is weight gain as it can boost your appetite.
Mirtazapine dosages include: Mirtazapine 7.5mg, 15mg, 30mg, and 45 mg doses.

Mirtazapine is a tetracyclic antidepressant in the NaSSA class. It has a similar efficacy to SSRIs and TCAs, and it may be a better option for certain populations.
Some recreational effects like euphoria, CEVs, enhancement of cannabis, and dream enhancement have been reported at certain doses. It's not clear how reliable those effects are.

The video provides an overview of Mirtazapine (Remeron), primarily used as an antidepressant with notable sedative effects, making it useful for insomnia. Compared to SSRIs, Mirtazapine has fewer sexual and gastrointestinal side effects, though it can lead to weight gain. FDA-approved for major depression, it's also used off-label for anxiety disorders, nausea, and appetite stimulation, particularly in cancer patients undergoing chemotherapy. Mechanistically, Mirtazapine acts on histaminergic, serotonergic, and noradrenergic pathways, blocking H1, 5-HT2A, 5-HT2C, 5-HT3, and alpha-2 adrenergic receptors. Its dosing regimen involves low doses primarily as an antihistamine, with increasing serotonergic effects at higher doses, such as Mirtazapine 15 mg and Mirtazapine 30 mg. Optimal doses for depression range from 15 to 30 mg, showing faster onset than SSRIs but may take up to four weeks for maximal effects. Mirtazapine also improves sleep latency and duration but may disrupt sleep at higher doses. Notably, it shows efficacy in reducing methamphetamine use disorder and risky sexual behaviors. Overall, Mirtazapine is considered a first-line antidepressant, especially for patients with insomnia, despite the potential for weight gain and sedation, offering advantages over other antidepressants in certain clinical contexts.

In this video, we review the popular antidepressant, mirtazapine. Mirtazapine is indicated for depression but may also be used in PTSD, OCD and insomnia. Mirtazapine has a unique mechanism of action that gives it an interesting side effect profile - drowsiness and weight gain are common while sexual dysfunction and nausea are rare. A large network meta analysis found that mirtazapine stacks up favourably compared to other antidepressants in terms of efficacy and acceptability. In my opinion, Mirtazapine is best suited for those with depression and insomnia who do not mind gaining a bit of weight.

In today's video I explore the taboo that surrounds taking medication for mental illness. I also talk about my personal experience of taking the anti depressant Mirtazapine (Zispin / SolTab / Remeron) including the early side effects, how and when to take it and how long it took to work. Hopefully it will give you some food for thought and a little insight if you're considering medication for your mental health and might also help us all to question why there is such a taboo surrounding medication for mental illness and help us question.

Welcome to our deep dive into Mirtazapine, a distinctive medication classified as a Noradrenergic and Specific Serotonergic Antagonist (NaSSA). In this video, Dr Sanil Rege, Consultant Psychiatrist, takes you on a journey exploring the fascinating dual effects of Mirtazapine at varying doses, shedding light on its unique action on receptor profiles and how this translates to its efficacy and tolerability in clinical practice.
Mirtazapine stands out for its ability to provide relief from anxiety, depression, and even nausea in certain medical conditions, thanks to its action on a variety of receptors, including the 5HT2A, 5HT2C, and 5HT3, as well as its histaminergic and noradrenergic activities.
At lower doses (15-30 mg), Mirtazapine acts primarily through serotonin-specific antagonism, offering benefits like anti-anxiety effects, improved sleep quality, and no sexual dysfunction, distinguishing it from other antidepressants.
However, its antihistamine properties act as a limiting factor due to weight gain.
As we increase the dose (45-60 mg), we unlock Mirtazapine’s noradrenergic effects, crucial for treating severe forms of depression through the potentiation of noradrenaline and dopamine.
Moreover, we will compare Mirtazapine with Mianserin, highlighting the reasons behind Mirtazapine's preference in clinical settings despite their similarities, including Mianserin's association with agranulocytosis.
This video is your comprehensive guide to understanding Mirtazapine's pharmacodynamics, its clinical applications, and how its receptor-specific actions contribute to its side effects and benefits at different dosages. Whether you're a healthcare professional, a student, or someone interested in the science behind psychiatric medication, join us to unravel the complexities of Mirtazapine and how it's used to combat various psychiatric and physical symptoms.

Dr. Pagnani of Rittenhouse Psychiatric Associates discusses the use of Remeron (Mirtazepine) in insomnia. Remeron is FDA approved for Major Depressive Disorder, and utilizing it for insomnia is off label. With that said, although it has the potential for various side effects, there may be advantages to utilizing this medication as opposed to sedatives that have higher risk for abuse, physiological dependence and withdrawal (like ambien [zolpidem] and lunesta [eszopiclone]). Remeron is unique in that for some individuals it is more effective for insomnia at LOWER doses, as it becomes more adrenergic at higher doses (causing less sedation).
Dr. Pagnani is the founder and medical director of Rittenhouse Psychiatric Associates, an Instructor at The Johns Hopkins Hospital for the Department of Psychiatry, he supervises psychiatric residents at Jefferson Hospital, he serves on the Young Professional Board of Uplift, the Center for Grieving Children and he maintains a private practice of Psychiatry in Philadelphia where he sees patients for one-time psychiatric evaluations and second opinions. He is a licensed physician and surgeon in NJ, PA, NY, MD, DE, FL and CA and can provide consultations to individuals via HIPAA compliant Telehealth visits living in these locations.
Rittenhouse Psychiatric Associates is a private practice of psychiatry consisting of about 25 psychiatrists, psychiatric nurse practitioners and psychotherapists with physical locations in Philadelphia and Paoli, PA and serving all of PA, NJ, NY and FL virtually via HIPAA compliant tele-psychiatry. Individuals seeking not only evaluations but long-term psychiatric care, can contact Rittenhouse Psychiatric Associates directly.
These videos aim to increase awareness and discussion of mental health topics and to encourage struggling individuals to schedule an appointment directly with a mental health provider. Mental health needs to be normalized, and simply speaking about mental health topics openly, is an important first step.
Dr. Pagnani and Rittenhouse Psychiatric Associates are NOT advocating for or against the use of any mentioned psychiatric medications for any specific individual. Additionally, any side effects mentioned are not a complete list of potential side effects or contraindications to taking any mentioned medications, and all risks and benefits of a medication should be discussed with an individual’s provider at an appointment, before starting, stopping or making changes to a medication regimen.

Dr. Pagnani of Rittenhouse Psychiatric Associates discusses Remeron (mirtazepine) an antidepressant that works slightly different than your SSRI's or SNRI's: it antagonizes alpha-2 adrenergic and serotonin 5-HT2 receptors and is rather anti-histaminic. This medication can cause several side effects, but some of these side effects can actually work to address some of the symptoms of depression (for some individuals), by combatting difficulty with sleep and poor appetite or weight loss.
