Corlanor® is an oral medicine for certain people who have persistent (chronic) heart failure caused by the lower-left part of their heart not contracting well. Corlanor® is approved to reduce hospitalization from worsening heart failure for patients who have symptoms of heart failure that are stable, a normal heartbeat with a resting heart rate of at least 70 beats per minute and are also taking beta blockers at the highest dose they can tolerate.

Ivabradine is a heart rate-lowering medication used to treat chronic heart failure and stable angina pectoris. Here's a brief overview of its pharmacology:

Mechanism of Action
Ivabradine selectively inhibits the funny current (If) in the sinoatrial (SA) node of the heart. This current is responsible for regulating the heart rate2. By inhibiting the If current, ivabradine reduces the heart rate without affecting myocardial contractility or ventricular repolarization.

Administration Routes
Ivabradine is taken orally, usually in tablet form.

Pharmacokinetics
Absorption: Ivabradine is well absorbed from the gastrointestinal tract, with a bioavailability of about 40%.

Distribution: It has a volume of distribution of approximately 100 L and is about 70% protein-bound.

Metabolism: Ivabradine is metabolized in the liver by the enzyme CYP3A4.

Excretion: It is excreted mainly through urine (4% as unchanged drug) and feces.

Half-Life: The elimination half-life is about 2 hours, with an effective half-life of around 6 hours.

Indications
Ivabradine is indicated for the treatment of:

Chronic heart failure in patients with a left ventricular ejection fraction (LVEF) of 35% or less, who are in sinus rhythm with a resting heart rate of 70 beats per minute (bpm) or higher, and who are either on maximally tolerated doses of beta-blockers or have a contraindication to beta-blocker use.

Stable angina pectoris in patients who cannot be treated with beta-blockers or in combination with beta-blockers when they are insufficiently controlled.

Adverse Effects
Common side effects include:

Visual disturbances (e.g., phosphenes)

Bradycardia (slow heart rate)

Dizziness

Headache

More serious side effects can include:

Severe bradycardia

Heart block

Ivabradine, an f-channel inhibitor that slows heart rate in patients with heart failure with reduced ejection fraction, significantly reduces the frequency of cardiovascular death or hospitalization for worsening heart failure and improves quality of life

Before you start taking ivabradine, make sure to read the Medication Guide provided by your pharmacist. Take this medication by mouth with food as directed by your doctor, usually twice daily. The dosage is based on your medical condition and response to treatment, and in children, it is also based on their weight. Your doctor may direct you to start at a low dose and gradually increase to reduce the risk of side effects. Other medications are usually used along with ivabradine to treat heart failure, so make sure to follow your doctor's directions for taking all your medications.

If you are using the liquid form of this medication, carefully measure the dose using a special measuring device/spoon and follow the Instructions for Use from the manufacturer. Take this medication regularly at the same times each day to get the most benefit from it. Avoid eating grapefruit or drinking grapefruit juice while using this medication unless your doctor or pharmacist says it is safe to do so, as grapefruit can increase the chance of side effects with this medicine. Remember to tell your doctor if your condition does not improve or if it worsens.
