ABOUT DR. BREWER
Dr. Brewer started as an Emergency Doctor. After seeing too many preventable heart attacks, he went to Johns Hopkins to learn Preventive Medicine. While there, he went on the run the post-graduate training program (residency) in Preventive Medicine. From there, he made a career of practicing and managing preventive medicine and primary care clinics. His later role in this area was Chief Medical Officer for Premise, which has close to 1,000 primary care/prevention clinics. He was also the Chief Medical Officer for MDLIVE, the second largest telemedicine company. More recently, he founded PrevMed, a heart attack, and stroke prevention clinic.

At PrevMed, we focus on heart attack, stroke, and cognitive decline. We serve patients who have already experienced an event as well as those who have not developed a diagnosis or event. In a previous video, we covered the importance of clot prevention in heart attack and stroke prevention. Plavix was the first option to coumadin. It is on the WHO list of Essential Medications. This means it's been found safe, effective and necessary. It costs about $25/month is the US.
The major side effects are the same as all the other anti-clotting or anti-platelet drugs: bleeding or hemorrhage.
There were other issues mentioned but not covered in depth:
relatively higher benefit for smokers;
dipyridamole;
CHAD2VAS2.
Here's the thing, the big issue - less than 1/3 of people needing this medication are actually taking it. Why? Patients blame docs, docs blame Pharma, ... Let's forget blame and get proper protection for people with atrial fibrillation, post-stroke, and other indications.

- What is Clopidogrel ( Plavix 75 mg )?

- Clopidogrel is an antiplatelet agent. The active metabolite of clopidogrel selectively inhibits the binding of adenosine diphosphate (ADP) to its platelet P2Y12 receptor and the subsequent ADP- mediated activation of the glycoprotein GPIIb/IIIa complex, thereby inhibiting platelet aggregation. This action is irreversible.

- It is available as 75 mg tablets.

- Clopidogrel is used to prevent heart attacks and strokes in persons with heart disease (recent heart attack), recent stroke, or blood circulation disease (peripheral vascular disease). It is also used with aspirin to treat new/worsening chest pain (new heart attack, unstable angina) and to keep blood vessels open and prevent blood clots after certain procedures (such as cardiac stent).

- The usual dosage of Clopidogrel
Recent MI, Recent Stroke, or Established Peripheral Arterial Disease: PO 75 mg once daily with or without food. Acute Coronary Syndrome (Unstable Angina/Non-Q-Wave MI): ADULTS: PO Start with a 300 mg loading dose, then continue at 75 mg once daily, initiating and continuing aspirin (75 to 325 mg/day) in combination with clopidogrel.

Clopidogrel is an anti platelet. It is prescribed by your doctor usually after you have suffered an acute coronary syndrome like an unstable angina where you have severe chest pain, heart attack, stroke, or have a condition called peripheral arterial disease where your leg is painful on walking or at rest.

Platelets are blood cells that help blood clot. These are important when you have an injury. However, too much clotting in the blood can block blood vessels in your heart to cause a heart attack and block the blood vessels in your brain to cause a stroke, this is where low-dose aspirin as antiplatelet comes into play.

Dependent on your condition, your doctor may pair up clopidogrel with aspirin. Instead of taking clopidogrel and aspirin separately, to reduce the cost, there are formulations where manufacturers combine both clopidogrel and aspirin into one tablet. If you are being prescribed clopidogrel and aspirin separately, Speak to your doctor about the combination formula

Clopidogrel is a prodrug, which means it has to be converted into its active form by an enzyme in the body called CYP2C19 before it can work. For some people, they are CYP2C19 poor metabolizers, it is harder for their body to convert Clopidogrel to its active form, thus clopidogrel will not be working properly. In this situation, the doctor will change the dosing of clopidogrel or change clopidogrel into something else. Approximately 2% of Caucasians, 4% of African Americans, and 14% of Chinese are CYP2C19 poor metabolizers. Your doctor may perform a test to identify if you are CYP2C19 poor metabolizers.

How much to take
The usual dose of clopidogrel is one 75 mg tablet daily.
If you have acute coronary syndrome, you may receive a single starting dose of 300 mg and then continue at one 75 mg tablet daily.

How to take it
It does not matter if you take this medicine before or after food.
Swallow the tablet with a glass of water. Do not crush or chew the tablets. more in the video

While you are on Clopidogrel
if you are seeing a new doctor or dentist or starting a new medication prescribed by a doctor or dentist, tell them that you are on Clopidogrel. if you plan to have surgery, tell your surgeon and anesthetist, that you are on clopidogrel. more in the video...

Side effects
- abnormal bruising or bleeding

Lifestyle Changes:
Now, if you are put on aspirin, it's a wake-up call, you need to have some lifestyle changes.
What you eat and how much you eat are important. Reducing fat and salt intake. Salt is commonly known as, Sodium the symbol is Na+ increases blood pressure, and many foods in the supermarket contain sodium. this includes your daily bread. So read the labels while you purchasing your groceries, to control how much salt you take in. It's a good idea to remove table salt from your dining table. An adult body only needs around 1 – 2g of salt (460 – 920mg sodium) per day to function. Licorice is also something that increases blood pressure, so avoid it.

If you are a smoker, quit smoking. if you drink too much alcohol, stop it. If you need help stopping these. contact your doctor. Exercises are important as well. Now if you just have a heart attack or stroke, speak to your doctor or cardiac rehabilitation team for advice on types of suitable exercises to begin. Follow your doctor’s advice and slowly build up your activity. and Remember, never hold your breath when lifting weights.

Generic Name: clopidogrel

Trade Name: Plavix

Indication: atherosclerotic events, MI, CVA, PVD, acute coronary syndrome

Action: inhibits platelet aggregation

Therapeutic Class: Antiplatelet agent

Pharmacologic Class: platelet aggregation inhibitors

Nursing Considerations:
 may cause GI bleeding, neutropenia, hypercholesterolemia
 may increase risk for bleeding in warfarin, aspirin, heparin
 can increase risk for bleeding with garlic, ginkgo, ginger
 monitor for signs of bleeding
 monitor bleeding times
 monitor CBC and platelet count
 discontinue use 5-7 days before surgery
