Doxylamine Pharmacology
Doxylamine Pharmacology Side Effects 1 Views • 4 months ago

In the podcast this week, I talk about doxylamine pharmacology. Doxylamine is a first-generation antihistamine; it is commonly an active ingredient in night-time medications like Unisom, Nyquil, and Mucinex. The pharmacology of doxylamine is similar to other first-generation antihistamines, it competitively inhibits the binding of histamine at H1 receptors. Its main uses are as sleep aides, in cough-and-cold medications, but doxylamine has also been given with pyridoxine to treat nausea and vomiting during pregnancy.

Doxylamine’s adverse reactions are related to its anticholinergic properties, they include dry eyes, dry mouth, increased fall risk, sedation, urinary retention, constipation, and confusion. Contraindications include concurrent use with a monoamine oxidase inhibitor, known hypersensitivities, concomitant alcohol use, and if the patient has the following conditions: elevated intraocular pressure, narrow-angle glaucoma, asthma, peptic ulcer disease, urinary bladder neck obstruction, or gastric outlet obstruction. It is also a Beer’s list drug due to its anticholinergic effects. The normal dose in adults is 25 mg. In cases of overdosage, the most common manifestation is exacerbations of its anticholinergic effects. The major complications of an overdose include arrhythmia, respiratory failure, seizures, hyperthermia, rhabdomyolysis, and coma.

When you know a patient is taking doxylamine, it’s important to be cognizant of their occupation, as well as what other conditions they may have. For example, doxylamine should be used with caution in patients that drive heavy machinery due to its sedating properties. You might be able to tell if a patient’s experiencing an adverse reaction exacerbation if they begin having worsening dementia symptoms or increased urinary retention. Other indications include the use of artificial tears, or saliva, or increased complaints of constipation. To monitor for doxylamine, it’s important to monitor the patient’s tolerability. The onset of doxylamine is relatively quick as well, with a peak concentration within 2-4 hours.

For drug-drug interactions, CYP interactions aren’t as concerning as usual. The main interaction to consider when a patient is taking doxylamine is additive anticholinergic effects. Sedative effects can increase when benzodiazepines, skeletal muscle relaxants, opioids, or antihistamines are concurrently taken. Doxylamine can also counteract the usefulness of dementia or BPH medications due to its anticholinergic properties. There is also a risk of increased anticholinergic burden when taken with skeletal muscle relaxants or tricyclic antidepressants.

Pharmacology of Doxylamine ; Mechanism of action, Pharmacokinetics, Uses, Effects
Pharmacology of Doxylamine ; Mechanism of action, Pharmacokinetics, Uses, Effects Side Effects 1 Views • 4 months ago

Doxylamine is a first-generation antihistamine with pronounced sedative properties. It is commonly used to treat insomnia and allergy symptoms, and in combination with pyridoxine (vitamin B6), it is used to treat morning sickness in pregnant women.

Mechanism of Action
Doxylamine works by blocking histamine H1 receptors, which are involved in the body's allergic response. By inhibiting these receptors, it helps reduce symptoms like sneezing, itching, and runny nose1. Additionally, its sedative effects are due to its ability to cross the blood-brain barrier and act on the central nervous system.

Indications
Insomnia: Often used as a short-term sleep aid.

Allergies: Helps alleviate symptoms such as sneezing, itching, and runny nose.

Morning Sickness: Combined with pyridoxine to treat nausea and vomiting during pregnancy.

Administration
Oral: Available in tablets, orally disintegrating tablets (ODT), and syrup.

Over-the-Counter (OTC): Commonly found in nighttime cold and allergy medications (e.g., NyQuil).

Dosage
Insomnia: Typically 25 mg taken 30 minutes before bedtime.

Allergies: Dosage varies based on the severity of symptoms and patient response.

Side Effects
Common: Drowsiness, dizziness, dry mouth, and constipation.

Serious: Potential for QT prolongation and extrapyramidal symptoms (e.g., acute dystonia, akathisia)2.

Precautions
QT Prolongation: Use with caution in patients with a history of QT prolongation or those taking other medications that prolong the QT interval.

Elderly: Increased risk of confusion, dry mouth, constipation, and other anticholinergic effects

Unisom vs Benadryl Review: Differences & Similarities (What's Better?)
Unisom vs Benadryl Review: Differences & Similarities (What's Better?) Side Effects 1 Views • 4 months ago

Unisom SleepTabs: https://amzn.to/2TigyGc
Benadryl UltraTabs: https://amzn.to/2IpEl13

Read our full comparison of the two including side effects, pros, cons, FAQ, etc here:
https://sleepinglucid.com/unis....om-vs-benadryl-diffe

FDA considers Unisom’s active ingredient doxylamine succinate to be safe for pregnant women. It's even sometimes recommended to women experiencing morning sickness symptoms.

Doxylamine succinate is a first-generation antihistamine. That's why it has a central effect on our bodies. This is also the reason why it is used as a sedative agent in Unisom.

Benadryl also has a first-generation antihistamine for its active ingredient diphenhydramine. This drug is mostly used to treat allergies but since it has a strong sedative action, people use it to help with insomnia too. Its relaxing effect is useful in preventing and treating motion sickness.

Effectivity and dosage: adults and teens above 12 years can take one Unisom SleepTabs 30 minutes before their bedtime. However, many users recommend that you start with a smaller dosage and slowly increase it until you find the dosage that fits you.

Also, remember that it's only meant for occasional sleeplessness. You shouldn’t take it for more than 14 days. Otherwise, your body will get accustomed to getting help in falling asleep, making it harder for you to sleep. The effects of doxylamine can last up to 10 or 12 hours. Make sure to only take it when you have ample time to sleep.

You can take up to 50 mg of diphenhydramine to help you to fall sleep. That means you can only take 2 Benadryl UltraTabs max. Only adults and children older than 12 should take Benadryl as a sleep aid. You can expect Benadryl to start to kick in about half an hour after taking it. After that, you’ll be able to feel its effects for 4-6 hours. Just like Unisom, only take Benadryl as a sleep medication occasionally.

Take note of the following precautions when taking Unisom:
Though doxylamine is regarded as safe for pregnant women, the drug can pass through breast milk. It's not recommended that lactating mothers take this sleep aid.
Kids under 12 should not take Unisom.
It's non-habit forming but can cause dependence if taken for a long time

If you’re planning to take Benadryl, here are some things that you need to keep in mind:
Never do anything that requires concentration after taking Benadryl. This includes driving and operating machinery.
Do not take tranquilizers or consume alcohol if you’ve taken this sleep aid.
If you have a history of conditions such as glaucoma, heart disease, diabetes, and asthma, check with your doctor first before taking Benadryl. The same goes if you’re currently taking medication or supplements.

In Conclusion, Unisom and Benadryl are both fast-acting, affordable and very effective sleep aids that can be bought without a prescription.

Their key difference lies in their half-life which is longer for Unisom and that Benadryl is also an allergy medicine.

So, if you’re not sick and just want to get some rest, I suggest trying out the Unisom first.

The active ingredient of the otc sleeping medications is either doxylamine or diphenhydramine. Both doxylamine and diphenhydramine are First Generation antihistamines. These antihistamines can cross the blood-brain barrier and have anticholinergic activity. Therefore, They can cause significant side effects.
The side effects include: Next-day drowsiness, constipation, dizziness, difficulty urinating, blurry vision, confusion, impaired coordination, paradoxical excitation, headache, dry mouth, nose, and/or throat etc.
Alcohol should be avoided when taking these medications because alcohol can aggravate the side effects of the medications. In addition, you should not combine OTC sleep drugs with other medications that contain 1st generation antihistamines such as Benadryl. Long-term use of diphenhydramine or benadryl has been associated with an increased risk of dementia.

The American Academy of Sleep Medicine does not recommend the use of diphenhydramine for insomnia. Similarly, doxylamine is not recommended for insomnia treatment either.
The American Geriatrics Society also advises against the use of these OTC sleep drugs in people 65 and older because of risks such as confusion, constipation, dry mouth, and cognitive decline.
The American Academy of Sleep Medicine does not recommend the use of melatonin, tryptophan or valerian as a treatment for insomnia in adults.

Cognitive behavioral therapy for insomnia provided by a counselor or a psychotherapist is recommended as a first-line treatment for insomnia.
OTC Sleeping medications may be used occasionally as adjunct treatment.
Routine use of otc sleeping medications for more than 2 weeks is not recommended
