In the body, most calcium is located in bones, only about 1% is in the blood and extracellular fluid. The amount of calcium in circulation is regulated by 2 hormones: parathyroid hormone, PTH, and calcitriol. When serum calcium level is low, PTH is UPregulated. PTH acts to PROMOTE calcium release from bones and REDUCE calcium loss from urine. At the same time, it stimulates production of calcitriol, which promotes absorption of calcium in the small intestine while also INcreases RE-absorption in the kidney. Together, they bring UP calcium levels back to normal. The REVERSE happens when calcium level is high. This feedback loop keeps serum calcium concentrations within the normal range.
HYPOcalcemia refers to ABnormally LOW levels of calcium in the blood and is generally defined as serum calcium level LOWER than 2.1 mmol/L. Because the total serum calcium includes albumin-bound and free-ionized calcium, of which only the LATTER is physiologically active, calcium levels must be corrected to account for albumin changes. For example, DEcreased albumin levels, such as in liver diseases, nephrotic syndrome, or malnutrition, produce LOWER serum calcium values but the amount of FREE calcium may STILL be normal. On the other hand, in conditions with high blood pH, albumin binds MORE calcium; leaving LESS FREE-ionized calcium in the serum while the total calcium level may appear normal.
The most common cause of hypocalcemia is PTH deficiency resulting from DEcreased function of the parathyroid glands, or HYPOparathyroidism. HYPOparathyroidism, in turn, may be caused by a variety of diseases and factors. These include:
- accidental removal or damage of the parathyroid glands during a surgery
- autoimmune disorders
- congenital disorders: mutations that set the “normal calcium levels” to a lower value
- other genetic disorders that produce underdeveloped or non-functional parathyroid glands
- magnesium deficiency
Other causes of hypocalcemia include low vitamin D intake/production, and excessive loss of calcium from the circulation such as in kidney diseases, tissue injuries or gastrointestinal diseases.
While chronic moderate hypocalcemia may be Asymptomatic, ACUTE and severe hypocalcemia can be life-threatening. Most symptoms of acute hypocalcemia can be attributed to the effect it has on action potential generation in neurons. Because extracellular calcium INHIBITS sodium channels, and consequently DEpolarization, REDUCED calcium level makes it EASIER for depolarization to occur. Hypocalcemia therefore INCREASES neuronal excitability, causing neuromuscular irritability and muscle spasms. Early symptoms often include numbness and tingling sensations around the mouth, in the fingers and toes. As the disease progresses, muscle spasms may manifest as tetany, wheezing, voice change, and dysphagia. Seizures may occur in severe cases. Effects of hypocalcemia on cardiac function include long QT interval due to prolonged ST fragment, congestive heart failure and hypotension.
Acute hypocalcemia should be treated promptly with intravenous calcium. Chronic hypocalcemia is usually treated with oral calcium and possibly vitamin D supplementation.

A bone fracture is a medical condition in which there is a break in the continuity of the bone.

A bone fracture can be the result of high force impact or stress, or trivial injury as a result of certain medical conditions that weaken the bones, such as osteoporosis, bone cancer, or osteogenesis imperfecta, etc.

The healing process can take weeks, months, or even years depending on the injury.


Food is one factor that can affect how quickly or slowly a broken bone heals.

Nutrition is one of many elements that influence bone strength.

Good nutrition lays a firm foundation for a healthy body and strong bones.

Dietary intake also plays a significant role in protecting the skeleton by maintaining healthy tissues to cushion the force of a fall.

Each stage of the fracture healing process brings with it increased nutritional demands.

For starters, the whole process requires a great deal of energy—which is generally supplied through the intake of calories in food.

Next, healing requires the synthesis of new proteins, which is dependent upon an ample supply of dietary proteins.

Calcium: Calcium is one of the main bone-forming minerals and an appropriate supply to the bone is essential at all stages of life.
Since calcium is the primary mineral in the composition of human bone, it comes as little surprise that it plays the central role in recovery from bone fractures or other bone injuries.
Foods high in calcium include dairy products such as milk, cheese and yoghurt; almonds; green leafy vegetables, pulses, soy products; and cruciferous vegetables, including broccoli, kale, collard and mustard and turnip greens.


Vitamin D: Vitamin D plays an important role in drawing calcium from your blood into the bones. Without adequate doses of vitamin D, dietary calcium can have difficulty finding its way into the bones that need the mineral to heal.

The sunshine vitamin can be acquired through exposure to unfiltered sunlight.

However few people get enough of this vitamin from the sun and need to eat foods rich in D or take supplemental doses.

Food sources of vitamin D include oily fish, such as mackerel, salmon, sardines and tuna; egg yolks; dairy products, including fortified milk.

Increasing your vitamin D also balances your phosphate and calcium ratio in your bones.

A 10 – 30 minutes exposure of sun on a daily basis without sunscreen can help the body to synthesise sufficient amounts of Vitamin D.

Vitamin K: This vitamin plays a key role in strengthening osteocalcin, a protein component of bone, without increasing the mineral density of bone.
This vitamin is abundantly found in green leafy vegetables, Brussels sprouts, broccoli, cauliflower, fish, liver, meat, eggs and cereals.

Vitamin C: A key player in the production of collagen is essential to bone healing. Good sources of Vitamin C include Lemon, Oranges, Mausami, Papaya, Tomato, Guava, Raw amla juice to name a few.

The amount of calcium in circulation is MAINLY regulated by 2 hormones: parathyroid hormone (PTH) and calcitriol. PTH is produced in the parathyroid gland while calcitriol is made in the kidney. When serum calcium level is low, PTH is UP-regulated. PTH acts to PROMOTE calcium release from bones and REDUCE calcium loss from urine. At the same time, it stimulates production of calcitriol, which promotes absorption of calcium in the small intestine while also INcreases RE-absorption in the kidney. Together, they bring UP calcium levels back to normal. The REVERSE happens when calcium level is high. This feedback loop keeps serum calcium concentrations within the normal range.
HYPERcalcemia is generally defined as serum calcium level GREATER than 2.6 mmol/L. Because the total serum calcium includes albumin-bound and free-ionized calcium, of which only the LATTER is physiologically active, calcium levels must be corrected to account for albumin changes. For example, INcreased albumin levels produce HIGHER serum calcium values but the amount of FREE calcium may STILL be normal. On the other hand, in conditions with low blood pH, albumin binds LESS calcium; releasing MORE FREE calcium while the total serum calcium may appear normal.
Most symptoms of hypercalcemia can be attributed to the effect it has on action potential generation in neurons. High levels of extracellular calcium INHIBIT sodium channels, which are essential for DEpolarization. Hypercalcemia therefore REDUCES neuronal excitability, causing confusion, lethargy, muscle weakness and constipation. In most cases, excess calcium in the blood is a direct result of calcium release from bones as they break down, becoming weak and painful. As the kidneys try to get rid of the extra calcium, MORE water is also removed, resulting in dehydration, excessive thirst and kidney stones. Extremely high extracellular calcium may also affect cardiac action potentials, causing arrhythmias. Typical ECG findings include short QT interval, and in severe cases, presence of Osborn waves.
While hypercalcemia may result from a variety of diseases and factors, HYPERparathyroidism and cancers are responsible for about 90% of cases, with the former being by far the most common cause. In HYPERparathyroidism, PTH is OVERproduced due to benign or malignant growths within the parathyroid gland.
An existing cancer elsewhere in the body can cause hypercalcemia in 2 major ways. First, some cancer cells produce a protein similar to PTH, called PTHrP, which acts like PTH to increase serum calcium. Unlike PTH, however, PTHrP is NOT subject to negative feedback; consequently, calcium levels may keep rising unchecked. Second, cancers may spread to bone tissues, causing bone resorption or osteolysis, and subsequent calcium release into the blood.
Hypercalcemia treatment consists of lowering blood calcium levels with a variety of drugs, and addressing the underlying cause. While treatment outcome for hyperparathyroidism is generally excellent, prognosis for malignancy-related hypercalcemia is poor, possibly because it usually occurs in later stages of cancer.

What are all of these things?
They refer to the various forms of Vitamin D in our bodies or in the food we eat. For example, vitamin D3 is also known as cholecalciferol; this is made naturally by the body in response to sunlight. Vitamin D2 is also known as Ergocalciferol and comes from plants. Vitamin D supplements come in the form of either Vitamin D3 or Vitamin D2. If you prefer, calcitriol or 1,25-dihydroxy vitamin D is the active form of vitamin D in our body. When sunlight contains ultraviolet B (UVB) radiation, and when it hits your skin cells, it turns 7-dehydrocholesterol into vitamin D3, meaning cholecalciferol. Vitamin D3, in turn, binds to vitamin D-binding protein (VDBP) and is transported to the liver, where it gets converted to calcidiol, aka 25-hydroxyvitamin D.

Calcidiol is the storage form of vitamin D in the body. Calcidiol is later converted to the active form of vitamin D in the body, meaning calcitriol, or 1,25-dihydroxy vitamin D. This conversion of calcidiol into calcitriol mainly occurs in the kidney. But it also takes place in different tissues and cells of the immune system, such as lymph nodes and alveolar macrophages. And not only in alveolar macrophages but the alveoli themselves.

Vitamin D3 is only found naturally in a few different food sources, mainly fatty fish, like cod, swordfish, tuna, and salmon. Milk doesn't naturally contain vitamin D3, but it has been fortified with it for almost 100 years now. But other dairy products made from milk like cheese and ice cream aren't typically fortified with vitamin D and contain only small amounts. So to meet the RDA of 800 IU, older adults would have to drink about 4 cups of fortified milk per day. Vitamin D3 mainly comes from sunlight, though, not food. But the skin's vitamin D production depends on several factors, only some of which you have control.

Depending on the season, where you live, where you travel, the time of day, the clothing you wear, and the umbrella you use, all of these factors determine how much UVB light hits your skin. The sun's rays are most direct between 10 a.m. and 3 p.m. However, the farther you live from the equator, the less UVB radiation you receive. People who live north of about 37° latitude can't make any vitamin D from sunlight from November to March.

Why is this the case?
During the winter months, the earth tilts away from the sun, ultimately leading to fewer sun rays hitting the earth. Having darker skin means less UVB absorption, which means less vitamin D3 production. Also, as we age, our bodies become less efficient at converting UVB light into vitamin D3. This is why older people and darker skin people are more prone to have lower vitamin D levels. Many older people have reduced exposure to sunlight for different reasons and may not be getting enough vitamin D in their diet to make matters worse.

Vitamin D signals the intestines to absorb calcium into the bloodstream.
This happens even if you have enough calcium in your diet. Besides strengthening bones, vitamin D helps reduce fractures in the elderly by preventing muscle deterioration and reducing the chances of falling.

The official definition of a vitamin deficiency means that specific health problems arise due to not having enough of a particular nutrient. True vitamin D deficiency in children causes rickets, a bone disease where the legs become bowed. This is rare in the United States. Vitamin D deficiency in adults can lead to osteomalacia and osteoporosis, leading to bone fractures. So what if you don't have deficiency per se, but you have less-than-ideal levels of a specific vitamin? Well, this can increase your risk of various health issues, even though they are not solely responsible for these problems. This is what we call "Insufficiency."

Most medical societies consider someone with vitamin D insufficiency if their level is between 12 to 20 ng/mL (30 to 50 nmol/L), and Vitamin D deficiency is less than 12 ng/mL (30 nmol/L). Most experts consider normal levels to be above 20 and less than 50 ng/ml. Most people in the United States have values around 20 ng/ml.

But having too high levels can cause other health issues. In one study, levels above 32 ng/ml resulted in people getting less quality sleep. The risk of vitamin D toxicity typically occurs at levels over 100 ng/mL in adults who also ingest substantial calcium amounts. Vitamin D toxicity, whether it's from vitamin D3 or vitamin D2, generally occurs after inappropriate use of vitamin D. It's essential to avoid excess vitamin D in pregnancy because that can cause calcium levels to rise to the point of causing seizures in the mother and developmental problems in the baby.
