Hydroxychloroquine sulfate is used to prevent and treat acute attacks of malaria. It is also used to treat discoid lupus erythematosus (DLE; a chronic inflammatory condition of the skin) or systemic lupus erythematosus (SLE; a chronic inflammatory condition of the body) and rheumatoid arthritis in patients whose symptoms have not improved with other treatments. Hydroxychloroquine is in a class of drugs called antimalarials. It works by killing the organisms that cause malaria. Hydroxychloroquine may work to treat rheumatoid arthritis and systemic lupus erythematosus by decreasing the activity of the immune system.

Hydroxychloroquine, commercially known as Plaquenil, is a medication used in the treatment & prevention of malaria in areas where malaria remains sensitive to chloroquine. In addition, it is used in the treatment of rheumatoid arthritis. Systemic lupus erythematosus. And porphyria cutanea tarda.
Hydroxychloroquine has different mechanisms of actions according to the disease being treated by the drug. In case of malaria, it is indicated for prophylaxis in geographic areas where chloroquine resistance is not reported. Additionally, it is used in the treatment of uncomplicated malaria caused by plasmodium vivax, plasmodium malariae, plasmodium ovale, and plasmodium falciparum. Here, hydroxychloroquine accumulates within the lysosomes of malaria parasite, raising the P H within. This causes inability to proteolyze human hemoglobin, and consequently, impaired growth & replication of the parasite.
In the treatment of rheumatoid arthritis, hydroxychloroquine acts as a disease modifying anti rheumatic drug. Though the mechanism of action is not well understood here, it reduces the inflammation, pain, and swelling associated with rheumatoid arthritis. It also reduces joint damage and helps prevent disability in the long term. Hydroxychloroquine is the first line drug in the treatment of systemic lupus erythematosus, regardless of the disease severity. Here it acts as an immunomodulator & reduces flare ups and improves survival of the patient.

Your medication Hydroxychloroquine

Hydroxychloroquine is also known by the brand name: Plaquenil.

Hydroxychloroquine comes in 200 mg tablets.

Hydroxychloroquine is most commonly used to treat or prevent malaria, or to treat rheumatoid arthritis. No matter what your doctor has you taking it for, hydroxychloroquine is taken by mouth once daily with food or milk. If taken for malaria prevention, hydroxychloroquine is taken once a week. If taken for treatment of rheumatoid arthritis, it is take once daily. If you miss a dose, take the missed dose as soon as you think about it. If it is close to time for your next dose, skip the missed dose and resume your normal dosing schedule. Do not take 2 doses at the same time or extra doses.

While taking hydroxychloroquine, your skin may become more sensitive to the sun. It is important to wear sunscreen and protective clothing.

Some common side effects with hydroxychloroquine include mood changes, bad dreams, fever, chills, headache, upset stomach, diarrhea, and fatigue.
Some less common side effects include ringing in the ears, seizures, and muscle weakness.

While taking hydroxychloroquine, remember it is important to tell your doctor or pharmacist if:
• You are pregnant or may become pregnant or are breast feeding
• You have a heart condition or diabetes
• You had any eye changes or changes in eyesight due to this drug or drugs like this one
• You are under the age of 12 or over the age of 65
• You are really tired, dizzy, confused, short of breath, or have chest tightness, swelling of the tongue, face, or throat
• You start taking new medications, vitamins, or supplements.

Chloroquine and Hydroxychloroquine (Plaquenil) are antimalarial drugs that help with symptoms of autoimmune disease. Unfortunately, LONG TERM use of these medications can cause retinal toxicity and can lead to permanent vision loss. In this video, I review the testing used to screen for plaquenil retinopathy during an eye exam.

NOTE: The risk of of Plaquenil Toxicity is higher when the cumulative dose is over 1000g not mg.
