Atorvastatin Side Effects


-Dr Myeong-Ki Hong (Yonsei University College of Medicine, KR) outlines the late-breaking results from the LODESTAR trial (NCT02579499).
LODESTAR (Yonsei University) was a randomized, open-label multicenter trial that aimed to compare the clinical efficacy and safety of atorvastatin vs rosuvastatin for the secondary prevention of coronary artery disease. 4400 coronary artery disease patients were enrolled in the trial and were randomized to receive either high-potent statin therapy or LDL-C goal statin therapy.
Previously, no data had compared results in patients treated with atorvastatin to those who had been treated with rosuvastatin. Results suggest that clinical outcomes were similar between both groups of patients. Rates of LDL-C were significantly lower in the rosuvastatin group, but this drug also carried a higher risk of new-onset diabetes compared to atorvastatin treatment.

Atorvastatin (Lipitor) – Side Effects, Uses, Mechanism of Action, Dosage, Interactions, Warnings
Atorvastatin, commonly known under the brand names Lipitor, Caduet, and Atorvaliq, is a statin medication used to lower cholesterol and triglyceride levels, thereby reducing the risk of heart attack and stroke in individuals with type 2 diabetes or coronary heart disease. It works by inhibiting HMG-CoA reductase, an enzyme in the liver responsible for cholesterol production, thus decreasing low-density lipoprotein (LDL) cholesterol and increasing high-density lipoprotein (HDL) cholesterol. Common side effects include muscle and joint pain, nasopharyngitis, nausea, and diarrhea, while serious but rare side effects can include myopathy, rhabdomyolysis, liver enzyme abnormalities, and increased blood sugar levels. Dosages for adults typically range from 10 to 80 mg once daily, with children aged 10-17 years recommended to start at 10 mg once daily, not exceeding 20 mg. Atorvastatin can interact with other cholesterol-lowering medications, certain antibiotics and antifungals, immunosuppressants, and grapefruit juice. Precautions include avoiding use during pregnancy and breastfeeding, in patients with active liver disease, and monitoring for unexplained muscle pain, tenderness, or weakness.

I've tried to boil down one of the most complicated topics in medicine to a YouTube length. Let's hope it worked! Statins are the most prescribed medication in the world but most of what I read and watch online is either completely vague so as to be useless, or just plain wrong.
This is one of the most requested topics I get, and while not the most exciting, it's an important one. So I hope it helps at least one person make a decision.
I wanted to keep it short to ensure people actually watched - an in-depth lecture would of course have offered the chance to really dive into the details but my objective here is to help as many people as possible.
If you find statins confusing, you're not alone! Cardiologists and GPs probably prescribe these more often than anyone else and yet even I took a long time to really understand the enormous mass of data and coverage (which of course is impossible to read in its entirety). I want to specifically credit Professor Darrel Francis from Imperial College in London for helping me see statins in the kind of reference frame that I present in the video. Prior to that, my understanding was that kind of vague "oh this will help you, probably. I think." that you might get if you just read guidelines, which of course exist to maximise health for a whole population.

Generic name: Atorvastatin.
Brand names: Lipitor.
Available as tablets and chewable tablets.
How to use it?
- Tablet: just swallow it whole with half a glass of water.
- Chewable tablet: just take the tablets and chew on it until it dissolves. If you want to you can also swallow this tablet whole.
It is best to take this medicine at fixed times. This will make you less likely to forget a dose.
When to use it?
Atorvastatin is part of a group of drugs called Statins. They inhibit the enzyme HMG-CoA reductase, which normally plays an essential role in the synthesis of cholesterol in the liver. This lowers the circulating LDL, the total cholesterol and increased HDL-cholesterol.
Therefore Atorvastatin can be used to treat a high cholesterol, familial hypercholesterolemia and it can be used to prevent cardiovascular disease.
What is the correct dosage?
The precise dose will depend on your specific indication of use, the side effect you might experience and the severity of your symptoms. Therefore always use the precise amount and frequency your doctor prescribed you.
- However, when treating adults most doctors will start with an initial dose of 10 mg 1x per day. Which if necessary, at intervals, can be increased to max. 80 mg 1x per day.
What are the side-effects?
- Common: Sore throat, nosebleed, allergic reaction, diarrhea, flatulence, nausea, constipation, joint pain, backache, muscle spasms, high blood glucose and headache.
- Uncommon: Fever, Malaise, Edema, Chest pain, Fatigue, Hair loss, Rash, Itching, Hepatitis, Vomiting, Abdominal pain, Belching, Weight gain, Tinnitus, Nightmares, Insomnia, Muscle fatigue, Muscle weakness, Low blood sugar, Dizziness and Taste disorders.
- Rare: it can cause several severe side effects.
Safety:
- First of all it is safe to drive when using atorvastatin.
- However, it rarely can cause liver abnormalities which can be increased with consumption of too much alcohol. Therefore it is often advised to not drink or at most 1 glass a day while using atorvastatin.
- In addition do not drink grapefruit juice, as this enhances the effect of atorvastatin, which can cause side effects.
- Furthermore, atorvastatin can have interactions with other drugs your are taking. always discuss this with your prescribing doctor.
- Lastly, do not use atorvastatin while pregnant or while breastfeeding, as it can be dangerous.
