Ticagrelor Side Effects

Patients with high platelet reactivity on standard-dose clopidogrel in conjunction with a drug-eluting stent for stable coronary disease obtain markedly greater platelet inhibition by switching to prasugrel at 10 mg/day than by doubling the clopidogrel dose to 150 mg/day, according to the Italian RESET trial. #Ticagrelor Can Be Discontinued 2 3 Days Prior to #CABG without Increasing #Bleeding Risk
According to new randomized data, in patients with ACS, performing CABG within a couple of days after ticagrelor cessation is noninferior to waiting 5 or 6 days in terms of bleeding events. Moreover, patients with delayed CABG had more presurgical ischemic events and longer hospital stays.
To prevent bleeding, current US guidelines recommend waiting at least 5 days to perform nonurgent CABG on patients with ACS who had been taking ticagrelor, while European guidelines shorten that period to at least 3 days.

Today we're going to talk about nattokinase and clotting, as well as some other nattokinase benefits.
Nattokinase is an enzyme that comes from nato. Nato is a fermented soybean. Nato is a really good source of vitamin K2. Vitamin K2 is great to help pull calcium build-up out of the arteries. Nattokinase may also be helpful for those with endometriosis, pulmonary embolism, or cirrhosis.
Many people take nattokinase as an alternative to aspirin to help lower the chance of stroke. Nattokinase may help get rid of clots and act as a natural blood thinner.
More potential health benefits of nattokinase for the heart and brain:
• Antithrombotic
• Antihypertensive
• Anticoagulation
• Anti-atherosclerosis
• Neuroprotective

Ticagrelor is a non thienopyridine antiplatelet agent belonging to a class known as cyclopentyl triazolo-pyrimidines. This binds to a site different from ADP, but blocks all the actions of ADP, acting as a reversible allosteric antagonist. It is not a prodrug and does not require activation by the liver cytochrome P450 pathway. It is rapidly absorbed after oral intake with a bioavailability of about one third and a peak plasma concentration at one and a half hours. The main metabolite is also pharmacologically active. Excretion of the drug is mainly by the hepatic route and renal excretion is less than one percent, hence does not require dose adjustment in renal disease.
PLATO trial was double blind phase III trial comparing ticagrelor and clopidogrel, involving over eight hundred and sixty centers in more than forty countries. 18,624 patients with acute coronary syndrome were evaluated and there was no increased overall bleeding risk noted with ticagrelor. But there was an increase in the non coronary artery bypass related bleeding including fatal intracranial bleed. Vascular events were lesser with ticagrelor compared to clopidogrel. The offset of action is faster with ticagrelor as the action is reversible and does not need the generation of new platelets.

Dr. Hsin-Yi Huang, a Post-Doctoral Research Fellow in the School of Pharmacy at the National Taiwan University in Taipei, shares insights from her article appearing in the November 2023 issue of Mayo Clinic Proceedings, where she found that although ticagrelor and clopidogrel have comparable results, ticagrelor has a higher risk of intracranial hemorrhage, and thus clinicians should use caution when prescribing it to acute myocardial infarction patients with a prior history of ischemic stroke. Dr George Dangas (Mount Sinai Hospital, New York, NY, US) discusses the results of the TWILIGHT-Stent trial, presented at ESC 2021. This study examined the incidence of ischemic and bleeding events in patients treated with contemporary drug-eluting stents over 3-15 months. This trial also examined ticagrelor monotherapy vs ticagrelor with aspirin.
Questions:
1. What were the main aims of TWILIGHT-Stent?
2. What are your key findings?
3. What are the next steps?

Stefanie Schuepke presenter of the ISAR REACT 5 trial explains the background and findings of this major trial. ISAR REACT 5 is the first large-sized trial which compared prasugrel and ticagrelor in ACS. The previously presented PRAGUE 18 trial was smaller and ultimately inconclusive. Hence, for the first time, we have evidence comparing the two drugs in a large scale randomized setting.

Additional effective antiplatelet agents in your armamentarium for treating patients with STEMI are always welcome. Before the Prague 18 trial, two of these – prasugrel and ticagrelor – had each been compared separately to clopidogrel, but had never been compared directly to each other. Join Dejan Milasinovic (Serbia) as he reports from ESC Congress 2016 the details of this illuminating trial: What were the primary and secondary endpoints? How did each of these agents perform? What is the importance of the so-called “neutral” results? Learn why the results of Prague 18 offer additional choices for DAPT today…
