Thiazide Diuretics Pharmacology Nursing NCLEX Review (Mechanism of Action & Side Effects)
Thiazide Diuretics Pharmacology Nursing NCLEX Review (Mechanism of Action & Side Effects) Side Effects 1 Views • 4 months ago

Thiazide diuretics pharmacology nursing NCLEX review about the mechanism of action, side effects, nursing implications, and patient education.

Thiazide diuretics work to inhibit the sodium-chloride cotransporter (NCC) in the early part of the distal convoluted tubule. This will decrease sodium reabsorption...meaning less sodium will enter back into the blood. Instead, the sodium will stay in the filtrate. Therefore, this will cause more water to stay in the filtrate and less to be reabsorbed back into the blood…hence, leading to diuresis (increased urination).

Thiazides are used to treat hypertension, heart failure, and renal calculi that are composed of calcium.

Side effects of thiazides include hypokalemia, hyponatremia, hypercalcemia, hyperuricemia, hyperglycemia, metabolic acidosis, and orthostatic hypotension.

Nursing implication for thiazide diuretics are monitoring for signs and symptoms of dehydration (low blood pressure, fast heart rate), strict intake and output measure, daily weights, monitor labs for electrolyte imbalances, avoid digoxin and lithium toxicity etc.

Hydrochlorothiazide (HCTZ) side effects of this commonly used blood pressure medication.
Hydrochlorothiazide (HCTZ) side effects of this commonly used blood pressure medication. Side Effects 0 Views • 4 months ago

Welcome to our educational video by Dr. Lukasz Kiljanek, a Board Certified Nephrologist, Hypertension Specialist, and Internal Medicine physician.

Hydrochlorothiazide (also known as HCTZ, and sold under the registered brand names Microzide and Hydrodiuril) is one of the most commonly prescribed medications (diuretics, water pills) for treating hypertension (high blood pressures) and edema (swelling) associated with heart failure, kidney disease. Here are some common side effects and important information to keep in mind:

Frequent Urination: Hydrochlorothiazide works by causing your kidneys to remove excess fluid from your body, which leads to increased urination. While this can be inconvenient, it is often a desired effect to help get rid of excess fluid in conditions like heart failure or edema.

Dizziness: Dizziness can happen, especially if you stand up too quickly. This is because Hydrochlorothiazide can lower your blood pressure, especially if the dose is too strong, or if you have other problems making you dehydrated, like diarrhea or vomiting. Dehydration can add to its effect and make the dizziness worse.

Electrolyte Imbalance: Hydrochlorothiazide can cause your electrolyte levels (such as potassium and sodium) to drop. Your healthcare provider will likely monitor your electrolyte levels through blood work, usually a few weeks after starting or increasing the dose, to ensure they remain within safe ranges.

Dehydration: Because Hydrochlorothiazide increases urination, it can lead to dehydration. Symptoms of dehydration include dry mouth, excessive thirst, dizziness, and reduced urine output.

Hearing Issues: Although rare, some people might experience ringing in the ears (tinnitus) or hearing loss. If you notice any hearing issues, notify your healthcare provider immediately.

Monitoring Blood Work: It is important to monitor your creatinine, potassium, and sodium levels through blood work, which can range from days to weeks after the initiation of Hydrochlorothiazide or after an increased dose. The timing of these checks will depend on the dose and your personal sensitivity to the medication.

Always notify your healthcare provider if you develop any side effects or have any concerns or questions about your medication. They can provide the best advice and support for your specific situation.
