Dr. Pagnani of Rittenhouse Psychiatric Associates discusses Lamictal (lamotrigine) and indications for its use. Lamictal is a medication that is frequently used in bipolar disorder as a maintenance medication (preventing both manias and depressions for individuals with BPAD).

Dr. Pagnani is the founder and medical director of Rittenhouse Psychiatric Associates, an Instructor at The Johns Hopkins Hospital for the Department of Psychiatry, he supervises psychiatric residents at Jefferson Hospital, he serves on the Young Professional Board of Uplift, the Center for Grieving Children and he maintains a private practice of Psychiatry in Philadelphia where he sees patients for one-time psychiatric evaluations and second opinions. He is a licensed physician and surgeon in NJ, PA, NY, MD, DE, FL and CA and can provide consultations to individuals via HIPAA compliant Telehealth visits living in these locations.

Rittenhouse Psychiatric Associates is a private practice of psychiatry consisting of about 25 psychiatrists, psychiatric nurse practitioners and psychotherapists with physical locations in Philadelphia and Paoli, PA and serving all of PA, NJ, NY and FL virtually via HIPAA compliant tele-psychiatry. Individuals seeking not only evaluations but long-term psychiatric care, can contact Rittenhouse Psychiatric Associates directly.

These videos aim to increase awareness and discussion of mental health topics and to encourage struggling individuals to schedule an appointment directly with a mental health provider. Mental health needs to be normalized, and simply speaking about mental health topics openly, is an important first step.

Dr. Pagnani and Rittenhouse Psychiatric Associates are NOT advocating for or against the use of any mentioned psychiatric medications for any specific individual. Additionally, any side effects mentioned are not a complete list of potential side effects or contraindications to taking any mentioned medications, and all risks and benefits of a medication should be discussed with an individual’s provider at an appointment, before starting, stopping or making changes to a medication regimen.

Can You Stop Your Bipolar Medication? – Maybe Here’s How
Can You Stop Your Bipolar Medication? – Maybe Here’s How Side Effects 0 Views • 2 months ago

Bipolar disorder is a serious mental illness that involves recurring episodes of depression and either hypomania or mania. It normally requires medication to treat the acute episodes as well as keep them from coming back once they resolve. But there are people who go periods of time without medication.

However, if you go too long without meds while you are having an episode, the period of time you spend in those episodes without treatment makes future episodes harder to treat. It’s called the kindling effect.

A therapy that can help bipolar disorder is interpersonal and social rhythm therapy. This therapy has been shown to reduce the time of episodes and delaying the onset of future episodes. This can be useful if you are trying to stay off medication in your interepisode period. Interpersonal therapy focuses on problems in your relationships and social rhythm therapy helps you establish regular daily routines.

Five item Social Rhythm Metric (SRM-5)
You would track times for the following activities
(1) Get out of bed, (2) First contact with another person, (3) Start work, housework or volunteer activities, (4) Have dinner, and (5) Go to bed

Mechanism of Action of lamotrigine:
The precise mechanism(s) by which lamotrigine exerts its anticonvulsant action are unknown. In
animal models designed to detect anticonvulsant activity, lamotrigine was effective in preventing
seizure spread in the maximum electroshock (MES) and pentylenetetrazol (scMet) tests, and
prevented seizures in the visually and electrically evoked after-discharge (EEAD) tests for
antiepileptic activity. Lamotrigine also displayed inhibitory properties in the kindling model in
rats both during kindling development and in the fully kindled state. The relevance of these
models to human epilepsy, however, is not known.
One proposed mechanism of action of lamotrigine, the relevance of which remains to be
established in humans, involves an effect on sodium channels. In vitro pharmacological studies
suggest that lamotrigine inhibits voltage-sensitive sodium channels, thereby stabilizing neuronal
membranes and consequently modulating presynaptic transmitter release of excitatory amino
acids (e.g., glutamate and asp)

Lamictal and Acute Depression
Lamictal and Acute Depression Side Effects 1 Views • 2 months ago

Dr. Pagnani of Rittenhouse Psychiatric Associates discusses lamictal (lamotrigine) and its use in acute bipolar depressions. While this medication can be very effective at preventing bipolar depressive episodes, it can take several months for a patient to achieve an effective dose (due to its risk of Stephen's Johnson Syndrome, a very serious rash) and individuals in an acute depression may benefit from stabilization with another medication, while this is being titrated safely.

Bipolar Disorder - Why One Medication May Not Work
Bipolar Disorder - Why One Medication May Not Work Side Effects 0 Views • 2 months ago

Combination therapy is very common in bipolar disorder because different medications have different purposes. The medication that gets you better, may not be the same medication that keeps you better. Get the Bipolar Medication Handout here https://markspsychiatry.com/bipolar-meds

What are Mixed Episodes? Watch here: https://youtu.be/Nw-1NEwarUg

Typically, people with bipolar disorder require a combination of medications that you take at the same time. Why? Because the medications treat different aspects of the disorder. The main four phases that we target in treating bipolar disorder are Acute depression, Acute mania, mixed states and the maintenance phase.

And acute means it’s the active state that you're in and it's relatively recent. maintenance phase is the in between episodes when things have stabilized.

Not all of the medications work for every phase. So you could be put on a medication because you're manic and then once that mania goes away you can have a depressive episode pop up because that original mania drug isn't doing a good enough job keeping your depression away.

Lamotrigine Medication Information (dosing, side effects, patient counseling)
Lamotrigine Medication Information (dosing, side effects, patient counseling) Side Effects 0 Views • 2 months ago

Your medication Lamotrigine

Lamotrigine is also known by the brand name: Lamictal, Lamictal ODT, and Lamictal XR.

Lamotrigine comes in 25, 100, 150, and 200mg tablets, 5, and 25mg chewable tablets, 25, 50, 100, and 200mg dissolvable tablets, and 25, 50, 100, 200, 250, and 300mg extended release tablets.

Lamotrigine is most commonly used to treat seizures and bipolar problems. No matter what your doctor has you taking it for, lamotrigine is taken by mouth with or without food once or twice a day. Do not take this drug with dairy products, like milk or yogurt, or calcium rich juices. If upset stomach occurs take lamotrigine with food. If you miss a dose, take the missed dose as soon as you think about it. If it is close to time for your next dose, skip the missed dose and resume your normal dosing schedule. Do not take 2 doses at the same time or extra doses. For best results take each dose at the same time each day. Extended-release tablets must be swallowed whole, do not split or break tablets. Use dry hands to take the dissolvable tablet out of the foil and place on top of your tongue. Allow tablet to melt, then swallow.

Some common side effects with lamotrigine include nausea, vomiting, dizziness, drowsiness, tiredness, weakness, and constipation.
Some less common side effects include a very bad skin reaction, headache, abnormal thinking, swelling of the arms or legs, and weight gain or loss.
Do not take lamotrigine if: you have an allergic reaction like a rash, water retention, hives, extensive itching, or mouth ulcers to lamotrigine or any component of the formulation.
While taking lamotrigine, remember it is important to tell your doctor or pharmacist if you have:
• Signs of an allergic reaction (rash, itching, swelling, tightness of the chest, etc.)
• An allergy to lamotrigine.
• A very bad skin reaction, it can cause bad health problems and sometimes death. Get medical attention if you have signs like red, swollen, blistered, or peeling skin with or without fever, red or irritated eye, or sores in your mouth, throat, nose, or eyes. Most cases have happened within 2 to 8 weeks of starting this drug, but some could show up as much as 6 months after.
• This medication should not be abruptly discontinued because of the possibility of increasing seizure frequency.
All forms of lamotrigine need to be stored at room temperature in a dry place, protected from light.

Seizure Risk of Stopping Lamictal (Lamotrigine) and Similar Medications that also Treat Seizures
Seizure Risk of Stopping Lamictal (Lamotrigine) and Similar Medications that also Treat Seizures Side Effects 0 Views • 2 months ago

Dr. Pagnani of Rittenhouse Psychiatric Associates discusses the inherent risks of stopping or quickly decreasing lamictal (lamotrigine), Carbamazepine (Tegretol), Trileptal (oxcarbazepine), Gabapentin (Neurontin), Klonopin (clonazepam) and other medications that are used for the treatment of seizure disorders. Even if the patient is utilizing one of these medications (or a similar medication) for a mood disorder (like bipolar disorder) and they do not have a history of seizures or a seizure disorder, withdrawal CAN result in that individual seizing. This is why individuals need to be under the direct care and supervision of a medical provider when taking these medications, and when considering changing doses or stopping a medication.

What Happens if You Take Lamictal for 10 Years? | Long Term Side Effects and More
What Happens if You Take Lamictal for 10 Years? | Long Term Side Effects and More Side Effects 0 Views • 2 months ago

Taking any medication long term can be scary, and psychiatric medications are no exception. I’ve been on Lamictal (aka Lamotrigine) for ten years, and this is my story. I talk in depth about long term side effects and whether it is still effective after all this time… which is typically what I get asked about when it comes to my treatment since it’s been a decade now.

But Lamictal is amazing and it saved my life, and it’s saved a lot of people’s lives. And while long term side effects can cause one to worry, it’s important to remember that it’s sometimes that specific medication that gave you the ability to get a long term side effect in the first place.

So, put it this way, I’m a very grateful person.

Enjoy :)

Lamictal and Side Effects
Lamictal and Side Effects Side Effects 0 Views • 2 months ago

Dr. Pagnani of Rittenhouse Psychiatric Associates discusses possible side effects of lamictal (lamotrigine). This mood stabilizing medication can be rather effective for preventing both manias and depressions, and unlike many mood stabilizing medications, may have low risk for weight gain and sexual side effects. With that said, it needs to be titrated slowly, as it does carry the risk of Stephen's Johnson Syndrome, Toxic Epidermal Necrolysis and Asceptic Meningitis (all rare, but possible side effects).
