Mechanism of action of ACE inhibitors is inhibition of angiotensin converting enzyme (ACE), which converts angiotensin I into angiotensin II. Angiotensin II increases blood pressure. So its reduced synthesis by ACE inhibitors helps reduce blood pressure in hypertension. Other uses of ACE inhibitors are heart failure/left ventricular failure, myocardial infarction, Prophylactically in patients with high risk of cardiovascular events, diabetic nephropathy and scleroderma renal crisis.
Common side effects of ACE inhibitors are: Hypotension especially after first dose, hyperkalaemia, dry cough, Angioedema, rash, renal failure in patient with bilateral renal arterial stenosis/heart failure, fetopathy, dysgeusia.
Among drug interaction, it should be used cautiously with potassium sparing diuretics due to increases risk of hyperkalaemia and with NSAIDs due to risk of diminishes antihypertensive response of ACE inhibitors.

Captopril, sold under the brand name Capoten among others, is a medication used to treat high blood pressure, heart failure, diabetic kidney disease, and for a short time after a heart attack. For high blood pressure it is one of a number of first line options. It is taken by mouth as a liquid or tablet. It may not work as well in Black people.

Common side effects include difficulty sleeping and stomach ulcers. Other side effects may include decreased appetite, flushing, tiredness, change in taste, high potassium, and Raynaud's, where fingers and toes turn white and blue. Use in pregnancy or breastfeeding may harm the baby. It is an angiotensin-converting enzyme (ACE) inhibitor and works by inhibiting the renin–angiotensin system.

Captopril was patented in 1976 and approved for medical use in 1980. It is available as a generic medication.

Generic Name: captopril
Trade Name: Capoten Indication hypertension, management of CHF, decrease progression of DM neuropathy
Action: block conversion of angiotensin I to angiotensin II, increases renin levels and decreases aldosterone leading to vasodilation
Therapeutic Class: antihypertensives
Pharmacologic Class: ACE Inhibitor
Nursing Considerations:
 can cause neutropenia - check WBCs regularly
 use cautiously with potassium supplements and potassium sparing diuretics
 use cautiously with diuretic therapy
 administer 1 hour before meals
 monitor blood pressure often
 monitor weight and fluid status
 monitor renal profile
 monitor CBC frequently
 may lead to Rhabdomyolysis
 dry cough

- Captopril ( Capoten 25 mg )?

- Captopril is an antihypertensive and vasodilator. It belongs to a category of drugs called angiotensin-converting enzyme inhibitors. It competitively inhibits angiotensin converting enzyme, preventing conversion of angiotensin I to angiotensin II, which is a potent vasoconstrictor that also stimulates aldosterone secretion. Results in decreased BP, potassium retention, and reduced sodium reabsorption.

- It is available as 25 and 50 mg tablets.

- Captopril is used in the management of Hypertension, Accelerated or malignant hypertension. Heart failure, Left ventricular dysfunction after myocardial infarction, Diabetic nephropathy, Hypertensive crisis, and Severe childhood hypertension.
