Video Summary:
ACE inhibitors (Angiotensin converting enzyme inhibitors) are group of drugs that target renin angiotensin aldosterone system, an important bodily mechanism involved in blood pressure regulation.
Drugs in this group are captopril, enalapril, Lisinopril etc.
Mechanism of action of ACE inhibitors is inhibition of angiotensin converting enzyme (ACE), which converts angiotensin I into angiotensin II. Angiotensin II increases blood pressure. So its reduced synthesis by ACE inhibitors helps reduce blood pressure in hypertension. Other uses of ACE inhibitors are heart failure/left ventricular failure, myocardial infarction, Prophylactically in patients with high risk of cardiovascular events, diabetic nephropathy and scleroderma renal crisis.
Common side effects of ACE inhibitors are: Hypotension especially after first dose, hyperkalaemia, dry cough, Angioedema, rash, renal failure in patient with bilateral renal arterial stenosis/heart failure, fetopathy, dysgeusia.
Among drug interaction, it should be used cautiously with potassium sparing diuretics due to increases risk of hyperkalaemia and with NSAIDs due to risk of diminishes antihypertensive response of ACE inhibitors.

Captopril is short acting and patients should not take this medicine on its own due to worsening renal function and hyperkalemia. It can also cause angioedema and cough. Many people take in sublingual/under tongue and it is not recommended because strokes and heart attacks has been reported. Moreover many ACEI and ARBs are now available which are long acting and can be taken once a day.

ACE inhibitors, or Angiotensin-Converting Enzyme inhibitors, are medications commonly used to manage hypertension and heart failure. They are effective but can have various side effects, which can be remembered using the mnemonic "CAPTOPRIL": Cough (a persistent dry cough due to bradykinin and substance P accumulation), Angioedema (swelling that can obstruct the airway), Proteinuria (can worsen existing kidney conditions), Taste changes (metallic or salty taste), Orthostatic hypotension (dizziness upon standing), Potassium elevation (hyperkalemia from reduced aldosterone), Renal failure (particularly in bilateral renal artery stenosis), Itching (pruritus affecting quality of life), and Low blood pressure (especially in sodium or volume-depleted patients). - Captopril is an antihypertensive and vasodilator. It belongs to a category of drugs called angiotensin-converting enzyme inhibitors. It competitively inhibits angiotensin converting enzyme, preventing conversion of angiotensin I to angiotensin II, which is a potent vasoconstrictor that also stimulates aldosterone secretion.
