Videoconsejo sanitario sobre rosuvastatina, para qué sirve este medicamento, cómo y cuándo debemos administrar este principio activo. Videoconsejo impartido por la farmacéutica Paula Mateos, elaborado por el Consejo General de Colegios Farmacéuticos y producido por Medicina TV.

Rosuvastatina es un fármaco perteneciente al grupo de las estatinas, como atorvastatina o simvastatina, destinado a reducir los niveles anormalmente elevados de colesterol en la sangre, que pueden ser responsables de enfermedades como la aterosclerosis o enfermedades cardiovasculares como el infarto de miocardio.

Rosuvastatina disminuye la producción de colesterol en el hígado (la mayor fuente de colesterol del organismo) y aumenta la eliminación del colesterol de la circulación sanguínea por el hígado. Respecto al colesterol LDL y HDL, rosuvastatina disminuye de forma significativa el colesterol LDL (el llamado colesterol malo) y, en la mayoría de los pacientes, eleva el colesterol HDL (el conocido como colesterol bueno).

De forma general, en adultos, los niveles considerados saludables son:
o Colesterol total: menos de 200 mg/dL.
o LDL: menos de 100 mg/dL. En algunos pacientes con riesgo cardiovascular alto, el nivel deseable de LDL puede ser incluso menor de 70 mg/dL.
o HDL: 40 mg/dL o mayor.

Se emplea fundamentalmente para el tratamiento de la hipercolesterolemia, así como también para la prevención de enfermedades como la angina de pecho, el infarto de miocardio o el ictus. También reduce la evolución de la arteriosclerosis.

El tratamiento con rosuvastatina no sustituye a otras medidas importantes de prevención, como mantener una dieta equilibrada y controlar la cantidad de colesterol de los alimentos que ingerimos. Así podemos disminuir tanto la cantidad de colesterol que ingerimos, como la que produce el organismo.

En la farmacia comunitaria, se dispone de presentaciones de rosuvastatina en forma de comprimidos de 5, 10, 15, 20, 30 y 40 mg. La dosis a emplear se debe ajustar de forma individual en base a la respuesta y tolerabilidad al tratamiento.

La dosis inicial en pacientes con hipercolesterolemia leve a moderada habitualmente es de 5 a 10 mg por día. El médico puede ajustarle la dosis hasta un máximo de 40 mg/día.

Para la prevención de enfermedades como la angina de pecho, el infarto de miocardio o el ictus, la dosis empleada es habitualmente de 20 mg por día.

Las reacciones adversas más frecuentes en el tratamiento con rosuvastatina son cansancio, debilidad, mareos y dolor de cabeza. También nauseas, estreñimiento o dolor abdominal.

Rosuvastatina puede ver aumentada su toxicidad por gran número de fármacos, como ciertos antibióticos, fármacos para el SIDA o antifúngicos. Por tanto, es muy importante que evite tomar medicamentos sin recomendación de su médico o farmacéutico, y por supuesto avise a estos profesionales de que está tomando rosuvastatina.

Debido a que rosuvastatina puede provocar daño hepático y toxicidad a nivel del músculo, si presenta náuseas, vómitos o dolor abdominal, coloración amarillenta de piel o del blanco de los ojos, oscurecimiento de la orina, o también dolor, debilidad o calambres musculares (especialmente si se acompañan de fiebre o malestar general), consulte rápidamente a su médico.

Rosuvastatin is a synthetic lipid-lowering medication belonging to the drug class of statins, also known as HMG-CoA reductase inhibitors.
It is one of the most potent statins used to lower cholesterol levels and prevent cardiovascular complications in susceptible individuals.
Major Indications for rosuvastatin therapy include the following.
For hyperlipidemia and mixed dyslipidemia to reduce blood cholesterol and triglyceride levels.
For the primary prevention of cardiovascular disease in high-risk patients.
And for the secondary prevention in patients with established cardiovascular disease, especially following myocardial infarction.
Rosuvastatin works by inhibiting the enzyme HMG-CoA reductase, which catalyzes the rate limiting step in cholesterol synthesis pathway.
Inhibition of this enzyme results in reduced cholesterol synthesis, primarily in hepatocytes.
Increased LDL receptor expression on liver cells, enhancing clearance of circulating LDL cholesterol from the blood.
Reduction of plasma triglycerides and modest increases in HDL cholesterol.
Statins also exert pleiotropic effects, which are unrelated to their major mechanism of action.
Some of these effects include improved endothelial function, stabilization of atherosclerotic plaques, and reduction of vascular inflammation.
Rosuvastatin is taken orally, typically in the evening or at night, because the synthesis of cholesterol is higher at night.
It has a low oral bioavailability and is highly plasma protein bound.
The drug is primarily metabolized in the liver and excreted unchanged in feces.
A small amount is excreted in urine.
Major Adverse Effects of rosuvastatin include the following.
Musculoskeletal effects such as Myalgia, or muscle pain, myopathy, and very rarely, rhabdomyolysis.
Hepatic effects such as Elevated liver enzyme levels, and rarely, acute liver failure.
Gastrointestinal effects such as nausea, abdominal pain, or diarrhea.
Neurological effects such as headache, dizziness, or memory issues.
Hypersensitivity reactions such as rash, pruritus, or angioedema.
It is important to note that statins are usually well-tolerated by most individuals and only a minority of patients experience these adverse effects.
Contraindications for rosuvastatin therapy include the following.
Hypersensitivity to rosuvastatin or its components.
Active liver disease or unexplained persistent elevations in liver transaminases.
Pregnancy and lactation.
Severe renal impairment.
And concomitant use with cyclosporine, gemfibrozil, or other drugs increasing statin toxicity.
