In addition to the huge variation in dose of B12 supplements, there are at least 4 different forms of the vitamin to choose from. The most common of these are cyanocobalamin and methylcobalamin.

But which of these is better? And why do some people argue that you should never take cyanocobalamin and that it is tantamount to poison?

Indeed, why is there poison – cyanide – in cyanocobalamin at all? Should we be concerned that this is an unnatural form of the vitamin?

Today we'll be exploring the different forms of B12 with a special focus on cyano- and methylcobalamin given their ubiquity, including manufacture, safety and conversion issues – and how I arrived at my recommendation for the form you should take.

Please note that the discussion in this video applies to healthy people with no known issues with B12 utilisation, and you should persist with the regimen prescribed by your health care provider.

In a video shared on TikTok, 10X Health System founder Gary Brecka is shown saying, "Have you heard of a Celsius energy drink? Celsius has four times the amount of cyanide that a human being is meant to ingest."
He also says, "These facilities that actually compound this B12, they don’t even pay for the hydrogen cyanide. You know where they get it? Human sewage treatment plants. It is the waste from a human sewage treatment plant. It’s called sludge. It’s foamy yellow sludge. They scrape it off the top, turn it into a powder, ship it to a facility, synthesize it with a metal, and put it in your vitamins."

Are you taking B12 injections and wondering if your cyanocobalamin B12 injection is poisonous? My name is Dr. Terranella, and in this video, we're going to look at different types of B12 and specifically the cyanocobalamin molecule, how it may be poisonous or problematic for you, and also the other side of it, when it's probably not a problem.
So, there are a few different types of B12.

But the most commonly used is cyanocobalamin. So, I wanted to look into this question because many people do use cyanocobalamin and ask if it is poisonous. Cyanocobalamin does contain a small amount of cyanide, and some people get worried when they find that out. And probably they should because cyanide is very toxic. Now, while cyanocobalamin is generally considered very safe, and many people do take it without any adverse problems, it does contain cyanide, which is a toxic substance. It is difficult to estimate how much cyanide is in each injection, but we can do some estimations to get a rough idea of how toxic or poisonous it could actually be.

All right, so the most common dose of B12, whether it's cyanocobalamin or any other cobalamin, is about 1,000 micrograms. Sometimes you'll do that once a week, maybe a little bit more in the beginning, and then as you go on, usually about once a month. So, if you're doing 1,000 micrograms, which is about 1 mL a week, how much cyanide are you actually getting?

First thing to note about cyanocobalamin is that it is a very stable structure. The cyanide molecule is bound to the cobalamin molecule very tightly. So, in order for you to get cyanide, it has to dissociate.

This will happen when that cyanocobalamin is converted into methylcobalamin, hydroxycobalamin, or adensylcobalamin, all of which are more functional forms of the vitamin B12 molecule. So, once that cyanocobalamin dissociates, you still have the cyanide molecule there, which can then attach to a potassium or a sodium molecule.

So, for every 1,000 micrograms of cyanocobalamin, you might yield about 1 microgram of cyanide. This is, again, something you might do once a month or on the high end, once a week. But cyanide is usually toxic in the several milligrams. So, you have a very small amount that you're going to yield from your injection, which is maybe 1 microgram versus several milligrams on the side of an actual toxic or major detrimental effect. If you get up to like 100 milligrams, that's what's going to be lethal.

Some estimates would be a little bit less, but of course it does depend on your size, the administration of it, and other susceptibility factors that you might have.

All this to say that it's actually pretty hard to get a toxic effect from cyanocobalamin. Part of that is because only small amounts are yielded of cyanide for each injection, and it's also a very stable molecule. Now, you might say to yourself, "Well, I don't really care if it's a small amount of poison. I don't want any poison in my body," and that's definitely a fair assessment as well, especially if you're in a more compromised health state. Most of the time when our health is compromised, it's coming from problems within the mitochondria. In particular, with cytochrome C and other parts of the electron transport chain. Well, cyanide is particularly toxic to cytochrome C and the whole electron transport chain.
So if you're having health problems, you're probably having mitochondrial problems, and this is not going to help that situation. In addition, most of us do have the ability to eliminate cyanide in small amounts when there's small amounts at a time, and therefore it gets eliminated before it causes any problems.

But in people that are in a more compromised health state, part of the problem is with toxic overload. detoxification systems are taxed or overrun. The cells, tissues and mitochondria are not able to eliminate even small amounts of toxins like cyanide when they're already overloaded.

So in a case like that, even small amount of cyanocobalamin might be problematic for someone. So now back to our question, Is cyanocobalamin poisonous. Well, I think it depends on what we mean by poisonous. There's certainly no instance where I can think of where cyanocobalamin has an injection to support your overall B vitamin status would actually be lethal or majorly problematic to your health. There are certainly cases in patients where I would think adding something like this could be more compromising to their health because they have problems with their mitochondria or electron transport chain.

Manufactured version of vitamin B12.

It is used to treat and prevent vitamin B12 deficiency anaemia, by mouth, intramuscular injection, or as a nasal spray.
The treatment includes food or supplements to get vitamin B12.
It is less preferred than hydroxocobalamin.
It has an antihypotensive effect.

Erythrocytes require vitamin B12 to be made.
Erythroblasts require folate and vitamin B12 for proliferation during their differentiation. Deficiency of folate or vitamin B12 inhibits purine and thymidylate syntheses, impairs DNA synthesis, and causes erythroblast apoptosis, resulting in anemia from ineffective erythropoiesis.

There are conditions and treatments that can stop absorbing enough vitamin B12 from the food.
Pernicious anemia is a type of vitamin B12 anemia. Intrinsic factor is a glycoprotein produced by the parietal cells (oxyntic cells) located at the body and fundus of the stomach. Intrinsic factor binds to transport and helps absorption of the vitamin B12 by the terminal ileum.
(Causes of pernicious anemia)
• low stomach acid due to atrophic gastritis or certain medicines
• stomach (gastric) surgery
• inflammation in the intestines because of conditions like Crohn's disease and coeliac disease
• infection in the stomach or intestine
• autoimmune condition in which the immune system attacks the intrinsic factor or the parietal cells of the stomach.
• congenital pernicious anemia: Intrinsic factor is not made enough. Rare.

Methylcobalamin vs cyanocobalamin: which one should you take? Both of these are versions of vitamin B12 but differ dramatically in how they are processed by your body and/or utilized. Even though they both contain vitamin B12 they are NOT created equal with one being significantly better than the other.

But first, why would you even worry about using a B12 supplement? Because B12 deficiency is so common! It is one of the most common nutrient deficiencies out there and it leads to symptoms such as weight gain, depression, and fatigue.

Because it's so common, B12 is often added to various supplements to help improve those symptoms. Unfortunately, many manufacturers of B12 supplements use the wrong form of B12 which can cause issues.

Methylcobalamin vs cyanocobalamin: What's the difference?

While both of these contain vitamin B12 the difference between these two is in what the B12 is bound to. In the case of cyanocobalamin, the B12 is bound to Cyanide.

This cyanide must be eliminated and detoxed out of the body.

Even though it doesn't cause damage to your body it's still something that should be considered. Lastly, cyanocobalamin is not naturally found in nature unlike methylcobalamin which means it's synthetic and manmade.

Methylcobalamin, on the other hand, is basically the exact opposite. Methylcobalamin is the form of B12 found in food and occurs naturally. In addition, it doesn't require detoxification of cyanide and comes in its pre-activated and pre-methylated form which means that it is easier for the body to use.

For these reasons, it's ideal to use methylcobalamin.

Is cyanocobalamin harmful because it contains cyanide. Studies have shown that it doesn't cause harm and the amount of Cyanide contained is so small as to not cause toxicity. Having said that, it doesn't sit well with me using a product that contains a potentially deadly product so I prefer to avoid it.

You also should be aware that there are several ways to put B12 into your body. These include: B12 injections, sublingual B12, and oral B12 capsules/tablets.

You may need to play around with what works best for you, but I have found that B12 shots tend to be the best as they bypass absorption in the gut and get straight into your cells.

Whoa, that video was packed to the hilt. The dosage needed to treat B12 deficiency, how oral is better than injectable, why the cheaper form (cyanocobalamin) is superior to the more expensive form (methylcobalamin), why you can’t just take it in a multivitamin because of the analogs, and you shouldn’t just take it in a pill you just swallow (because it needs to mix with that saliva protein), and then with the toothpaste thing totally coming out of left field.
