What is Tacrolimus? What is Tacrolimus used for?

Tacrolimus, known commercially as Prograf®, is a medication that suppresses the immune system. Its primary use is to reduce the body’s natural immune response, helping prevent or manage graft-versus-host disease (GVHD) in patients who have undergone bone marrow transplantation.

Tacrolimus is available in capsule forms of 0.5 mg, 1 mg, and 5 mg, as well as an orally-administered suspension (liquid) prepared by pharmacies. It can also be administered as a clear liquid through intravenous infusion (IV).

Potential Side Effects of Tacrolimus

Initial Side Effects:

Nausea and vomiting
Later Side Effects (occurring usually after one day of treatment):
Reduced kidney function
Diminished liver function
High blood pressure
Headaches
Increased infection risk
Abdominal pain
Insomnia
Tremors
Visual disturbances
Seizures
Low blood magnesium
Elevated blood potassium
High blood sugar levels
These are some common side effects, but others may also occur.

Specific Usage Instructions for Tacrolimus

Tacrolimus should be taken either one hour before or two hours after meals on an empty stomach. If stomach issues arise, it can be taken with food.
Consistency is key. Take it at the same time each day, maintaining a consistent relationship with meal times to ensure stable tacrolimus levels in the body.

Capsules must be swallowed whole without crushing, cutting, or chewing.
Store capsules at room temperature and shake the oral liquid well before each use. Dispose of any remaining liquid after the expiration date.
Keep out of reach of children and pets.
If a dose is missed and there's more than 4 hours until the next dose, take it immediately. If the next dose is due within 4 hours, skip the missed dose and continue as scheduled. Never double dose unless directed.
Mix liquid tacrolimus with orange or apple juice (avoid grapefruit juice) for improved taste, and consume immediately.
Re-administer the dose if vomiting occurs within 20 minutes of taking it.
Drink plenty of fluids to reduce kidney damage risk.
Avoid antacids like Tums® or Mylanta® within 2 hours of taking tacrolimus.
Regular blood tests will monitor potassium, magnesium, kidney, and liver function.
Ensure a consistent supply of the medication, contacting the pharmacy for refills when necessary.
Tacrolimus increases sun sensitivity; wear protective clothing and sunscreen, and avoid direct sun exposure and tanning bulbs.
Some medications can affect tacrolimus levels or side effects. Always inform healthcare providers about all medications being taken.
When receiving tacrolimus via IV, it will be administered over 24 hours in a hospital setting with monitoring by medical staff.
Blood levels of tacrolimus will be regularly checked. Do not take the medication on the day of a blood test until after the sample is collected. Inform your doctor or pharmacist if tacrolimus has been taken before the test.

Generic name: Tacrolimus.
Brand names: Protopic, Advagraf and Prograf.
Available as a capsule, tablet, suspension, injection and ointment.

How to use it?
- Ointment: dry your skin and apply it in a thin layer. Wash your hands thoroughly with water and soap afterwards. Avoid contact with water the firsts 30 minutes after applying it.
- Capsules or tablets: Swallow it completely with half a glass of water. Make sure you don’t break them and you don’t chew on them, because this can decrease the effectivity. Capsules or tablets should also be taken on an empty stomach, 1 hour before a meal or 2 hours after a meal. This improves the effectivity.

Duration of treatment?
The duration of treatment depends on many factors, mostly the indication of use and the severity of this indication. Therefore the duration of use can be weeks, years and in some cases even a life time.

When to use it?
Tacrolimus is an immune suppressive drug.
As an ointment it can be used to treat mild to severe eczema.
As a capsule, tablet, suspension or injection, it can be used to prevent an organ rejection after an organ transplant or to treat several autoimmune bowel diseases.

What is the correct dosage?
Always use the dosage and frequency prescribed by your own doctor.
Ointment starting dose for adults: twice a day for a maximum of 6 weeks.
Ointment maintenance dose for adults: twice a week on affected areas.
Capsules, tablets, injections or suspensions: dosage is based on your blood levels.

What are the side-effects?
Ointment:
- Very common (10%): itching and burning sensation.
- Common (1-10%): sensation of warmth, pain, irritation, rash, itching and local skin infection.

Tablet/Capsule/Injection/Suspension:
Very common (10%): high blood pressure, headache, insomnia, nausea, diarrhea, kidney problems and diabetes.
Common (1-10%): fast heart rate, heart problems, cough, fever, anxiety, confusion, mood swings, vomiting, abdominal pain, rash, acne and sweating.

Safety:
Do not eat grapefruits.
Do not use alcohol.
Avoid sun exposure and apply sunscreen.

Pregnancy and Lactation:
Tacrolimus can be dangerous when pregnant and while breastfeeding. Therefore always discuss this with your prescribing physician.

Tacrólimus es un fármaco inmunosupresor que ejerce su efecto mediante la inhibición de una proteína llamada calcineurina, que promueve la activación de linfocitos T y la producción de citocinas proinflamatorias.

Está autorizado para su uso en la prevención del rechazo tras trasplante de órgano sólido, en concreto de hígado, riñón y corazón; y también para tratar el rechazo agudo resistente a otros inmunosupresores de cualquier órgano sólido trasplantado. Su uso en pacientes trasplantados es el más habitual, pero también se puede usar localmente para atajar los brotes y como terapia de mantenimiento de dermatitis atópica moderada-grave.

En farmacias, tacrólimus está disponible en diversas presentaciones de uso por vía oral [cápsulas normales, cápsulas de liberación prolongada, comprimidos de liberación prolongada y granulado en sobres, con distintas dosis: desde 0,2 a 5 mg por unidad] y en pomada para uso tópico. Incluso una presentación en solución intravenosa u oral está disponible para uso hospitalario. Todas ellas están sujetas a receta médica.

La dosis será individualizada por el médico para cada paciente, según resultados clínicos y tolerabilidad, y variará dependiendo del tipo de órgano trasplantado. Es común que se monitoricen los niveles del fármaco en sangre, con varios ajustes de dosis hasta alcanzar la concentración deseada, que suele ser mayor en los momentos inmediatamente después del trasplante.
• La biodisponibilidad de tacrólimus varía mucho según el medicamento empleado, por lo que es fundamental que sigas estrictamente las recomendaciones de tu médico para evitar que aumente el riesgo de reacciones adversas (incluso de rechazo del órgano).
• No hay límite para la duración del tratamiento, y el uso de tacrólimus junto con otros fármacos inmunosupresores es bastante común. Por ello, hay que tener en cuenta el alto riesgo de interacciones farmacológicas de este fármaco, incluso con inmunosupresores como ciclosporina o micofenolato. También hay que tener cuidado con el uso conjunto de fármacos inductores o inhibidores enzimáticos, por lo que resulta esencial informar al médico sobre cualquier otro medicamento o complemento alimenticio que estés tomando.
• Debes vigilar el perfil de seguridad del fármaco, sabiendo que algunas de las reacciones adversas más frecuentes a tacrólimus son: alteraciones gastrointestinales (náuseas, diarrea y vómitos), toxicidad en el hígado, hipertensión arterial, dolor de cabeza, insomnio, diabetes o fallo renal.
