Tacrolimus (Prograf) is prescribed to help prevent rejection of a transplanted organ after surgery.

Guidelines for use:
Frequency is twice a day, 12 hours apart (most patients).
Never miss a dose, unless otherwise instructed.
Take with or without food.
For blood work, have your blood drawn 12 hours after evening medication and before your morning dose.
Do not cut in half, crush, or open capsules.
As your prescribed dose changes, write down new instructions as they are given to you.

Please contact your transplant nurse coordinator with any questions you may have.
