This week, Neurology News Network covered the FDA approval of sodium oxybate (Xyrem) oral solution to include pediatric narcolepsy patients aged 7 years and older with cataplexy or excessive daytime sleepiness, the findings from a phase 2a clinical trial of ANAVEX 2-73, an investigational therapy for Alzheimer disease, a conversation with Dr. Richard Lipton about ubrogepant, and an interview with Dr. George Koshy Vilanilam to discuss the findings of a study he conducted to assess the trends and identify the risk factors for patients with migraine of developing ischemic stroke.

Chia-Chun Chiang, MD, Mayo Clinic, Rochester, MN, discusses the real-world use of calcitonin gene-related peptide (CGRP) monoclonal antibodies and gepants as preventive and acute treatments for migraine. Four CGRP monoclonal antibodies are currently available for the prevention of migraine, including erenumab, fremanezumab, ocrelizumab, and eptinezumab. More recently, the FDA has approved two gepants for the acute treatment of migraine: ubrogepant and atogepant; and rimegepant approved for both the acute and preventive treatment of migraine. Dr Chiang discusses how the CGRP monoclonal antibodies performed in a real-world setting – stating that many patients with previous unsuccessful treatment trials reported a reduction in frequency and severity of headaches. The efficacy was reported to be similar to that reported in the Phase III clinical trials – inferring success in the real world. However, the real-world studies with the CGRP monoclonal antibodies demonstrated a significantly higher rate of adverse side effects than the clinical trials. In terms of gepants, a real-world study demonstrated similar results to that of the CGRP monoclonal antibodies – whereby gepants had high efficacy levels like in the clinical trials. Yet, a higher percentage of patients also reported adverse side effects in the real-world study. This interview took place at the AAN 2022 Congress in Seattle, WA.

Andrew Blumenfeld, MD, The Neurology Center of Southern California, Carlsbad, CA, discusses a post-hoc analysis of pooled data from the pivotal trials ACHIEVE I and II (NCT02828020, NCT02867709) to determine the efficacy of ubrogepant for acute treatment of migraine based on prior exposure and response to triptans. Dr Blumenfeld highlights that the use of triptans can be limited by insufficient efficacy, poor tolerability, or contraindications. Therefore, gepants such as ubrogepant may represent an effective alternative treatment option. Results from the pooled analysis indicate that failure to respond to triptans did not predict a failure to respond to ubrogepant, suggesting that ubrogepant may be effective and well-tolerated for the acute treatment of migraine in participants with an insufficient response to triptans (due to lack of efficacy or intolerance). This interview took place during the European Academy of Neurology 2021 congress.

Jessica Ailani, MD, FAAN, Medstar Georgetown University Hospital, Washington, DC, discusses the findings of a post-hoc analysis of ubrogepant safety and tolerability across various demographic and clinical subpopulations. Ubrogepant is an oral calcitonin gene–related peptide (CGRP) receptor antagonist, known as a gepant, FDA approved for acute migraine treatment in adults. The pooled population of over 1900 patients from the ACHIEVE I and II trials (NCT02828020; NCT02867709) showed that the safety and tolerability of ubrogepant were consistent across all subgroups. Rates of adverse events were independent of age group, race, gender, BMI, and anxiety/depression medication use. This interview took place during the American Academy of Neurology (AAN) 2021 Annual Meeting.

Jessica Ailani, MD, MedStar Georgetown University Hospital, Georgetown, Washington, DC, discusses the results of the Phase III extension trial (NCT02873221) evaluating ubrogepant for acute migraines. In the extension study, it was found that there were no additional safety concerns when ubrogepant was used repeatedly long term. The ability of ubrogepant to be used at 50 and 100mg as well in combination with triptans as a rescue give the patients more control and flexibility in their treatment. This interview took place during the Virtual World Congress on Controversies in Neurology (CONy) 2020.

With the recent approval of a number of highly effective preventive medications for patients with migraine, whether chronic or episodic, the ability of physicians to treat these individuals has become much easier. However, many challenges persist, and a number of treatments are still sorely needed.

One of the topics of conversation at the 2019 American Headache Society Annual Meeting, July 11-14, in Philadelphia, Pennsylvania, was a major area of unmet need in headache medicine: acute migraine treatments. Although new preventives can greatly reduce the number of migraines patients have, they still have a need for acute therapies for the attacks that they do. Data have shown that many patients fail these acute therapies or experience adverse effects, and perhaps even more fail to remain adherent.

Jessica Ailani, MD, director, MedStar Georgetown Headache Center, and associate professor, neurology, MedStar Georgetown University Hospital, is one of the migraine specialists involved in clinical trials of several potential new agents for this purpose. One of these, ubrogepant, has shown particularly exciting data in this area, and in March, a new drug application for the oral calcitonin gene-related peptide (CGRP) receptor antagonist was accepted for review by the FDA.

Another study found that Ubrelvy was effective in treating migraines across multiple attacks, with many patients experiencing sustained relief of symptoms over time.
These studies and others like them have led to the approval of Ubrelvy by the US Food and Drug Administration (FDA) for the acute treatment of migraines in adults.
