I've had many patients over the years that have suffered with migraines for decades. For those patients willing to make the right changes, almost all of them became headache-free.

Yes, even the new fancy (and expensive) calcitonin gene-related peptide (CGRP) antagonists like Aimovig and Ubrelvy do NOT make blood vessels healthy.
It's not that we don't give migraine meds out despite not fixing the problem. It's just part of the "plan" to help migraine patients (because making lifestyle changes is so hard!).
But are these new migraine meds any better than the old migraine meds like the triptan family of drugs? The new drugs are certainly WAY more expensive (i.e. make more money for the drug companies) and more potential side effects.
But hey--they're shinier and more expensive so they've got to work better, right??

A new type of migraine medication has been found effective to prevent severe migraine. The drug is called ubrogepant and has not yet been approved by authorities. But large scale trials show that it can be considered safe and effective. In the new trial, ubrogepant was tested with two doses: 25 mg and 50 mg. The lower dose had 20.7% of participants feeling pain-free after two hours and 34.1% relieved of the most bothersome symptoms. The higher dose had 21.8% and 38.9% respectively. The study clearly shows the drug is effective. However, further research is required to analyze the long-term safety of these drugs.

This is a quick review of the drug Nurtec ODT by Pfizer pharmaceutical for episodic migraines and prevention of migraines with or with Aura. Very similar drug to Ubrelvy but it is a tablet that dissolves on the tounge. It takes about 2-3 hours to kick in and will help fade pounding migraines away. It may have side effects and is expensive at $1000 for a pack of 8 tabs. This drug inhibits CGRP( Calcitonin gene related peptide) from latching to receptors that may lead to inflammation and pain. Dear colleagues, I'm Hans-Christoph Diener from the Faculty of Medicine at the University of Duisburg-Essen in Germany. In this month's video, I would like to give you an update on what was reported during the International Headache Congress in Seoul in September.
CGRP Receptor Agonists
Let me start with the treatment of acute migraine attacks. Until recently, we had analgesics, nonsteroidal anti-inflammatory drugs like ibuprofen, ergot alkaloids, and triptans. There are new developments, which are small molecules that are antagonists at the calcitonin gene–related peptide (CGRP) receptor. At the moment, we have three of them: rimegepant 75 mg, ubrogepant 50 mg or 100 mg, and zavegepant (a nasal spray) 10 mg.
These are all effective and superior to placebo. The 2-hour pain-free rate is somewhere between 25% and 30%. They have very few side effects; these include a little bit of nausea, somnolence, nasopharyngitis, and for zavegepant, the nasal spray, taste disturbance. In indirect comparisons, the so-called gepants are about as effective as ibuprofen and aspirin, and they seem to be less effective than sumatriptan 100 mg.
Unfortunately, until now, we have no direct comparison with triptans and we have no data demonstrating whether they are effective in people where triptans do not work. The major shortcoming is the cost in the United States. The cost per tablet or nasal spray is somewhere between $80 and $200. This means we definitely need more studies for these gepants.
Migraine Prophylaxis
Let me move to the prophylaxis of migraine with drugs. Previously and still, we have all medications like beta-blockers, flunarizine, topiramate, valproic acid, amitriptyline, and candesartan, and for chronic migraine, onabotulinumtoxinA. We have now 5 years' experience with the monoclonal antibodies against CGRP or the CGRP receptor like eptinezumab, erenumab, fremanezumab, and galcanezumab.
These are all equally effective. They reduce migraine-days between 3 and 7 per month. They are effective both in episodic and chronic migraine, and most importantly, they are effective in people with medication overuse headaches. The 50% responder rates are somewhere between 40% and 60%, and there are no significant differences between the four monoclonal antibodies.
The major advantage is a very good tolerability profile; very few patients terminate treatment because of adverse events. There has been, with one exception, no direct comparison of the monoclonal antibodies with traditional migraine preventive drugs or onabotulinumtoxinA. The only exception is a trial that compared topiramate and erenumab, showing that erenumab was definitely more effective and better tolerated.
At the moment, the recommendation is to use these monoclonal antibodies for 12 months in episodic migraine and 24 months in chronic migraine and then pause. It usually turns out that between 50% and 70% of these patients need to continue the treatment. If they are not working, there is a possibility to switch between the monoclonal antibodies, and the success rate after this is somewhere between 15% and 30%.
Gepants were also developed for the prevention of migraine. Here, we have rimegepant 75 mg every other day or atogepant 60 mg daily. They are effective, but in indirect comparisons, they are less effective than the monoclonal antibodies. At present, we have no comparative trials with monoclonal antibodies or the traditional migraine preventive drugs.
Potential patients are those who have needle phobia or patients who do not respond to monoclonal antibodies. Again, the biggest shortcoming is cost in the United States. The cost per year for migraine prevention or prophylaxis is between $12,000 and $20,000.
Finally, we also had very exciting news. There is a new therapeutic approach via PACAP. PACAP is pituitary adenylate cyclase-activating polypeptide, which has similar biological actions as CGRP but with additional actions. It could very well be that people who do not respond to a monoclonal antibody would respond to a monoclonal antibody against PACAP.
At the congress, the first randomized, placebo-controlled trial with a monoclonal antibody against PACAP was presented. This monoclonal antibody was effective in a population of people in whom prior preventive therapy had failed. A phase 3 study is planned, and most probably the PACAP monoclonal could work in people who do not respond to monoclonal antibodies against CGRP.

In this edition of The Headache Channel, we take a close look at Nurtec ODT / rimegepant, the exciting new oral dissolving tablet for acute treatment of migraine with or without aura in adults.
Nurtec ODT (rimegepant) is the second example of an exciting new class of migraine treatments called the Gepants. (The other currently available Gepant is Ubrelvy / ubrogepant -- playlist here: https://www.youtube.com/playli....st?list=PL3adjIeYuuK )
For years, people with migraines only had 3 basic choices for acute migraine treatment: triptans (like Imitrex / sumatriptan, Maxalt / rizatriptan, etc.), ergots (like Cafergot) and dihydroergotamine / DHE (like D.H.E. 45). These helped a lot of people over the years, but had a lot of shortcomings. They often didn't get rid of all the migraine headache pain. They may have helped pain, but not the other disabling symptoms of migraine like photophobia / light sensitivity, nausea, or phonophobia (sound sensitivity). They may have helped for a couple of hours, but then the pain came back. For a lot of people the side effects of the traditional acute migraine treatments were worse "headaches" than the migraine itself: chest tightness, jaw pain, fatigue, nausea. And traditional treatments are contraindicated / off limits for people who have heart disease, strokes, TIAs, high blood pressure, migraine with brainstem aura or hemiplegic migraine.
Nurtec ODT is an acute migraine treatment that works by blocking CGRP receptors, turning down and turning off the migraine attack.
Nurtec is different from the injectable CGRP-blockers used for migraine prevention (Aimovig / erenumab, Ajovy / fremanezumab, Emgality / galcanezumab, and Vyepti / epitinezumab).
A single dose of Nurtec starts working in 60 minutes. By 2 hours 60% of people have pain relief and 40% of people can return to their daily activities. 86% of people who took Nurtec needed no migraine rescue treatment for 24 hours.
The most common side effect of Nurtec is nausea. Other side effects are possible.
Nurtec ODT is not for women who are pregnant or breastfeeding, and is not for people with very bad kidneys or livers. Nurtec ODT is also not for people who are taking certain medicines that increase or decrease levels of Nurtec ODT in the body. Nurtec ODT is also not for people who are allergic to rimegepant or to any of the inactive ingredients. Nurtec ODT is made with gelatin from fish, so it's not vegan. It's also not specifically halal or kosher.
Recurrent sinus headache without fever is migraine until proven otherwise.

The FDA has approved a new drug for those who suffer from migraines called "Ubrogepant."
On Monday the FDA announced the drug's approval.
It will be sold under the name Ubrelvy by the pharmaceutical company Allergan.
It could be an alternative for those who don't respond to or are unable to take medications now on the market.
Last month a study published that nausea, sleepiness and dry mouth were the most commonly reported side effects.
It's estimated that 40 million people in the U.S. and a billion worldwide suffer from migraines.

Ubrelvy - Unboxing a NEW Acute Migraine Pill - Not a Triptan
In this episode of The Headache Channel, we unbox Ubrelvy, the amazing new migraine medicine from Allergan.
Ubrelvy (ubrogepant) is a life-changing new acute migraine treatment, the first of a new class of drugs: the gepants. Gepants are oral medications whose mechanism of action is to block and inhibit calcitonin gene related peptide (CGRP) and shut down migraine pain, and disabling migraine symptoms of nausea, photophobia (light sensitivity) and phonophobia (sound senstivity).
Ubrelvy can be used by people who have migraine with or without aura, and works even when other treatments (like sumatriptan, rizatriptan, and DHE) have failed. Ubrelvy can be taken by people on common migraine prevention treatments (like topiramate / topamax, amitriptyline, divalproex / depakote, and propranolol), and by people who are using other acute migraine treatments like sumatriptan / Imitrex, rizatriptan (Maxalt), naratriptan (Amerge) or frovatriptan (Frova).
Ubrelvy does not produce medication overuse headache (unlike butalbital / Fioricet / Fiorinal, stadol, hydrocodone, oycodone and codeine) and has no addictive potential.
The side effects of Ubrelvy are few and mild: Nausea, sleepiness and dry mouth. Much more tolerable than the triptans. With Ubrelvy there's no excuse to delay treatment to avoid side effects.
The dose of Ubrelvy is 50 or 100 mg by mouth, as early in the migraine attack as possible. Ubrelvy starts to work in 15 minutes, and 7 in 10 users are PAIN FREE and free of other disabling symptoms within 4 hours. Ubrelvy can be taken again in 2 hours, with a maximum dose of 200 mg a day, 8 days a month, for most users.
Ubrelvy is dispensed in quantities of 10 pills. 10 pills costs $850, but a U-Save Card from the manufacturer (Allergan) reduces the cost for commercially-insured patients to $1 per pill!

Anti-CGRP medications can be an excellent option for the prevention of episodic or chronic migraine, but like all migraine medications, there are pros and cons. I wanted to discuss why I decided NOT to use CGRP antagonists as part of my migriane treatment. This is all just MY interpretation of the data that I found.
This is not nearly all the data or information that there is, so make sure you do your own research and talk to your doctor about whether CGRP Antagonists are right for your migraine treatment. I'm not an expert, just a migraine patient!

Drug Cards Daily | Episode #15: ubrogepant (Ubrelvy) | Treatment for Acute Migraines in Adults
Ubrogepant is currently brand only and known as Ubrelvy. This drug is a Calcitonin Gene-Related Peptide (CGRP) Receptor Antagonist used to treat migraines. There are two tablet strengths being the 50 mg and 100 mg dose. Initiated begins between 50 - 100 mg po for one dose and may be repeated after 2 hours for one more dose. The max dose is 200 mg per day. Dose adjustments should be made in patients with renal and/or hepatic impairment. This drug is primarily metabolized hepatically through the CYP3A4 pathway. Strong inhibitors or inducers for CYP3A4 should be avoided or used with dose adjustments. Ubrelvy’s time to peak is around 1.5 hours with a half-life of 5-7 hours. Patients should be aware of drowsiness, and nausea and should not use drugs or substances that may enhance drowsiness such as alcohol. Ubrelvy can be taken with or without food and for best results not taken with a high-fat meal.

UBRELVY is a prescription medicine used for the acute treatment of migraine attacks with or without aura in adults. UBRELVY is not used to prevent migraine headaches.
*IMPORTANT SAFETY INFORMATION*
Do not take UBRELVY if you are taking medicines known as strong CYP3A4 inhibitors, such as ketoconazole, clarithromycin, or itraconazole, or if you are allergic to UBRELVY or any of its ingredients.
*Before taking UBRELVY, tell your healthcare provider about all your medical conditions, including if you:*
- Have high blood pressure
- Have circulation problems in your fingers and toes
- Have liver problems
- Have kidney problems
- Are pregnant or plan to become pregnant
- Are breastfeeding or plan to breastfeed
*Tell your healthcare provider about all the medicines you take,* including prescription and over-the-counter medicines, vitamins, and herbal supplements. Your healthcare provider can tell you if it is safe to take UBRELVY with other medicines.
UBRELVY may cause serious side effects, including:
- *Allergic reactions:* Most reactions happened within hours after taking UBRELVY and were not serious. Some reactions may occur days after taking UBRELVY. Call your healthcare provider or get emergency help right away if you have swelling of the face, mouth, tongue, or throat or trouble breathing.
- *High blood pressure:* New or worsening of high blood pressure can happen. Contact your healthcare provider if you have an increase in blood pressure.
- *Raynaud’s phenomenon:* A type of circulation problem can worsen or happen. Raynaud’s phenomenon can lead to your fingers or toes feeling numb, cool, or painful, or changing color from pale, to blue, to red. Contact your healthcare provider if these symptoms occur.
*The most common side effects* of UBRELVY are nausea (4%) and sleepiness (3%). These are not all of the possible side effects of UBRELVY.
