I don’t personally suffer from migraines. But I know plenty of people who do.

If you’re one of them … I feel for you. They are debilitating.

And of the things you want to have happen is for them to go away as soon as possible.

For that to happen you need medications.

One of the newer ones we have to treat acute migraines is Ubrelvy.

I’ve talked about Ubrelvy before in another video (
https://youtu.be/_HI6xabcJuQ). But that begs the question … how does it compare to the older acute migraine medications (i.e, the triptans) like Imitrex.

Here’s my take on that question.

The Differences

The most important difference right off the bat is how they act in your body.

Ubrelvy Is what's called a CGRP. This is a fancy way of saying that it can block the inflammation that happens when you get a migraine.

Imitrex, on the other hand, acts by constricting blood vessels in your brain which can help with the migraine symptoms.

Now because Imitrex causes vasoconstriction it can be a concern to use in people who have a history of cardiovascular issues - especially strokes and heart attacks.

So that makes Ubrelvy a good option in that case.

On the other hand Imitrex is an older drug and it’s been around for a long time. That means it's generally cheaper and doctors are going to be a lot more comfortable using it right off the bat.

Dosing

Ironically dosing these medications is very similar.

Both medications should be taken right away when you feel a migraine coming on. Also in both medications you can repeat the dose in 2 hours if you're not getting relief. But after that you're essentially maxed out and can't take anymore.

An interesting side note - Ubrelvy and Imitrex can be taken together. Not at the same time but they can be taken in succession.

What I mean by that is if you took a dose of Imitrex and the migraine didn't go away instead of taking another dose of Imitrex in 2 hours - you could take Ubrelvy at the two hour mark instead.

I don't think this would ever be a common occurrence. But it is something you should know about.

Side Effects

I want to be really careful here. Because when I say this people are going to take it as a rule and that's not what I mean.

But generally speaking triptans like Imitrex tend to carry a few more side effects.

Again this is a generality but over the years I found this to be the case.

Bottom line is that in most cases your doctor is probably going to start you on Imitrex unless you have reasons that you can't take it (like a stroke). But if you're in the box where Imitrex isn't a good option Ubrelvy is probably going to be your next best option.

What Is Ubrelvy Used For?

Ubrelvy is a new oral medication used to treat migraine attacks. There are other drugs in the same drug class as Ubrelvy but they are all injectables and are designed to prevent migraines - not treat attacks.

This new class of drug is called CGRP blockers. We know that when you get a migraine one of the things that can happen is you can get inflammation and pain and we know a substance called CGRP plays a role in that. These drugs block CGRP which can lower or prevent that inflammation and pain.

This is different than the other major class of drugs that are used to treat acute migraines, the “triptans” (for example, sumatriptan which also goes by the brand name Imitrex). They act by constricting blood vessels in the brain.

Dosage


Ubrelvy comes in 50 and 100 mg doses. It's designed to be taken 50 to 100 mg at the onset of a migraine.



If you still have symptoms from your migraine in 2 hours you can take another dose. The normal Max dose per day is 200 mg.

However, if you have any liver or kidney disease your doctor is going to need to make sure to drop your dose down. This means you won't be able to take more than 100mg a day it's a Max dose. Again your doctor is going to have to draw your blood labs to see if that's a concern for you.


Side Effects


Overall Ubrelvy is a drug that's very well tolerated and doesn't have a lot of side effects.

Most common side effects are nausea and drowsiness with nausea occurring in about three to four percent of people. And drowsiness happening in about two to three percent of people in studies.


How Long Does It Take To Work?



Of course one of the biggest questions when you take you probably is how long will it take to start getting rid of migraine.

That's going to differ for each person but it takes one and a half hours for your blood levels to reach their maximum. So for sure you'll see something in that time frame. But it could be as little as 15 minutes to more like half hour to an hour before you really notice a difference


Contraindications?



Like I mentioned above Ubrelvy is pretty safe but there are certain people where it's not a good choice.


If you have any sort of serious kidney disease you're going to have a tough time clearing the drug from your system. So it may not be the best choice.


Again, your doctor will measure your labs to see if you fall in that category.


Drug Interactions



While Ubrelvy is not known for having drug interactions you do need to be very careful with what are called CYP3A4 inhibitors.

Basically CYP3A4 is a liver enzyme. And it helps break drugs down and gets them eliminated from your system. But certain drugs can actually block this enzyme.

When they're strong blockers of the things that can happen is you'll get a buildup of Ubrelvy in your system. So these combinations should be avoided. The main groups of drugs that are strong Inhibitors of this enzyme are: macrolide antibiotics (i.e., erythromycin), 'azole' antifungals (i.e., fluconazole) and HIV meds.

Ubrelvy / ubrogepant for Migraine: What YOU Need to Know

Ubrelvy (ubrogepant) is a life-changing new acute migraine treatment, the first of a new class of drugs: the gepants. Gepants are oral medications whose mechanism of action is to block and inhibit calcitonin gene related peptide (CGRP) and shut down migraine pain, and disabling migraine symptoms of nausea, photophobia (light sensitivity) and phonophobia (sound senstivity).

Ubrelvy can be used by people who have migraine with or without aura, and works even when other treatments (like sumatriptan, rizatriptan, and DHE) have failed. Ubrelvy can be taken by people on common migraine prevention treatments (like topiramate / topamax, amitriptyline, divalproex / depakote, and propranolol), and by people who are using other acute migraine treatments like sumatriptan / Imitrex, rizatriptan (Maxalt), naratriptan (Amerge) or frovatriptan (Frova).
Ubrelvy does not produce medication overuse headache (unlike butalbital / Fioricet / Fiorinal, stadol, hydrocodone, oycodone and codeine) and has no addictive potential.

The side effects of Ubrelvy are few and mild: Nausea, sleepiness and dry mouth. Much more tolerable than the triptans. With Ubrelvy there's no excuse to delay treatment to avoid side effects.

The dose of Ubrelvy is 50 or 100 mg by mouth, as early in the migraine attack as possible. Ubrelvy starts to work in 15 minutes, and 7 in 10 users are PAIN FREE and free of other disabling symptoms within 4 hours. Ubrelvy can be taken again in 2 hours, with a maximum dose of 200 mg a day, 8 days a month, for most users.

Ubrelvy is dispensed in quantities of 10 pills. 10 pills costs $850, but a U-Save Card from the manufacturer (Allergan) reduces the cost for commercially-insured patients to $1 per pill!
