▪ Metronidazole antibiotic is basically an antiprotozoal and anti-amebic drug. Metronidazole is a bactericidal antibiotic. This video will help you to understand pharmacology, mechanism of action, side effects and clinical uses of metronidazole. Metronidazole is the drug of choice for the treatment of pseudo membranous colitis caused by the an aerobic, gram-positive bacillus Clostridium difficile and is also effective in the treatment of brain abscesses caused by these organisms.

▪ Mechanism of Action of Metronidazole:
Metronidazole is selectively toxic to anaerobic micro organisms. Metronidazole is a prodrug that is activated by reduction of the nitro group by susceptible organisms. anaerobic and micro aerophilic pathogens contain electron transport components, that have a sufficiently negative redox potential to donate electrons to metronidazole. After entering the cell by diffusion, the nitro group of metronidazole, is reduced by certain redox proteins, operative only in an aerobic microbes to highly reactive nitro radical which exerts cytotoxicity. The nitro group of metronidazole is able to serve as an electron acceptor, forming reduced cytotoxic compounds that bind to proteins and DNA, resulting in cell death.

▪ Pharmacokinetics:
Metronidazole is completely and rapidly absorbed after oral administration. Metronidazole distributes well throughout body tissues and fluids. Less than 20% of the drug is bound to plasma proteins. With the exception of the placenta, metronidazole penetrates well into body tissues and fluids, including vaginal secretions, seminal fluid, saliva, and breast milk. Therapeutic concentrations also are achieved in CSF. It is metabolized in liver primarily by oxidation and glucuronide conjugation. And excreted in urine, with Plasma half life of about 8 hrs.

▪ Side Effects of Metronidazole:
The most common adverse effects are those associated with the gastrointestinal tract. including nausea, vomiting, epigastric distress, and abdominal cramps. An unpleasant, metallic taste is often experienced. Other effects include oral moniliasis which is basically a yeast infection of mouth. And, rarely cause, neuro-toxicologic problems, such as dizziness, vertigo, seizures and peripheral neuropathy.

▪ Metronidazole is primarily indicated in conditions like:
* Amoebiasis (Drug of First Choice)
* Giardiasis
* Trichomonas vaginitis (Drug of Choice)
* Metronidazole is the drug of choice for the treatment of pseudomembranous colitis.
* Metronidazole also is used in patients with Crohn’s disease, and Helicobacter pylori infection.

Metronidazole is an antibiotic.

It's used to treat skin infections, rosacea and mouth infections, including infected gums and dental abscesses. It's also used to treat conditions such as bacterial vaginosis and pelvic inflammatory disease.

It's also used for infected insect bites, skin ulcers, bed sores and wounds, and to treat and prevent bacterial and parasitic infections.

Metronidazole is only available on prescription.

It comes as tablets, gel, vaginal gel, cream, a liquid you drink or a suppository which is a medicine that you push gently into your anus (bottom). It's also given by injection, but this is usually only done in hospital.

Key facts
The most common side effects of metronidazole tablets, liquid, suppositories or vaginal gel are feeling sick (nausea) or being sick (vomiting), diarrhoea, and a slight metallic taste in your mouth.
It's unusual to have any side effects from metronidazole cream or gel.
Do not drink alcohol while taking a course of metronidazole tablets, liquid, suppositories or vaginal gel, or for 2 days after finishing treatment. Alcohol can cause side effects such as feeling and being sick, stomach pain, hot flushes, difficulty breathing, a pounding heartbeat (palpitations) and headaches.
For most infections, you'll start to feel better in a few days, but for some it may take longer. When treating rosacea, you may only notice a difference after several weeks.
It's important to take metronidazole as your doctor tells you, and finish your full course of treatment.

Generic name: Metronidazole.
Brand names: Flagyl and Metrogel.
Available as: an ovule, oral suspension, tablet or injection

How to use it?
Tablets: Swallow the tablet whole with half a glass of water. Don’t break it and don’t chew on it.
Oral suspension: shake the bottle before use and measure the correct amount with the supplied measuring spoon/cup.
Ovule: needs to be inserted deep into the vagina.

Duration of treatment?
Usually 1-10 days.
It is important that you do not interrupt the treatment and finish it completely.

When to use it?
Metronidazole is used for ”serious” infections caused by micro-organisms sensitive to Metronidazole.

What is the correct dosage?
Always use the dosage and frequency prescribed by your own doctor.

What are the side-effects?
Common: (1-10%): Increased sensitivity for additional infections.
Uncommon (0.1-1%): general body weakness.
Rare (0.01-0.1%): severe allergic reaction and convulsions.

Safety:
You should not drive while using Metronidazole and you should not combine it with alcohol.
Furthermore, the usage of Metronidazole can have interactions with some other drugs and it can be dangerous for people with liver and neurological problems. Therefore always ask your prescribing physician if metronidazole is safe for you.

Lesson on the antibiotic Metronidazole (Flagyl). Metronidazole is a small sized antibiotic, which allows it to be easily absorbed and diffusely distributed throughout the body. This makes it particularly important in bone and brain infections. Metronidazole has important activity against obligate anaerobes and some protozoal species, but has limited effects on aerobes. Metronidazole’s mechanism of action (which is described in the video) allows it to be an effective bacteriocidal antibiotic. Metronidazole has a wide variety of adverse effects, including GI side effects and specific side effects when alcohol is consumed when taking it.

Commonly known as Flagyl, metronidazole is an antibiotic effective against anaerobic bacteria and certain parasites.

What is Metronidazole (Flagyl) Used For?
Metronidazole is used for treating bacterial vaginosis.
To treat Infections caused by parasites such as Giardia in the small intestine, amebic liver abscess, and amebic dysentery, a colon infection that results in bloody diarrhea.
It is also used in treating infection of the colon caused by a bacterium called Clostridium difficile.
The topical gel of metronidazole is used to treat acne rosacea, as also known as adult acne. It reduces redness and inflammation and the number of pimples.
H. pylori, a bacterium that can cause stomach or intestinal ulcers, is also treated with metronidazole in combination with other medications.
Metronidazole is used to treat anaerobic bacteria-related abscesses in the liver, pelvis, abdomen, and brain.

What are the Side Effects of Metronidazole (Flagyl)?
Nausea, vomiting, loss of appetite and stomach pain.
Sometimes diarrhea or constipation can result.
Headache, dizziness and weakness could result from taking the medication.
Depression, trouble sleeping or feeling irritable.
Rash and itching;
Unpleasant metallic taste.
Swollen, red, or "hairy" tongue.
Mouth sores.
Vaginal itching or discharge and pain during sex.
Dark-colored urine.

Serious Side Effects of Metronidazole (Flagyl):
Signs of an allergic reaction to Flagyl like hives, itching, warmth or tingling; fever, joint pain, dry mouth, stuffy nose or difficult breathing.
Worsening symptoms of infection.
Confusion.
Seizure.
Fever, neck stiffness, and increased sensitivity to light.
Numbness, tingling, or burning pain in your hands or feet.
And Muscle weakness, problems with speech or coordination.

Precautions Before Using Metronidazole (Flagyl):
Avoid metronidazole if you had alcohol or disulfiram (Antabuse) within the last 2 weeks.
Avoid drinking alcohol when taking metronidazole and for at least 3 days after stopping it.
Avoid Flagyl if you are allergic to metronidazole, secnidazole, or tinidazole.
Avoid metronidazole in the first trimester of pregnancy – harmful to the baby.
Inform your doctor of any blood, renal, liver, or Crohn's disease you currently have or have ever had.
Metronidazole cannot treat flu or the common cold.
Yeast infections in the vagina cannot be treated with metronidazole.
Do not share this medicine with another person, even if they have the same symptoms, you have.

In this video Doctor O'Donovan explains key facts you need to know about Metronidazole - an antibiotic tablet that has many uses - including for rosacea.

What is Metronidazole?

Metronidazole is an antibiotic. You might know it by its brand name - Flagyl. Here in the UK, metronidazole is only available on prescription. It comes as tablets, gel, vaginal gel, cream, a liquid you drink or a suppository which is a medicine that you push gently into your anus (bottom). It's also given by injection, but this is usually only done in hospital.

Metronidazole uses:

It's used to treat skin infections, rosacea and mouth infections, including infected gums and dental abscesses. It's also used to treat conditions such as bacterial vaginosis and pelvic inflammatory disease. It's also used for infected insect bites, skin ulcers, bed sores and wounds, and to treat and prevent bacterial and parasitic infections.

What are the strengths and doses of metronidazole?

This will vary from person to person so always follow the specific instructions given to you by your own doctor or pharmacist.

Can I drink alcohol if I take metronidazole?

According to NHS UK "Do not drink alcohol while taking a course of metronidazole tablets, liquid, suppositories or vaginal gel, or for 2 days after finishing treatment. Alcohol can cause side effects such as feeling and being sick, stomach pain, hot flushes, difficulty breathing, a pounding heartbeat (palpitations) and headaches."

Are there any side effects of metronidazole?

In this video we cover some of the common, plus more rare (but serious) side effects to be aware of. It is always really important to read the information leaflet inside the medication packet before taking any medication. Serious side effects happen in less than 1 in 1000 people but they can include:

- the whites of your eyes turn yellow, or your skin turns yellow although this may be less obvious on brown or black skin – these can be signs of liver or gallbladder problems
- you get unexpected infections, mouth ulcers, bruising, bleeding gums, or extreme tiredness – these can be caused by a blood problem
- you have bad stomach pains which may reach through to your back – this can be a sign of pancreatitis
- you have blurred vision or double vision
- anaphylaxis

In this video I discuss some of the side effects associated with a commonly prescribed antibiotic called metronidazole, also known as Flagyl.

In many, these adverse effects resolve with discontinuation of the drug. However, some people are left with longstanding illness which continues despite no longer taking this medication.

Research shows that metronidazole can readily convert into a thiamine (vitamin B1) analog which inhibits the activation of this vitamin inside the cell. Most of the symptoms of metronidazole toxicity are identical to neurological beri-beri and Wernicke's Encephalopathy, both conditions which are caused by deficiency of thiamine. Some researchers have concluded that this drug may "reinforce" or unmask an underlying insufficiency, triggering extreme symptom changes.

Many people report improvement in toxicity symptoms through using bioavailble forms of thiamine in conjunction with other B vitamins and magnesium supplementation.
