Clozapine represents a breakthrough anti-psychotic medicine. Clozapine was introduced to the United States market in 1990 and is on the World Health Organization of essential medicines. Not only does Clozapine treat schizophrenia and related conditions, but it appears more effective than the standard anti-psychotic therapies. Clozapine also improves resistant schizophrenia and may additionally reduce the risk of suicide in these individuals.

Due to safety concerns that originated in Finland, the use of Clozapine has decreased. Current practitioners lack familiarity with this drug, and as a result of some negative publicity seem disinclined to prescribe it.

The two most worrisome issues involve a decline, sometimes serious, in the number of bacteria fighting neutrophils – a type of white blood cell. This occurs in about 3% of exposed individuals most commonly in the first 4 months of therapy. During this phase of Clozapine therapy, most recommend weekly laboratory tests.

Perhaps even more troublesome is a slowing of movement of muscular activity of gastrointestinal tract. This may lead to constipation, fecal impaction and bowel obstruction. Pre-emptive therapy with laxatives minimizes this issue. Other adverse drug reactions may occur.

In spite of the side effect profile, a recent study ranked Clozapine as the most effective drug in its class. As a generic, the financial burden it poses is minimal and more akin to the generic versions of Abilify, Zyprexa and Risperdal.

Generic name: Clozapine.
Brand names: Clozaril and Leponex.
Available as a tablet and a drink.

How to use it?
Swallow the tablet completely with half a glass of water.
When using it as a drink: measure the correct amount you need to take with a measuring spoon or measuring cup. And drink it.

Duration of treatment:
The duration of treatment depends on the indication of use and the severity of your symptoms. If your symptoms improve with the use of Clozapine you usually have to keep using it for 12-24 more months. And sometimes multiple years.

When to use it?
Clozapine is part of a group of drugs called antipsychotics. It can be used to treat several mental health conditions:
- Treatment-resistant schizophrenia.
- Treatment-resistant psychosis.
- Psychotic disorders in Parkinson's disease.
- Delirium in Parkinson's or 'Lewy body' dementia.

What is the correct dosage?
Unfortunately I can’t provide you with guidelines for the correct dosages, as these differ a lot depending on the specific indication, the side effects you experience and the severity of your complaints.
Usually your doctor starts with a low dose and quickly builds it up over a few days. Afterwards your maintenance dose will be adapted throughout your treatment based on the blood levels of Clozapine you will be having.
Therefore always use the prescribed dose and frequency your doctor gave you.

What are the side-effects?
Very common (10%): somnolence, dizziness, constipation and a fast heartbeat.
Common (1-10%): changes in your blood values, dry mouth, low blood pressure, high blood pressure, stomach and intestine complaints, weight gain, headache, tremor, stiffness, fatigue, fever and sweating.
Uncommon (0.1-1%): stuttering and falling.

Safety:
You should be careful to combine Clozapine with alcohol. As this might increase the dazing effect of Clozapine.
In addition it is usually safe to drive while using this drug, however it can cause you to become drowsy, sleepy and/or dizzy. If you experiencing any of these side effects then please do not drive.
Furthermore, smoking cigarettes decreases the effectivity of clozapine. Always discuss this with your prescribing doctor.
Lastly, Clozapine can have interactions with several drugs. Therefore always discuss this with your treating physician.

Pregnancy and Lactation:
Clozapine can be dangerous in both cases, therefore always discuss this with your doctor.

Careful Clozaril withdrawal can minimize the potential for rebound psychosis and cholinergic crises. Clozaril belongs to a class of drugs called SGA’s or second-generation antipsychotics.
Used in the treatment of certain mental conditions such as schizophrenia and psychosis, Clozaril is often prescribed only AFTER other antipsychotics did not work or caused severe side effects.
Alternative to Meds has been the expert on antipsychotic withdrawal and Clozaril alternatives for over 15 years. We have published evidence regarding our success. Underlying issues can in many cases be addressed in much less toxic ways. It is possible that some medical conditions could have been overlooked. Commonly some or all of the original contributing factors have improved or changed, and the diagnosis one was given may no longer lead to recovery.


What Is Clozaril (clozapine) Used For?


Clozaril is used in the treatment of certain mental disorders, where other (usually at least two) other antipsychotic medications have not worked, or where the side effects were too harsh and could not be tolerated by the patient. A candidate for a prescription of Clozaril could be someone diagnosed with conditions such as:

Treatment-resistant schizophrenia.
Treatment-resistant schizoaffective disorder with a known risk for suicide.
Please note that elderly patients with dementia-related psychosis should NOT be prescribed Clozaril (or other antipsychotic medications) due to the increased risk of death.


Discontinuing/Quitting Clozaril (clozapine)
There are circumstances where immediate withdrawal of Clozaril is medically needed as a life-saving intervention. These include potentially life-threatening adverse reactions to the drug which are delineated in detail under the adverse effects section, for instance, cardiovascular compromise, risk of stroke, signs of tardive dyskinesia, and other severe reactions.

In the majority of cases, gradual Clozaril tapering methods are recommended for coming off the drug.

Clozapine is the most effective of all antipsychotic medications. And for many people with schizophrenia, clozapine is a game-changing treatment that makes it possible to go back to school, go back to work, live independently, or be in relationships.

It does have side effects (all medicines do), but many of them can be prevented or minimized.

Sedation is one of the most common of clozapine’s side effects, and this lecture talks possible causes and potential treatments for clozapine-associated sedation.

Here, we look at the prevalence of clozapine-associated sedation, discuss pharmacological actions of clozapine that may contribute to sedation, and survey some treatment strategies to reduce or eliminate this side effect.

Clozapine is the most effective medication for schizophrenia. It is the only medication with FDA approval for reducing suicide risk. About 30% of people with schizophrenia do not get significant benefit from standard antipsychotic medication -- yet in that subset of individuals, clozapine is successful in about 50% of cases (the success rate of other antipsychotic medications is about 7% to 14%).

This lecture discusses:
1. Why do some people with schizophrenia-like symptoms not respond to standard, first-line antipsychotic medications?
2. What are the unique advantages of clozapine?
3. What are the most common or worrisome clozapine side effects, and how can their risks be minimized?
4. Are there any alternatives to clozapine?
