Auvelity a new antidepressant that works like ketamine. This antidepressant is a combination of dextromethorphan and bupropion. The dextromethorphan component is a cough suppressant that came to the market in 1954. The dextromethorphan was discovered to affect Glutamate by inhibiting the NMDA receptor. The NMDA receptor is inhibitory and blocking it causes increased Glutamate. This mechanism of action is similar to ketamine and esketamine also known as Spravato. It increases glutamate thus increasing excitatory neurotransmitter in the prefrontal cortex bringing about a antidepressant quality. The bupropion otherwise known as Wellbutrin or Zyban is used at 105 mg which is a sub therapeutic dose but at this dose, it inhibits the metabolism of the dextromethorphan, allowing the dextromethorphan to be at higher levels for a longer period of time. So bupropion or Wellbutrin or Zyban is being used to block the metabolism of the dextromethorphan. This is an interesting mechanism is the first time a medicine is being used that can be taken orally and increase glutamate for the treatment of depression.
