Acyclovir (ACV), also called aciclovir, is an antiviral drug primarily used to treat the herpes simplex virus (HSV) infection, chickenpox, and shingles.

It works by killing the growth and spread of the herpes virus. Acyclovir is available as buccal tablets, capsules, and suspension. It also comes in the form of cream and ointments to apply over the skin.

Importantly, it is a prescription drug and may also be used for other virus infections, as determined by your doctor.

- In this video we'll briefly discuss what is Acyclovir, how it works, what it's used for, the dose and side effects, contraindications and precautions.

- Acyclovir is an antiviral active against herpes simplex virus and varicella zoster virus. It inhibits viral DNA replication by interfering with viral DNA polymerase.

- It is available as 200 mg, 400 mg and 800 mg tablets, and as 40 mg/ml oral suspension. Also available for topical use and injection.

- Acyclovir uses:

- Treatment of recurrent or extensive oral and oesophageal herpes in immunocompromised patients.
- Treatment of herpetic kerato-uveitis.
- Treatment of genital herpes.
- Secondary prophylaxis of herpes in patients with frequent and/or severe recurrences.
- Treatment of severe forms of zoster: necrotic or extensive forms, facial or ophthalmic zoster.

Dosage of Acyclovir:

- Treatment of recurrent or extensive oral and oesophageal herpes in immunocompromised patients, treatment of herpetic kerato-uveitis
Child under 2 years: 200 mg 5 times per day for 7 days
Child over 2 years and adult: 400 mg 5 times per day for 7 days
- Treatment of genital herpes
Child over 2 years and adult: 400 mg 3 times per day for 7 days; in immunocompromised patients, continue treatment until clinical resolution
- Secondary prophylaxis of herpes in patients with frequent and/or severe recurrences
Child under 2 years: 200 mg 2 times per day
Child over 2 years and adult: 400 mg 2 times per day
- Treatment of severe forms of zoster
Adult: 800 mg 5 times per day for 7 days

And for topical use, for both adults and children: Apply to lesions every 3 hr, 6 times/day.

- Contraindications:

- Do not administer to patients with hypersensitivity to acyclovir.

- Acyclovir side effects:

- Acyclovir ( Zovirax ) may cause: headache, skin rash, allergic reactions, gastrointestinal disturbances, raised transaminases, neurologic disorders in patients with renal impairment and elderly patients; rarely, haematological disorders.: Reactions range from mild to life threatening.

- Precautions:

- Reduce dosage in patients with renal impairment.
- Drink a lot of liquid during treatment.
- Genital herpes: Sexual intercourse must be avoided when lesions are present. Use of acyclovir does not prevent transmission.
- Pregnancy: no contra-indication
- Breast-feeding: no contra-indication

Valtrex or the generic valacyclovir treats infection due to herpes simplex type 1 and type 2 which translates to cold sores, fever blisters and genital herpes. It also offers benefits for the skin rash of shingles or herpes zoster. Timing is critically important in both conditions.

Beginning therapy for cold sores or genital herpes during the prodromal symptoms – itching, burning, tingling, numbness – prior to appearance of the lesion offers maximal benefit. Delaying onset until a day or so after the eruption guarantees lack of effect. Similarly for shingles, the window of opportunity requires instituting therapy within 72 hours of onset. Additionally Valtrex does not appear to alter the course of post-herpetic neuralgia, the lingering pain that persists for months after an attack of shingles.

Valacyclovir must be converted in the body to its active form. Removal of the amino acid valine leaves acyclovir to perform the antiviral function. Blood levels of acyclovir from valacyclovir average several times greater than those resulting from taking Zovirax or acyclovir itself.

Dosing for recurrent herpes simplex often involves one 500 mg pill twice daily for 3 days. An alternative regimen is 1000 mg immediately and again at 12 hours. Either seem acceptable. With shingles a higher dose of 1000 mg three times daily for 10 days seems optimal.

For those with frequent recurrences of herpes simplex, a suppressive dose generally reduces the frequency of attacks. A daily dose of 500 or 1000 mg for 6 months often allows a reset. Discontinuing the drug may demonstrate a long symptom free interval. If necessary the valacyclovir may be resumed with the knowledge that with age the frequency of attacks diminishes.

Unfortunately valacyclovir only reduces the frequency of clinical disease. It neither eliminates the virus from its resting place in the nerve nor does it guarantee freedom from attacks while on therapy. Reduces, yes; eliminates, no. The same applies to asymptomatic shedding of the virus and the likelihood of passage to an uninfected partner. Reduces, yes; eliminates, no.

Valacyclovir rarely causes any adverse reaction and even seems safe for pregnant women where the benefits outweigh the negligible risks. Resistance of the virus to the drug appears clinically irrelevant. Valacyclovir should not be present in the circulation when a person receives the chickenpox vaccine.

What is Acyclovir?
Acyclovir (or Aciclovir) is a human-made purine nucleoside analog which suppresses the activity of all two types of herpes simplex virus (HSV-1 and 2), and varicella-zoster virus (VZV). The primary purpose is to inhibit the multiplication of the virus. It decreases the production of the virus's DNA.

How does it work?
Its affinity for thymidine kinase (TK) encoded by HSV and VZV, determines the high selectivity of acyclovir. With the help of viral TK, it is transformed into a nucleotide analog; acyclovir monophosphate which is then converted by host cell kinases to acyclovir triphosphate. ACP-TP prevents further viral DNA synthesis without affecting the natural cellular processes. There are three primary mechanisms of suppression: integration into DNA chain and termination of it, competitive suppression, and deactivation of the DNA polymerase. A high affinity towards the viral enzyme, thymidine kinase (TK) by Acyclovir makes the drug more effective on herpes simplex.

Acyclovir, a synthetic nucleoside analogue belongs to a class of medications called antiviral drugs.

Acyclovir works by stopping the growth of virus in the body and reducing the symptoms of the infection; however it cannot cure the disease.

Acyclovir is prescribed against herpes simplex virus and varicella virus which include herpes zoster infections or shingles, chickenpox or varicella, and genital herpes.

It is used to relieve the pain and discomfort, and helps in speeding up the healing process of blisters or sores caused by the viral infections. Acyclovir can be prescribed in combination with hydrocortisone 1% to treat topically or for external use.

Introduction: Acyclovir, an antiviral agent primarily targeting Herpes Simplex Virus (HSV) infections, presents predominantly mild adverse effects such as malaise (10%), gastrointestinal symptoms, and elevated liver function tests (1-10%). However, more severe complications, notably associated with intravenous administration, encompass rare occurrences of acute renal failure, neurological toxicity, and sporadic cases of bradycardia. A review of literature revealed 2 instances linking acyclovir to bradycardia, with no documented reports on junctional rhythm. A phase IV clinical study examining FDA data identified 1,424 individuals encountering side effects from Acyclovir, with only 6 individuals (0.42%) reporting bradycardia, primarily observed in males aged 60 and above using the medication for less than a month. Here we report a rare case of junctional rhythm caused by acyclovir.
