Each year millions of Americans visit their doctor for a prescription to treat AFib. For years now, the most commonly used anticoagulant has been difficult for many to swallow. Now, a new line of drugs is proving simpler to use than Coumadin or Warfarin.

“Their advantage is a reduction in the likelihood of interaction with other medications, a reduction in the likelihood of interaction with certain foods or beverages that could be problematic with Warfarin or Coumadin, and importantly, the lack of need for regular blood test to monitor the control of the anticoagulant,” says Lee Memorial Health System cardiologist, Dr. Richard Chazal.
Studies find that overall, next gen drugs including the brand names Xarelto and Pradaxa were slightly more effective.
“The data thus far with the newer agents compared with Coumadin has generally shown a reduction in risk of stroke, a reduction in risk of death, a reduction in risk of bleeding into the brain,” says Dr. Chazal.
These new drugs are considered more patient-friendly. But there are still things to consider: they are more expensive, may increase the risk of stomach bleeding and at this point there are few options to reverse their effects if needed.

However, their half life in the blood stream is only about 24 hours, so they can wear off fairly rapidly in the event of bleeding,” says Dr. Chazal.
In the coming years, more people are expected to need treatment; and drugs manufacturers are keeping pace.

“There’s a lot of new developments in this area that are very promising,” says Dr. Chazal.

Discussion on conversion to and from dabigatran to other anticoagulants.
Conversion to and from dabigatran to other anticoagulants depends on the period of activity of the drug currently being used.
Initiate dabigatran 2 hours or more prior to the time of the next scheduled dose of the parenteral anticoagulant like heparin or low molecular weight heparin. If it is from an infusion of heparin that you are trying to convert, initiate the drug at the time of discontinuation of heparin infusion.
If the creatinine clearance is 30ml/min or more, wait 12 hours after the last dose of dabigatran for initiating parenteral anticoagulant.
When dabigatran is used for thromboprophylaxis after orthopaedic surgery, it can be started 24 hours after the last dose of dabigatran, instead of the 12 hours when it is for prevention of embolism in atrial fibrillation.
If the creatinine clearance is less than 30 ml/min, you have to wait for one day instead of the usual 12 hours.
Discontinue warfarin and initiate dabigatran when INR is less than 2.
When to start warfarin while discontinuing dabigatran is depending on the creatinine clearance. If the creatinine clearance is more than 50 ml/min, warfarin can be started 3 days before discontinuation of the drug.
If it is between 31 and 50 ml/min, warfarin is initiated two days before discontinuation of the drug. When the creatinine clearance is between 15 ml/min and 30 ml/min, initiate warfarin one day prior to discontinuation of the drug.
Please note that the effect warfarin on the INR will be better reflected after dabigatran has been discontinued for two or more days.

A blood thinner is a drug which is prescribed to a patient to help prevent blood clots. One type of this drug is Pradaxa, manufactured by Boehringer Ingelheim Pharmaceuticals, Incorporated. The drug helps those with atrial fibrillation, which is an irregular heartbeat.

There are a few other blood thinners designed for this use, including the prescription drugs Coumadin, Jantoven and Marfarin. These are more traditional type blood thinners, while Pradaxa was touted as being safer for the patient and less reactive to certain types of food. However, with the other drugs, if a bleed occurred as a side effect it was more manageable and able to be stopped. With Pradaxa, there is no real effective antidote and the risk can become very serious and lead to internal and cranial bleed events.

Because the drug has a half-life in the body's system that can last up to 12 hours, it is difficult to stop the bleeding and more drastic measures need to be taken. Dialysis seems to be the only option after a patient suffers significant bleed. The process takes several hours and is used to remove the drug from the patient's system. Patients who already suffer renal and kidney issues may have an even harder time removing the drug from their system, putting them at an even greater risk.
