Labetalol functions by obstructing the adrenergic nervous system's receptors. This system sends nerves from the spinal cord to arteries, releasing norepinephrine, which binds to receptors on the arteries, causing them to constrict. This constriction leads to increased blood pressure. However, when labetalol binds to these receptors, it causes the arteries to dilate, thereby reducing blood pressure.

We'll also look at the common side effects associated with labetalol, which include fatigue, dizziness, nausea, headache, diarrhea, edema (fluid retention), and shortness of breath. It's important to note that labetalol can sometimes cause postural hypotension, a condition characterized by a sudden drop in blood pressure upon standing, potentially leading to fainting or light-headedness. This effect is especially worth monitoring within a few hours of the first dose or after any dosage adjustments.

Other significant side effects can include sexual dysfunction, abnormal heart rhythms, slow heart rate, liver failure, and allergic reactions. Regarding dosage, the typical starting oral dose is 100 mg twice daily, which can be adjusted based on blood pressure response. Maintenance doses generally range between 200-400 mg twice daily, with some severe hypertension cases requiring up to 2.4 g daily. Side effects may be minimized when the medication is divided into three daily doses. It's crucial that any changes or cessation of the medication be done under a physician's guidance to avoid serious complications.

For intravenous administration, an initial dose of 20 mg over 2 minutes is standard, with subsequent doses of 40 or 80 mg every 10 minutes as necessary, up to a maximum of 300 mg. Alternatively, labetalol can be given via intravenous infusion at a rate of 1-2 mg/minute.

Furthermore, labetalol has interactions with various drugs. It can mask the symptoms of hypoglycemia, especially tremors and an increased heart rate, posing a challenge for diabetic patients who use insulin or oral antidiabetic medications. When combined with tricyclic antidepressants, such as amitriptyline, imipramine, or nortriptyline, labetalol may increase tremors, a common side effect of these antidepressants. If used with adrenergic stimulating drugs for asthma, like albuterol or pirbuterol, labetalol's blocking effects might reduce the effectiveness of these asthma treatments. Drugs like glutethimide and cimetidine may affect labetalol's effectiveness by altering its elimination or increasing its blood levels. Additionally, halothane anesthesia can enhance labetalol's blood pressure-lowering effects.

As for its safety during pregnancy and breastfeeding, labetalol's effects have not been adequately studied in pregnant women, and it is known to be excreted in breast milk. Therefore, caution is advised for nursing mothers. Lastly, labetalol is a prescription medication, and it should be stored at temperatures ranging from 20º Celsius to 30º Celsius (68º Fahrenheit to 86º Fahrenheit).
