Meningitis (infection of the membranes surrounding the brain and spinal cord) and skin and abdominal (stomach area) infections brought on by bacteria are treated in adults and children 3 months of age and older with meropenem injections. Meropenem injection belongs to the category of drugs known as antibiotics. It functions by eradicating the infection-causing germs.

Colds, the flu, and other viral diseases cannot be treated with antibiotics like meropenem injection. Antibiotic use that is not necessary raises the likelihood of developing a later infection that is resistant to antibiotic treatment.

Meropenem is a broad-spectrum carbapenem antibiotic used to treat a variety of bacterial infections. Here are some key points about its pharmacology:

Mechanism of Action
Meropenem inhibits bacterial cell wall synthesis by binding to penicillin-binding proteins (PBPs), which inhibits the final transpeptidation step of peptidoglycan synthesis. This leads to bacterial cell lysis due to ongoing activity of cell wall autolytic enzymes2.

Spectrum of Activity
Meropenem is effective against a wide range of Gram-positive and Gram-negative bacteria, including Staphylococcus aureus (methicillin-susceptible isolates), Streptococcus pyogenes, Streptococcus agalactiae, viridans group streptococci, Enterococcus faecalis (vancomycin-susceptible isolates), Pseudomonas aeruginosa, Escherichia coli, Proteus mirabilis, Bacteroides fragilis, and Peptostreptococcus species.

Pharmacokinetics
Absorption: Administered intravenously or intramuscularly.

Distribution: Penetrates into most tissues and body fluids, including the urinary tract, peritoneal fluid, bone, bile, lung, bronchial mucosa, muscle tissue, and heart valves.

Metabolism: Not extensively metabolized; primarily excreted unchanged by the kidneys.

Elimination: The elimination half-life is approximately 1 hour in adults, but this can vary depending on renal function.

Protein Binding: Approximately 2%.

Dosage and Administration
Adults: Typically, the dose is 1 gram IV every 8 hours, not to exceed 2 grams IV every 8 hours.

Children: Dosage is adjusted based on body weight and renal function.

Neonates and Infants: Dosage is also adjusted based on body weight and renal function.

Side Effects
Common side effects include gastrointestinal disturbances (nausea, vomiting, diarrhea), allergic reactions, and injection site reactions. Rarely, it can cause nephrotoxicity and ototoxicity1.

Drug Interactions
Meropenem can interact with other nephrotoxic drugs, increasing the risk of kidney damage. It should be used cautiously with medications that affect renal function1.

Indications
Meropenem is indicated for the treatment of various infections caused by susceptible bacteria, including complicated skin and skin structure infections, intra-abdominal infections, and bacterial meningitis

MEROPENEM is a carbapenem antibiotic. It treats some infections caused by bacteria. It will not work for colds, the flu, or other viruses.

How to use this medication?
This drug is injected into a vein. It is usually given by a health care provider in a hospital or clinic setting.
If you get this drug at home, you will be taught how to prepare and give it.
Use exactly as directed.
Take it as directed on the prescription label at the same time every day. Keep taking it unless your health care provider tells you to stop.
It is important that you put your used needles and syringes in a special sharps container.
Do not put them in a trash can. If you do not have a sharps container, call your pharmacist or health care provider to get one.
Talk to your health care provider about the use of this drug in children. While it may be prescribed for children as young as newborns for selected conditions, precautions do apply.
Overdosage: If you think you have taken too much of this medicine, contact a poison control center or emergency room at once.
NOTE: This medicine is only for you. Do not share this medicine with others.
What may interact with this medication?

What should I watch for while using this medication?
Your condition will be monitored carefully while you are receiving this medicine. Tell your doctor or health care provider if your symptoms do not start to get better or if they get worse.
This medicine may cause serious skin reactions. They can happen weeks to months after starting the medicine. Contact your health care provider right away if you notice fevers or flu-like symptoms with a rash. The rash may be red or purple and then turn into blisters or peeling of the skin. Or, you might notice a red rash with swelling of the face, lips or lymph nodes in your neck or under your arms.
Do not treat diarrhea with over the counter products. Contact your doctor if you have diarrhea that lasts more than 2 days or if it is severe and watery.
Do not drive, use machinery, or do anything that needs mental alertness until you know how this medicine affects you.
What side effects may I notice from receiving this medication?
Side effects that you should report to your doctor or health care professional as soon as possible:
allergic reactions like skin rash, itching or hives, swelling of the face, lips, or tongue
bloody or watery diarrhea
fever
fever with rash, swollen lymph nodes, or swelling of the face
pain, redness, or irritation at site where injected
pain, tingling, numbness in the hands or feet
rash, fever, and swollen lymph nodes
redness, blistering, peeling or loosening of the skin, including inside the mouth
seizures
unusual bleeding or bruising
Side effects that usually do not require medical attention (report to your doctor or health care professional if they continue or are bothersome):
constipation
diarrhea
dizziness
headache
nausea, vomiting
stomach pain
vaginal discharge, itching, or odor in women
Where should I keep my medication?
Keep out of the reach of children and pets.

Meropenem+vaborbactam
Meropenem+vaborbactam is a powerful antibiotic combining meropenem, a carbapenem, with vaborbactam, a novel non-β-lactam β-lactamase inhibitor. It is used to treat infections caused by certain strains of carbapenem-resistant Enterobacterales, Acinetobacter baumannii and Pseudomonas aeruginosa [2]. Vaborbactam protects meropenem from degradation by targeting and inactivating certain serine β-lactamases (Ambler class B).
This medication is considered a Reserve antibiotic, meaning it is reserved for specific situations as a last resort due to the risk of emerging resistance. The World Health Organization (WHO) recommends reserving this antibiotic for targeted treatment of severe infections caused by laboratory-confirmed Klebsiella pneumoniae carbapenemase-producing Enterobacterales. It could also be considered for severe complicated urinary tract infections, intra-abdominal infections, and hospital-acquired pneumonia when other options are unsuitable. Empiric use, where the specific pathogen is unknown, is only recommended in very select cases of seriously ill patients, such as those with sepsis or septic shock, where a multidrug-resistant pathogen is suspected.
Meropenem+vaborbactam exhibits activity against a wide range of bacteria, including Gram-positive aerobic bacteria, Gram-negative aerobic bacteria, and anaerobic bacteria. Importantly, vaborbactam boosts meropenem's effectiveness against many multidrug-resistant Gram-negative bacteria by inhibiting ESBLs, AmpC beta-lactamases, and Klebsiella pneumoniae carbapenemases. However, it is crucial to understand that it is not effective against bacteria producing metallo-beta-lactamases (NDM, VIM, IMP) or Ambler class D carbapenemases like OXA-48.
The standard adult dosage is 4g (2g meropenem + 2g vaborbactam) administered intravenously every 8 hours. Importantly, this medication is currently not licensed for use in children or neonates.
While generally well-tolerated, with side effects similar to meropenem alone, meropenem+vaborbactam is notably less harmful to the kidneys compared to other antibiotics used for similar infections.

Meropenem belongs to a class of medications called carbapenem antibiotics.
