Drug Cards Daily | Episode #37: gabapentin (Neurontin) | For Seizures and Neuropathic Pain

Gabapentin has a wide variety of uses but the main indications are for focal seizures in adults and children and for postherpatic neuralgia in adults. There are many commonly used off label indications such as for neuropathic pain, fibromyalgia, and alcohol dependence. Max dosing for gabapentin is around 3600 mg/day. Although the exact mechanism of action is unknown, it is clear that it is similar in chemical structure to GABA. This is important because GABA is responsible for modulating excitatory neurotransmitter release. Gabapentin is poorly protein bound at less than 3% and not metabolized. When discontinuing longer term therapies, doses should be tapered down over at least 7 days. Some of the most common side effects are dizziness, fatigue, and weight gain. The serious side effects are depression, suicidal thoughts and behaviors, and withdrawal related seizures.

Gabapentin belongs to a group of medicines called anticonvulsants.
It is a medication used to control repeated seizures in epilepsy and also to treat neuropathic pain due to nerve damage. It is a doctor's prescription-only
medication.

How does it work?
Gabapentin is thought to work by controlling brain chemicals that send signals to the nerves to help control seizures or neuropathic pain.

Before you start to take gabapentin Tell your doctor if

- you have any Allergies
- if you have any kidney problems • mixed seizure disorders which include absence seizures or any other medical condition
- if you taking any other medication which includes supplements you buy without a script.

How to take it?

The dosage of gabapentin is dependent on your age, and if you are taking any other medication.
Your doctor will usually start you with a low dose and slowly increase it to the lowest dose to control your condition.

Swallow the capsules whole with a full glass of water. It does not matter if you take this medicine before or after food.

Take it at the same time every day. Use an alarm to remind you if needed. If you are taking Gabapentin three times a day, do not allow more than 12 hours between doses.

What if you forget?

If you forget to take it, If it is almost time for your next dose which is within 4 hours, skip the dose you missed and take your next dose when you are meant to. Otherwise, take it as soon as you remember, and then go back to taking Gabapentin as you would normally
. Do not take a double dose to make up for the dose you missed.

While you are on GabapentinYou must:

- Tell all health professionals who are treating of looking after you that you are on gabapentin
- Call your doctor immediately if you have any thoughts of suicide or self-harm, any unusual changes in mood or behaviour, or show signs of depression.
- if you become pregnant

Make sure you keep all your doctor's appointments as your doctor will need to monitor your progress and may want to take some tests from time to time to prevent unwanted side effects. Gabapentin can cause drowsiness, do not drive or operate machinery until you know how it affects you
.

Side effects: All medicines can have side effects. Sometimes they are serious, most of the time they are not. Do not
be alarmed by it, because you may not even experience any of them. You may need medical attention if you get some of the side effects. For gabapentin, It can be difficult to tell whether the side effects are the result of taking this medicine; or side effects from other medicines you may be taking, or it is actually from your own condition; so it is important to tell your doctor of any change in your condition. If you are over 65 years old, you may have an increased chance of getting side effects.

Side effects information can be obtained from the leaflet that your pharmacist gives you. Here, I would like to draw attention to a few side effects, which if you experience it, you need to contact your doctor as soon as possible.
- weakness, unsteadiness when walking including falling, reduced coordination or slowed reactions
- unusual changes in mood or behaviour such as restlessness, nervousness, or excitement
- signs of new onset of, or increased irritability or agitation
- signs of depression
- seeing or hearing things that are not there, irrational thinking
- blurred or double vision, uncontrollable jerky eye movements, or difficulty seeing
- signs of frequent infections such as fever, severe chills, sore throat or mouth ulcers
- trouble breathing or shallow breaths
- loss of consciousness.

Send yourself to the hospital, if you have
- an anaphylactic reaction
- more frequent or more severe seizures or fits
- chest pain or a very fast heart rate

Side effects information is there for your reference, so you know what to do if you experience it. It is not there to scare you or to stop you from getting appropriate treatment.

Patty McGoldrick: Neurontin is also known as gabapentin and it's another medication that can be used for control of seizures and epilepsy. It's well tolerated, it's used for generalized, impartial seizures. It's available both in pills that can be crushed or it's available in an oral formulation, so a liquid for small children.

Dr. Steve Wolf: The major side effects is sedation.

Patty: Right.

Dr. Wolf: Sleepiness. It can cause a little bit of an increased appetite and weight gain, so you need to monitor that very closely. Though it's also useful for patients who are having trouble sleeping at night. It already helps to sort of increase the sleep control.

Patty: And actually we use it a lot for people who are having difficulties sleeping. It's also used for pain, for neuropathic pain.

Dr. Wolf: So it's a good medication Another option in our armory of people with difficult to control seizures but not one of your first line kind of drugs.

Patty: So Neurontin or gabapentin, third or fourth line drug for control of generalized, impartial seizures. A big side effect is drowsiness. It's also used for pain and it's well tolerated for the most part.

La gabapentina es un fármaco que se emplea para el tratamiento de la epilepsia y del dolor neuropático.

Gabapentina está disponible en el mercado en forma de cápsulas y comprimidos.

La dosis a emplear y la duración del tratamiento se deben ajustar individualmente y tienen que ser definidas por el médico. De forma general, para el tratamiento tanto de la epilepsia como del dolor neuropático, la dosis inicial va de 300 mg una vez al día a 900 mg al día dividido en tres tomas. Posteriormente, en base a la respuesta individual del paciente y la tolerabilidad, la dosis puede aumentarse en incrementos de 300 mg/día cada 2-3 días hasta una dosis máxima de 3600 mg/día.

En pacientes con insuficiencia renal, es necesario realizar un ajuste individualizado. Gabapentina se puede tomar con o sin alimentos y es aconsejable no consumir alcohol durante el tratamiento.

Entre sus reacciones adversas más frecuentes, destacamos el aumento de apetito, cambios en el estado de ánimo o dolores de cabeza. También es habitual que aparezca somnolencia y mareos, por lo que se debe tener precaución a la hora de conducir o realizar actividades peligrosas que requieran tu atención.

Entre las principales recomendaciones, es fundamental no usar gabapentina sin receta médica. No suspendas el tratamiento hasta que así lo indique tu médico y sigue sus indicaciones para realizar una suspensión gradual. Y finalmente, desecha el tratamiento que te haya sobrado en el punto SIGRE de tu farmacia.

What is Gabapentin

Gabapentin is a epilepsy medication in the class of medicine known as anticonvulsants, but it is so much more than that. Its primarily used to treat partial seizures and neuropathic pain. It is a first-line medication for the treatment of neuropathic pain caused by diabetic neuropathy- nerve damage that can occur if you have diabetes, postherpetic neuralgia- when you get chronic pain after shingles, and central pain. You also see it used for many other uses as well such as restless leg syndrome, non neuropathic pain, anxiety, and bipolar disorder.

Gabapentin mechanism of action. How does gabapentin work

Scientists aren't entirely sure how it works exactly but It appears to work by altering electrical activity in the brain, at a specific site (called the alpha2-delta site) located on voltage-gated calcium channels. Doing so, relieves nerve-pain and lowers the risk of seizures by influencing the activity of chemicals called neurotransmitters, which send messages between nerve cells in our brain. Once they start to slow down, this lowers the excitement our neurons face thereby controlling seizures as well as how we perceive pain.

How to take Gabapentin

Take gabapentin exactly as your doctor has told you to take it, and follow the directions on the bottle label and packaging. If you're not sure, check with your doctor or pharmacist.

Gabapentin side effects

Some common side effects of gabapentin that are usually mild and go away by themselves is: feeling sleepy, tired, or dizzy, nauseous, vomiting or diarrhea, mood changes are another common one, weight gain as gabapentin can make you feel more hungry, memory problems especially with short term memory, and dry mouth.

Available as a capsule and as a tablet.

How to use it?
Make sure to swallow the capsule or tablet whole with half a glass of water.
In addition, it is recommended to take gabapentin at fixed times. This will make it less likely that you forget a dose.
Furthermore, after a few days to months your doctor will evaluate if gabapentin is sufficiently effective for you. If necessary your doctor will adapt your dose or will switch to another drug.

When to use it?
Gabapentin is part of a group of drugs called anti-epileptic drugs. However it can be used to treat several medical conditions, like:
- Epilepsy
- Neuropathic pain

What is the correct dosage?
The precise dose will depend on your specific indication of use, the side effect you might experience and the severity of your symptoms.Therefore always use the precise amount and frequency your doctor prescribed you.

- Uncommon (0.1-1%): allergic reactions, high blood sugar, agitation, palpitations, swallowing disorder and oedema.
- Rare (0.1%): palpitations, low blood sugar, loss of consciousness and respiratory depression.

Safety:
First of all be careful to combine gabapentin with alcohol. As this might increase its side effects.
In addition, it can be dangerous to drive while using this drug. As it can cause you to become drowsy, sleepy and/or dizzy. If you experiencing any of these side effects then please do not drive.
