▪ Tetracyclines are antimicrobial antibiotics that inhibit bacterial ribosomes which are the organelles that make proteins. Tetracyclines bind to the A-site on the 30s subunit of the bacterial ribosome which inhibits the binding of tRNAs to the mRNA-ribosome complex, and shuts down protein synthesis before it even begins.
▪ Tetracyclines are divided according to their duration of action: Short acting tetracyclines, like tetracycline itself, have a half life of around 8 hours, and long acting tetracyclines ones like doxycycline and minocycline last for 16 hours or more. Administration of tetracyclines is usually topical, or per-oral, but tigecycline is used IV only.
▪ Tetracyclines are broad spectrum, meaning they can target a wide variety of Gram positive and negative bacteria. They are often used to treat severe acne or community acquired pneumonia. It can also be given to prevent malaria, which is a parasitic protozoa that infect blood cells and hepatocytes. Both doxycycline and tigecycline are also effective against MRSA, a notoriously tough bug that resists many common antibiotics.
▪ Counselling points about tetracycline antibiotics;
- This medication can make your skin more sensitive to the sun, and you can burn more easily. Use sunscreen, wear protective clothing and avoid the sun.
- Take with a full glass of water and stay upright for 30 minutes to avoid Gl irritation.
- This medication should be taken twice daily, 1-2 hours before, or 4 - 6 hours after antacids, vitamins, magnesium, calcium, iron, zinc, dairy products.
- Do not use this medication if you are pregnant, if you could become pregnant, or if you are breastfeeding.

In this video we will cover everything you need to know about Tetracycline, also known as Ala-Tet, Brodspec or Panmycin.

Generic name: Tetracycline.
Brand names: Ala-Tet, Brodspec or Panmycin.
Available as a capsule, oral suspension, ointment, cream and drops.

How to use it?
- Capsule: swallow the capsule whole with half a glass of water, preferably on an empty stomach.
- Oral suspension: first shake the bottle, afterwards measure the right dose with a syringe or measuring spoon. The oral suspension is mostly used to treat sores in the mouth, therefore hold the liquid in your mouth for 5 minutes. Then spit it out. Do not swallow the oral suspension.
- Cream/Ointment: first clean the infected areas, remove crusts by soaking them in baby oil. Afterwards apply the ointment or cream thinly. If needed you can cover this area with a sterile bandage.
- Drops: Clean your hands, remove any crusts around your eyelids, go stand before a mirror, gently pull down your lower eyelid, bend your head backwards, place the tube above your lower eyelid, squeeze it so the drops will fall into your lower eyelid and than blink a few times.

When to use it?
Tetracycline is an antibiotic which can inhibit the replication of certain bacteria. It can be used to treat infections by microorganisms sensitive to tetracycline, such as infections of:
- The respiratory tract, like a pneumonia.
- Infections of someone's urogenital tract, like STI’s or an urinary tract infection.
- Infections of the skin, like acne.
- Gastrointestinal infections.
- Eye infections.

What is the correct dosage?
The precise dose will depend on your specific indication of use, the side effect you might experience and the severity of your symptoms. Therefore always use the precise amount and frequency your doctor prescribed you.

- However, when treating most infections your doctor will prescribe a capsule of tetracycline of 250mg 4 times a day, which can be increase to a maximum dose of 2 gram per day.
- For eye infections your doctor might prescribe drops: 6 drops/day dripped into your lower eyelids. This dose can be increase up to 1 drop every hour.
- For skin infections your doctor might prescribe you to apply a thin layer of ointment or cream to the affected skin 2–3×/day.

What are common the side-effects?
Skin reactions, rash, nausea, vomiting, diarrhea, inflammation of your stomach, itching, liver damage, abnormalities in blood values, burning sensation of the eyes, irritation and pain.

Safety:
First of all there are no limitations for this drug regarding the usage of alcohol or driving a car.
However, it is important you do not consume milk or milk products, such as cheese or yogurt, 2 hours before to 1 hour after taking tetracycline as a capsule. As the lime in these products decreases the effectivity of tetracycline.
Furthermore, tetracycline can have interactions with other drugs your are taking. Always discuss this with your prescribing doctor.
Here it is important to mention that tetracycline can be dangerous for you and your unborn baby during pregnancy. Therefore its use is often strongly discouraged, especially when using capsules.
When breastfeeding tetracycline can often be used safely for a short period (less than 3 weeks). However avoid prolonged use. Always discuss this with your personal doctor.
