Valium (diazepam) has an active metabolite that takes 100 hours to decrease in someone's body by 50%. This means that if someone is taking valium, it will take approximately 21 days to fully leave their system, once they stop taking it. Many individuals do not realize this, and have severe difficulties trying to decrease or stop this medication, because they decrease too fast. For example, they may make a drop, then after 3 or 4 days think, "this is going well, I'm ok to make another." That second drop then catches up with them (severe anxiety and withdrawal, which can be dangerous) because they had not come to a new baseline. We often recommend waiting 1-2 months after a decrease in valium, for patients who have taken this medication long-term and wish to taper off of it.
Do not start, stop or change your medication dose, without speaking directly with your provider.

What is Valium?
Valium, also known as Diazepam, is a benzodiazepine drug that produces a calming effect. It is used to treat anxiety, seizures and even insomnia.
How Does Valium Work?
Valium works by binding to GABA ion channels and promoting GABA activity in your nervous system.
GABA is an inhibitory neurotransmitter, so by increasing GABA activity, valium effectively inhibits the nervous system, reducing excitement signals and calming your brain and your body.
What are the side effects?
Valium can cause dizziness, drowsiness, weakness and confusion.
Don’t take valium with alcohol, it will amplify the effects which can lead to respiratory depression and loss of consciousness.
Also, be careful with grapefruit! It is metabolised by an enzyme called CYP3A4, and so is valium. When combined, it can slow your metabolism so you absorb more of the drug, making it far more potent!
Finally, if you use valium regularly, you will build up a tolerance, meaning you will need more of the drug to get the same calming effects, but this can be very dangerous and potentially lead to an overdose.
If you find yourself taking Valium regularly, try to get some help and find an alternative relaxant.

Valium (Diazepam) is a benzodiazepine which is commonly used to treat anxiety disorders. It is also frequently used to treat alcohol and sedative withdrawal (in detox facilities) as it has a very long half-life. There are significant risks of side effects, including abuse, dependence, tolerance and withdrawal.
Dr. Pagnani discusses tapering and stopping this medication in patients as well. Because it has a very long half-life (taking up to 100 hours for half of its metabolites to leave someone's system) patients often do not wait long enough after a decrease in their dose (to allow themselves to come to a new baseline), before making an additional decrease. It can take up to 3 weeks, to fully leave someone's system (when they stop the medication)! This long half-life can lead to a compounding effect of withdrawal, and patients believing that they need to stay on the medication "forever," when in fact, they can often taper off of it, with a more calculated and slow approach. Providers can even use compounding pharmacies to make incredibly small decreases overtime.
Starting, stopping or changing doses of this medication needs to be done by an individual's psychiatric provider, or patients can have serious adverse outcomes.

Benzodiazepines are a class of medication, often used to treat short-term anxiety conditions, including panic attacks and specific phobias. Some examples of Benzodiazepines include diazepam, lorazepam, and alprazolam. Benzodiazepines lead to generalized decreased neuronal activity, leading to characteristic signs and symptoms, which we discuss in this lesson.

Clotrimazole belongs to a class of medications called broad-spectrum antifungal drugs.
Clotrimazole’s mechanism of action is to alter the permeability of the fungal cell wall leading to loss of rigidity, thus resulting in fungal cell death.
Clotrimazole is prescribed for treating diaper rash, oral thrush or oropharyngeal candidiasis, athlete’s foot (tinea pedis), tinea corporis or ringworm infections, jock itch or tinea cruris, vaginal yeast infections or vulvovaginal candidiasis, pityriasis versicolor (skin eruption on the trunk and proximal extremities), and skin diseases with associated fungal infections.
Clotrimazole throat paints must be applied to the lesions by using a clean cotton bud after the meals.
Clotrimazole vaginal cream or suppositories can be placed by using the applicator provided.

Clotrimazole is an imidazole antifungal that exerts its pharmacological effect by inhibiting the synthesis of ergosterol, an essential component of fungal cell membranes. This inhibition compromises membrane integrity, leading to leakage of cellular contents and ultimately fungal cell death.
Clotrimazole is primarily used topically due to poor systemic absorption when administered via the skin or mucous membranes, which limits systemic side effects.
When clotrimazole is used intravaginally or orally in lozenge form, localized concentrations are sufficient to treat mucocutaneous infections without significant systemic exposure.
Pay attention when clotrimazole is used frequently to treat Candida infections as corticosteroids, immunosuppression, and antibiotics may increase the risk of this type of infection.

Clotrimazole Uses, Side Effects, Precautions, Interactions
Uses
Clotrimazole is used to treat skin infections such as athlete's foot, jock itch, ringworm, and other fungal skin infections (candidiasis). This medication is also used to treat a skin condition known as pityriasis (tinea versicolor), a fungal infection that causes a lightening or darkening of the skin of the neck, chest, arms, or legs. Clotrimazole is an azole antifungal that works by preventing the growth of fungus.
How to use Clotrimazole
Use this medication on the skin only. Clean and thoroughly dry the area to be treated. Apply this medication to the affected skin, usually twice a day or as directed by your doctor. Dosage and length of treatment depends on the type of infection being treated. Do not apply this more often than prescribed. Your condition will not clear faster, but side effects may be increased.
Apply enough medication to cover the affected area and some of the surrounding skin. After applying this medication, wash your hands. Do not wrap, cover or bandage the area unless directed to do so by your doctor.
Do not apply this medication in the eyes, nose, mouth, or vagina.
Use this medication regularly in order to get the most benefit from it. Remember to use it at the same times each day.
Continue to use this medication until the full prescribed amount is finished, even if symptoms disappear after a few days. Stopping the medication too early may result in a return of the infection.
Tell your doctor if your condition lasts after 4 weeks of treatment or gets worse at any time.
Side Effects
Burning, stinging, swelling, irritation, redness, pimple-like bumps, tenderness, or flaking of the treated skin may occur. If any of these effects last or get worse, notify your doctor or pharmacist promptly.
If your doctor has directed you to use this medication, remember that your doctor has judged that the benefit to you is greater than the risk of side effects. Many people using this medication do not have serious side effects.
Tell your doctor right away if you have any serious side effects, including: blistering, oozing, open sores.
A very serious allergic reaction to this drug is rare. However, get medical help right away if you notice any symptoms of a serious allergic reaction, including: rash, itching/swelling (especially of the face/tongue/throat), severe dizziness, trouble breathing.
This is not a complete list of possible side effects. If you notice other effects not listed above, contact your doctor or pharmacist.
Precautions
Before using clotrimazole, tell your doctor or pharmacist if you are allergic to it; or to other azole antifungals such as econazole, ketoconazole, or miconazole; or if you have any other allergies. This product may contain inactive ingredients, which can cause allergic reactions or other problems. Talk to your pharmacist for more details.
Before using this medication, tell your doctor or pharmacist your medical history.
Before having surgery, tell your doctor or dentist about all the products you use (including prescription drugs, nonprescription drugs, and herbal products).
During pregnancy, this medication should be used only when clearly needed. Discuss the risks and benefits with your doctor.
It is unknown if this drug passes into breast milk. Consult your doctor before breast-feeding.
Interactions
Drug interactions may change how your medications work or increase your risk for serious side effects. This document does not contain all possible drug interactions. Benzodiazepine or benzos for short, is the class of drug that works at the GABA receptors in your brain to produce a relaxing or sedative effect. We use benzodiazepines to treat short-term anxiety and sleep. Some doctors will use them as a muscle relaxer – Valium is pretty typical for this. They can also be used to stop a seizure. You may get valium or Ativan prior to a procedure like a colonoscopy.
The common benzodiazepines or benzos for short that we use for anxiety are Ativan, Klonopin, Xanax and Valium.
Benzodiazepines are habit forming and it’s easy to become addicted to them. Here’s why. Benzos cause a tolerance effect. This means if you take them regularly like daily or several times a week, overtime the effect can wear off and you;ll need to take more to get the same effect. So now instead of a half pill or one pill working, you notice you need two pills.
This is a medication tolerance effect. It can happen to anyone and is not the same as addiction. If this happens to you, it doesn’t mean you’ve been misusing the medication or have an addition. Addiction involves developing a psychological dependence on it where you need to take it.
Daily, long term use is associated with cognitive changes like not being as mentally sharp. It can create balance problems in some people where you just aren’t quite solid on your feet. This can be a big problem with elderly people by increasing their risk of falls.
In this video I discuss the details of what dependence looks like versus addiction. I also discuss how to avoid developing a dependence.

Generic name: Clonazepam.
Brand names: Klonopin and Rivotril.
Available as tablet, drops or injection.
How to use it?
Tablet: Swallow the tablets with half a glass of water.
Drops: put the number of drops you need to take on a spoon. Mix them with something to drink and drink it.
Injection: If you want to use it as an injection. Your doctor or nurse will administrate it.
Duration of use?
It is often discouraged to use Clonazepam for more than several weeks straight. As this can increase your risk for becoming dependent on this drug. Which means that you will get withdrawal symptoms when you stop taking it.
When to use it?
Clonazepam is part of a group of drugs called Benzodiazepine. It can be used to decrease symptoms of several medical conditions:
- Epilepsia
- Panic disorder
- social phobia
- And muscle cramps
What is the correct dosage?
The precise dose will depend on your specific indication of use, the side effect you might experience and the severity of your symptoms.Therefore always use the precise amount and frequency your doctor prescribed you.
- When treating epilepsy in Adults the initial oral dose is 1-2 mg/day. This can be slowly increased up to a maintenance dose of 2-4 mg/day. The maximum dose would be 20mg/day.
What are common the side-effects?
- Uncommon (affects 10 to 30 in 100 people): Drowsiness and fatigue.
- Rare (affects 1 to 10 people in 100): Slurred speech, difficulty concentrating, slowed thinking, memory problems, impaired coordination, double vision, muscle weakness, balance problems and dizziness.
- Very rare (affects less than 1 in 100 people): Nausea, diarrhoea, headache, incontinence, difficulty breathing, rash, itching, impotence, seizures and bruising.
Safety:
First of all be careful to combine clonazepam with alcohol. As this might increase it’s side effects.
In addition, it can be dangerous to drive while using this drug. As it can cause you to become drowsy, sleepy and/or dizzy. If you experiencing any of these side effects than please do not drive.
Furthermore, clonazepam can have interactions with other drugs your are taking. always discuss this with your prescribing doctor.
Pregnancy and Lactation:
Research shows that use of clonazepam could be dangerous. However, there are some exceptions. Therefore always discuss this with your personal doctor.
