PCRI’s Executive Director, Mark Scholz, MD, discusses common side effects that men experience while undergoing hormone blockade and their treatments.

1:50 The most important side effects to be aware of while on hormone therapy (TIP) are the loss of muscle mass and fatigue. Without testosterone, some men will experience the loss of around 30-40% of their muscle mass and thus will experience intense fatigue. These side effects are manageable through weight training 2-3 times per week, and some men may even gain muscle mass while on hormone blockade if they are consistent with their weight training.

3:28 While taking TIP men will experience a drop in their metabolic rate and if a man keeps the same diet as before the TIP then he will most likely gain weight. Therefore, men should consider lowering their caloric intake in anticipation of and during hormone blockade.

3:58 The loss of interest in sexual activity is a notable but temporary side effect while on TIP. Men might be able to perform as they normally would with the use of medications like Viagra and Cialis; however, men will experience a loss of the desire to have sex for the duration of the hormone blockade and a short time afterward.

4:46 Breast enlargement occurs in around 33%-50% of men on hormone blockade depending on the specific medicine. Once enlargement occurs, it is only treatable with plastic surgery. However, there are preventative measures a man can take to prevent breast enlargement before it happens. One is to take an estrogen-blocking pill called Femara. There are no side effects for men. Another is to have radiation treatments to the nipple area prior to hormone blockade. Men who are uncompromising about breast growth may consider both preventative options.

6:04 While not dangerous hot flashes are an uncomfortable and intrusive symptom of TIP. Luckily, it can be treated effectively. Perhaps the most effective treatment is a small dose of estrogen delivered by a patch. Other less effective treatment options are the use of Effexor (an antidepressant) or Neurontin (typically used for peripheral neuropathy for seizures).

7:06 Men taking TIP may begin to experience a greater range of emotions than before treatment. While some welcome this change, others may find it intense and uncomfortable. For those men a medication like Effexor is effective for stabilizing their mood.

7:48 The side effects of TIP may show up as laboratory abnormalities that are important to be aware of especially when talking with a doctor who is unaware of the side effects of TIP. For example, men on testosterone blockade will see a usually inconsequential drop in their red blood cell count (but may result in fatigue in extreme cases) that may appear as something more serious to your doctor.

8:35 There are some TIP (esp. Zytiga, Casodex) that cause liver irritation in certain men. Thus, it is important to have blood work done periodically (especially at the beginning of treatment) to ensure that there is no irritation of the liver. If there is, then that medicine must be stopped immediately.

9:01 High blood pressure has been observed as a side effect in 10-20% of men who are using second-generation TIP (e.g. Zytiga, Xtandi). It can usually be treated with typical blood pressure medicines. If the high blood pressure is too severe, however, then treatment with that medication may need to be stopped.

9:31 Osteoporosis, or loss of calcium in their bones, is another possible side effect for men on hormone blockade.

Estimated one in seven men will be diagnosed with prostate cancer in their lifetime. An estimated 220,000 men in the U.S. will be diagnosed with prostate cancer this year. Prostate cancer can be a complex issue for the patient and his family to understand. There are many treatment options for prostate cancer including surgical removal of the prostate, external beam radiation (IMRT / SBRT), proton beam therapy, cryotherapy, hormone treatment, chemotherapy, focal treatment, and even not immediately treating prostate cancer for low risk cases (Active surveillance). Early detection of prostate cancer is important. Most prostate cancers are detected when the cancer is still contained within the prostate gland, but some cancers are advanced and have moved outside of the prostate gland (metastatic). Each person with prostate cancer has their own unique conditions and situations that can affect treatment decisions going forward. Cancer staging radiographs (CT scan, bone scan, PET scan, MRI, etc) are often done by the urologist that performed the prostate biopsy to assess for spread of cancer outside of the prostate.

Regardless, of the treatment you choose for prostate cancer, optimizing overall health and wellness is an important part of any successful treatment plan.

Action: Inhibits synthesis of bacterial cell wall leading to cell death.

Therapeutic Class: anti-infectives, antiulcer agent

Nursing Considerations

Your medication Augmentin

Amoxicillin / potassium clavulanate is also known by the brand names: Augmentin, Augmentin ES-600, and Augmentin XR.

Augmentin comes in oral suspensions, tablets, chewable tablets, and extended release tablets.

Augmentin is most commonly used for bacterial infections. No matter you’re your doctor has your taking it for, Augmentin is usually taken by mouth with food twice a day. If you miss a dose, take the missed dose as soon as you think about it. If it is close to time for your next dose, skip the missed dose and resume your normal dosing schedule. Do not take 2 doses at the same time or extra doses.

Some common side effects with Augmentin include diarrhea and upset stomach.
Some less common side effects with Augmentin include a diaper rash in small children, skin rash, itching, a vaginal yeast infection, and pain or discomfort in the stomach.

While taking Augmentin, remember it is important to tell your doctor or pharmacist if:
• You have signs of an allergic reaction (rash, itching, swelling, tightness of the chest, etc.)
• You have an allergy to any penicillin, amoxicillin, clavulanate potassium or any other part of this drug.
• You have mononucleosis
• You are taking probenecid
• You start taking new medications, vitamins or supplements

Augmentin liquid needs to be stored in the refrigerator and shaken well before each dose is given using the measuring device received from the pharmacist or doctor. Any liquid remaining after 10 days needs to be thrown away. All other forms of Augmentin are kept at room temperature, in a dry place.
It is important to take this medication until full prescribed amount is finished, even if symptoms disappear.

This is an informational overview video on Amoxicillin. It is available as a powder for oral suspension, a tablet and an injection.

When to use it?
Amoxicillin is a wide-spectrum antibiotic. Which means it can be used to treat thousands different types of bacteria. Therefore amoxicillin is mostly used to treat infections caused by unknown microorganisms, such as:
An upper respiratory tract infection (like a sinusitis)
An lower respiratory tract infection (pneumonia)
An urogenital tract infection (Chlamydia)
An gastrointestinal infection (H. pylori bacteria that can cause stomach ulcers);
Infections in your bones (e.g., osteomyelitis, prosthetic joint infections);
And lastly severe skin and soft tissue infections.

How to use it?
Swallow it tablet whole, with a glass of water. Do not chew on it or crush it. Or resolve the powder in half a glass of water and drink it.
Always take it at fixed times: preferably in the morning and in the evening. This lowers your chances on forgetting a dose. Furthermore, do not interrupt or stop the treatment and finish it completely.

What is the correct dosage?
It is important to always follow the precise prescription your doctor gave you.
Dose for adults and children 40kg:
250-500 mg every 8 hours
750-1000 mg every 12 hours

In serious infections
750–1000 mg every 8 hours

Did you forget to take a dose? And does it take less than 4 hours till your next dose? Than skip the dose you missed and take the forgotten dose at the end of your treatment.

What are the side-effects?
(10%): diarrhea.
(1-10%): nausea, vomiting, loss of appetite, skin reactions (such as exanthema and itching).
(0.1-1%): Dizziness, headache, super infections.
(0.01-0.1%): anaphylactic shock, convulsions, drug fever.

Safety:
You can combine amoxicillin with any type of food and even with alcohol.
Don’t use amoxicillin if you are hypersensitivity to penicillin.

Pregnancy and Lactation:
You can safely use amoxicillin if you are pregnant or breastfeeding.
However, you should stop with breastfeeding, if you are using amoxicillin and you child gets diarrhea, candidiasis and / or a skin rash.

Amoxicillin is one of the most consumed antibiotics in the primary care setting.
It is an amino penicillin created by adding an extra amino group to penicillin to battle antibiotic resistance.
Amoxicillin belongs to the class of beta lactam antibiotics.
They act by binding to the penicillin binding proteins in bacterial cell walls, and inhibiting a process called transpeptidation.
Transpeptidation is the cross-linking process of peptides during bacterial cell wall synthesis, and it is essential for the replication & survival of bacteria.
So, inhibition of transpeptidation leads to cell wall lysis and death of the bacteria.
This type of action is called the bactericidal activity since bacteria get killed by the antibiotic.
Amoxicillin covers a wide variety of gram-positive bacteria, with some additional gram-negative coverage compared to standard penicillins.
Like penicillin, it covers most streptococcus species, and is also effective against listeria monocytogens, and enterococcus species.

And post exposure prophylaxis for anthrax.
Off label uses of amoxicillin include the following.
Lyme disease, if doxycycline is contraindicated.
Prophylaxis for infective endocarditis.
Periodontitis, in combination with metronidazole.
And actinomycosis.
Bactericidal antibiotics like amoxicillin are most effective when given in a time dependent manner rather than a concentration dependent manner.
Time dependent refers to the time that serum concentrations of the drug exceed the minimal inhibitory concentration for the microorganism.
Therefore, these drugs are dosed more frequently.
Amoxicillin is only taken per orally.
It is available in immediate and extended-release tablets.
It also comes as a chewable tablet or a suspension.
Common adverse effects of amoxicillin include the following.
Gastrointestinal disturbances such as nausea and vomiting, and diarrhea.
Superinfection, especially with candida species and clostridium difficile.
Nephrotoxicity.
Hepatotoxicity.
And hypersensitivity reactions like rash, and in more serious situations, anaphylactic shock.
Amoxicillin is safe in pregnancy and breastfeeding.

Uncontrolled high blood pressure increases your risk of serious health problems, including heart attack and stroke. So bottomline, If you have high blood pressure, and your doctor has you on losartan, know that this helps you prevent future heart disease, heart attacks and strokes by making sure the pressure stays at a healthy and normal range.

What is losartan used for

Losartan is also used to reduce your risk of a stroke if you have high blood pressure with left ventricular hypertrophy (LVH), which is just a fancy way of saying the walls in the heart’s left ventricle have began to thicken, its also used to treat kidney disease caused by diabetes.

How does losartan work to lower blood pressure?

Losartan is a type of medicine called an ARB, an angiotensin 2 receptor blocker, and in the family known as the -sartans. Like other ARBs, you may have heard of, valsartan, telmisartan, olmesartan, irbesartan, these medications work by blocking the action of angiotensin II, a chemical in your body that causes your blood vessels to tighten and narrow. Losartan will help to relax and widen your blood vessels to lower your blood pressure, which will in turn, lower your risk of stroke.

What should I keep in mind with losartan potassium 50 mg

Losartan is available in tablet form at 25, 50, and 100 mg quantities. For adults: usually your doctor will have you on 50 mg once daily but may increase it to 100 mg per day if needed.

Is losartan safe to take now 2020

Losartan is not recommended in pregnancy or while breastfeeding as it is actually a 2014 black box warning. So if you are taking losartan, and are planning to become pregnant, let your doctor know before hand, of if there are any signs of current pregnancy as this can be harmful to you or your child. Certain lot numbers of losartan are now safe to take.

How to take losartan potassium?

Take losartan or cozaar exactly as your doctor has told you take it, and follow the directions on the label. If you're not sure, check with your doctor or pharmacist. Your doctor may suggest that you take your first dose before bedtime, because it can make you dizzy. But After the very first dose, you can take losartan at any time of day. Try to take it at the same time every day. Another important tip is to: Ensure that you are adequately hydrated before starting this medicine. You can take losartan tablets with or without food, but make sure to swallow the tablets with a full glass of water.

What are the side effects of losartan?

Like all medicines, losartan can cause side effects, although not everyone gets them. Here are some that you should be aware of and let your doctor know if they become too troublesome.
Hypotension, headaches, nausea, muscle pain.
Talk to your doctor or pharmacist if these side effects bother you or don't go away. These are not all the side effects of losartan. So For a full list, see the leaflet inside your medicines packet.

When should I call the doc for losartan side effects

An Allergic reaction:
This includes Itching or hives, swelling in your face or hands, swelling or tingling in your mouth or throat, chest tightness and troubled breathing.

Severe stomach pain, pale skin, feeling tired, fainting, purple spots, any sign of bleeding, sore throat and fever, feeling weakness, an irregular heartbeat, pins and needles and muscle cramps. Then there is the High potassium blood levels with symptoms of heart rhythm problems, like muscle weakness and a slow heart rate. Also: Swelling in the feet, ankles, and hands should be brought up to your doctor as well.

What to know when taking losartan dosage

Losartan or cozaar starts to reduce high blood pressure after about 6 hours but it may take 3 to 6 weeks to fully take effect.

Generic name: Losartan.
Brand names: Cozaar.
Available as a tablet.

How to use it?
- Tablet: Just swallow it with half a glass of water.
It is usually best to take this drug at a fixed time. If you experience dizziness as a side effect, you could best take it in the evening before going to bed.

When to use it?
Losartan is part of a group of drugs called angiotensin receptor blockers (ARB). These drugs cause the dilation of the blood vessels and reduced aldosterone secretion. This causes a drop in blood volume and blood pressure.
Therefore Losartan can be used to treat; a high blood pressure, heart failure and kidney problems.

What is the correct dosage?
The precise dose will depend on your specific indication of use, the side effect you might experience and the severity of your symptoms. Therefore always use the precise amount and frequency your doctor prescribed you.
- However, when treating a high blood pressure in adults, most doctors will start with an initial dose of 50 mg 1x/day. Which if necessary can be increased up to a max. dose of 100 mg 1/day.
- When treating heart failure, the initial dose is; 12,5mg 1/day. Which if needed can be build up to a maximum dose of 150mg 1/day.

What are the side-effects?
- Common (1–10%): low blood pressure, dizziness, fatigue, kidney function problems and renal failure.
- Uncommon (0.1–1%): Palpitations, chest pain, oedema, cough, shortness of breath, abdominal pain, nausea, vomiting, constipation, diarrhoea, headache, somnolence, rash and itching.
- Rare (0.01-0.1%): various serious side effects.

Safety:
- It is safe to drive while using this drug.
- Furthermore, alcohol widens the blood vessels. It can therefore increase the side effect of dizziness at the beginning of treatment. Try drinking alcohol in moderation first so you can estimate its effects.
- Losartan can also have interactions with other drugs your are taking. Always discuss this with your prescribing doctor.
- Lastly, Losartan could be dangerous for pregnant women and women who are breastfeeding. To be sure, always discuss this with your personal doctor.

Hypertension affects over a billion people around the world, and it is a major risk factor for heart disease, and stroke.
Losartan is a medication that is used primarily in the treatment of hypertension.
It belongs to the class of angiotensin receptor blocker drugs, which also includes valsartan and candesartan.
In addition to hypertension, losartan is used to treat heart failure and diabetic nephropathy.
One major adverse effect of losartan is hyperkalemia.
Therefore, people who are taking losartan should follow a low potassium diet.
Other side effects of losartan include the following.
Renal insufficiency.

Angioedema.
Gastrointestinal disturbances such as constipation, nausea, and vomiting.
Hematological problems such as anemia.
Central nervous system problems such as headache and dizziness.
Skin manifestations such as rash and photosensitivity.
Musculoskeletal problems such as muscle and joint pain.
And edema.
Finally, losartan is contraindicated in pregnancy.
Another contraindication is losartan hypersensitivity.
And it should not be used in combination with Aliskiren in patients with diabetes.
