Your Telmisartan is causing higher serum potassium! ARBs work by blocking the angiotensin II receptor which decreases sodium absorption and potassium excretion. In turn, this can cause hyperkalemia, however, the occurrence is rare in healthy individuals. Hyperkalemia is an issue as it can cause cardiac arrhythmia, muscle weakness, paralysis, and death. Keeping it less than 5.5 mEq/L is the recommendation and we can make dietary adjustments prior to stopping the ARB and making dose adjustments as well.
