It’s an analgesic and opioid

Parenteral doses are 5x more potent than oral

IMMEDIATE RELEASE:
Dose:
Tabs 2 to 4mg q 4-6hrs PRN
Liquid 2.6mg or 1.5mg q3-6hrs
I.V. 0.2-1mg q 2-3hrs
I.V. 0.5mg-1mg/hr infusion
I.M./S.C-with care
I.M is not good because of pain
S.C =0.167 to 4mg/hour continuous infusion

Patient- controlled analgesia (PCA)
Loading dose 0.4mg
Range 0.1-0.4mg
Lockout internal = 10 minutes

Controlled – release capsule e.g.
Hydromorph-contin
Opioid tolerant 12mg-18mg daily or BID or TID
Opioid naïve 3mg q 12hrs

Extended release tabs [Exalgo]
-For opioid tolerant only
-Dose is variable
