Pioglitazone is an oral medication used to manage type 2 diabetes mellitus. It belongs to the thiazolidinedione class of drugs and works by improving insulin sensitivity in the body, helping to control blood sugar levels. Below are its uses and typical dosing:

Uses of Pioglitazone
1. Type 2 Diabetes Management:
- Used as an adjunct to diet and exercise to improve glycemic control.
- Can be used alone or in combination with other antidiabetic medications, such as metformin, sulfonylureas, or insulin.

2. Polycystic Ovary Syndrome (PCOS)(off-label):
- Sometimes prescribed to improve insulin resistance in women with PCOS.

Dosage of Pioglitazone:
Standard Starting Dose: 15–30 mg once daily.
Maximum Dose: 45 mg once daily.
Administration: Taken orally, with or without food.
Adjustments:
- Dose adjustments may be made based on blood glucose levels and tolerability.
- If used in combination with insulin or other antidiabetic drugs, the dose of these medications may need to be reduced to avoid hypoglycemia.

Important Considerations
1. Monitoring:
- Regular monitoring of blood glucose levels, HbA1c, and liver function tests is recommended.
2. Contraindications:
- Not recommended for patients with heart failure (NYHA Class III or IV) or active liver disease.
3. Side Effects:
- Common: Weight gain, fluid retention, and edema.
Rare but serious: Bladder cancer (long-term use), fractures (especially in women), and liver toxicity.
4. Pregnancy and Lactation:
Not recommended during pregnancy or breastfeeding.

Always consult a healthcare provider for personalized dosing and to ensure safe use of pioglitazone.
