Telmisartan – Side Effects Overview
Telmisartan is an angiotensin II receptor blocker (ARB) commonly prescribed to manage high blood pressure and protect against cardiovascular and kidney complications, especially in diabetic patients. 1. **Hyponatremia**: Telmisartan can impair renal sodium handling, especially when used in combination with diuretics or in patients with underlying renal impairment. This can result in abnormally low sodium levels, leading to symptoms like confusion, weakness, or seizures in severe cases.
2. **Dehydration**: Due to its blood pressure-lowering and diuretic-sparing effects, Telmisartan may contribute to volume depletion, particularly in patients on low-salt diets, elderly individuals, or those taking concurrent diuretics. This can lead to dizziness, fatigue, and dry mouth.

5. **Hyperkalemia**: Telmisartan reduces aldosterone secretion, which can lead to potassium retention. In susceptible individuals—especially those with kidney impairment or those taking potassium supplements or potassium-sparing diuretics—this can cause elevated serum potassium levels, potentially leading to cardiac arrhythmias.
6. **Birth Defects**: Telmisartan is contraindicated during pregnancy, particularly in the second and third trimesters, due to the risk of fetal toxicity. Exposure can result in developmental abnormalities, renal dysfunction, oligohydramnios, and even fetal death.
