In this video, we aim to investigate the most widely used and prescribed drug for acne - Isotretinoin. It is famously known for its long list of side effects, which can be very concerning and intimidating for lots of people struggling with acne, to choose whether they want to go down this route or not. We define what acne is, the three main types of acne, discuss the success rate of Isotretinoin followed by an in-depth walkthrough of the drug’s mechanism and effects on the body using a 6-month course of treatment timeline. Then, side effects are also discussed and organized by severity, as well as a list of common questions one can ask to their dermatologist when about to go on Isotretinoin. We have received valuable feedback from Dr. Mohannad Abu Hilal, a licensed dermatologist working for McMaster Dermatology Center who kindly expert-reviewed our work. This video was made using Clip Champ, with stock videos available to use provided by the editing software.
This video was made by McMaster University students Mohit Sehjal, Iffah Shaikh, and Sadiqa Sheikh in collaboration with the Demystifying Medicine McMaster Program.

Is Accutane a panacea or a dangerous drug? Dermatologist Dr. Richard Thomas provides important answers about Accutane. A must watch for those with moderate to severe acne who are considering Accutane.
Isotretinoin, the active in Accutane, is a retinoid, a vitamin A derivative. It's an amazingly effective drug that has been in use since 1982, but it's also a controversial drug that has had a turbulent history. At the same time, dermatology practice without this drug is difficult to imagine. Isotretinoin is used for nodular or cystic acne, and often reserved for severe cases. Sometimes it's used for milder acne where the acne is impactful on the patient. The trend has been to treat the patient earlier before the symptoms become too severe, and the risk of serious scarring increase.
The controversial aspects: It's teratogenic, it causes birth defects. Avoiding pregnancy, so birth control is absolutely crucial. Other controversies surrounding Accutane is depression and there have been some links. Some studies have suggested a weak link between depression and Accutane use. My own suspicion is that it's idiosyncratic. A very small number of people are likely to be susceptible to depression when on this drug. I think we need to be aware of this, and patients need to be very aware of this possibility. Most recently, there's been issues of a class action lawsuit in the United States regarding the connection between inflammatory bowel disease and Accutane use. Ulcerative colitis and accutane have also been reported. These are slight increases, according to most of the studies. The reality, I think is that the risks affect very very few people, but it's important to understand these possibilities and to pull the plug when some of these symptoms appear. The drug leaves the system very quickly, and in most cases, with it the side-effects also seem to fade.
On the plus side, this drug has revolutionized the whole practice of dermatology. There are very few true cures to anything in dermatology, and even medicine as a a whole. Accutane is one of those. It's a very powerful drug, that has to be monitored very closely, do the blood work, but it has also helped a ton of patients and have changed their life for the better in significant ways.

I'm John Barbieri, MD, a board-certified dermatologist and acne expert at the Brigham and Women’s Hospital and Harvard Medical School. In this video we will discuss how to prevent common Accutane (isotretinoin) side effects such as dryness. Although often thought of as a difficult treatment, it doesn’t have to be this way!
However, isotretinoin is not the right treatment for everyone, so make sure to discuss with a healthcare professional before considering whether to take isotretinoin.
