Drug Cards Daily | Episode #33: valsartan (Diovan) | Treating NSTEMI, STEMI, Heart Failure and Hypertension
Valsartan, also known as Diovan, is an angiotensin II receptor blocker (ARB). It is used for treating NYHA class II to IV heart failure along with both treating and managing hypertension. Another common indication is for both STEMI and NSTEMI patients. Typical initiation dosing for heart failure, STEMI and NSTEMI is 20 mg twice daily with titrations up to 160 mg twice daily. When initiating treatment for hypertension the range begins at 80-160 mg po every day up to a max dose of 320 mg per day. Valsartan works by blocking AT2 from the AT1 receptor and is considered to be more efficient than ACEs along with having less side effects such a cough. Some common side effects are orthostatic hypotension, dizziness, and lightheadedness. Common monitoring parameters for valsartan are blood pressure, blood pressure urea, pregnancy, and electrolytes. Patients should avoid salt substitutes containing potassium. There is a black box warning for fetal toxicity because drugs that affect the renin-angiotensin system can cause injury/death to the fetus.

Phenytoin (Dilantin): Anticonvulsant Drug for Tonic-clonic or Partial Seizures in Epilepsy Patients

Phenytoin belongs to a class of medications called anticonvulsants or anti-epileptic drugs.

Dilantin is one of our oldest anti seizure medications that has been around. It’s indicated for patients who have seizures that start in one area of the brain and spread, so partial complex seizures. The problem with Dilantin is that, it works great for a rescue medication and it’s used a lot in cases of status epilepticus. It’s used post neurosurgery or epilepsy surgery but it has some bad long term side effects so it’s not used as much as it once was. It can cause gum hyperplasia and sort of thickening and coarsening of the facial features. There are people that are still on it who have been on it for years, because it’s the only thing that works but it’s not usually a first choice. It has a big issue with bone health, so patients who are on Dilantin, it could affect their bones, cause wasting of the bones, can cause thinning of the hair as well. Again, it’s an old school medication, not one of our first choices, but as Patty said, it’s very good in the operating room, emergency room, but you can’t give it too quickly in the IV, you have to give it slowly because it can cause cardiac problems. So, it’s one of the medications that they’re sort of phasing out of using in acute care, but clearly many patients are still on it, so it’s important to talk to your doctor, see if you should stay on that medication, are there other options, and to monitor the side effects closely.

- What is Trazodone ( Desyrel )?
- Trazodone is an Antidepressant. It is a weak inhibitor of serotonin re uptake. So therapeutic benefit appears to be related to its ability to block postsynaptic serotonin type 2 receptors. It is also sedating, probably because of its potent histamine H1-blocking activity.
- It is available as 50, 100 and 150 mg tablets.
- Trazodone is indicated for the treatment of major depressive disorder. It has been used off-label for adjunct therapy in alcohol dependence, and off-label to treat anxiety, panic disorder, and insomnia.
- The usual dosage of Trazodone
- Initial dose: 150 mg orally per day in divided doses; this may be increased by 50 mg orally per day every 3 to 4 days.
- Maximum dose:
- Inpatients: 600 mg/day
- Outpatients: 400 mg/day

Do not take Trazodone tablets:
-if you are allergic to trazodone hydrochloride or any of the other ingredients of this medicine. The signs of an allergic reaction include a red and lumpy skin rash, swallowing or breathing problems, swelling of your eyelids, lips, face, throat, or tongue, unexplained high temperature (fever), and feeling faint
-if you have recently had a heart attack
-if you are a heavy drinker or if you are taking sleeping capsules
-if you are under 18 years of age.
-Do not take trazodone if any of the above applies to you. If you are not sure, talk to your doctor or pharmacist prior to taking this medicine.

Firstly it's vital to know that you are not HIV positive. For this reason you will need to have testing for hiv before starting PrEP to make sure you are not already HIV positive. Truvada is also associated with some changes in kidney function so renal tests need to be ordered before staring this medication. There is some evidence that there can be reduced bone density. We recommend lettin gyour doctor know if you have any bone issues, as well as getting plenty of calcium rich foods, vitamin D as well as weight bareing exercise. Some people have diarrhoea, bloating and gastric side effects when starting Truvada, this settles quite quickly in most cases.

In my Instagram poll, nearly everyone taking Descovy reported receiving the drug effectively for free, largely due to Gilead’s copay coupons. However, as with anything in life, nothing really ever is free. More patients taking Descovy rather than generic Truvada means higher overall spending by insurance companies. This eventually comes back to haunt us in the form of higher premiums, and is why we shouldn’t simply adopt a costlier drug like Descovy without good reason to do so."

Everything has a good side and a bad side, even Tylenol. PrEP is no different. The most common thing that people can experience when you first taking the medication, your body knows something new is going on. And you have a little upset stomach, a little nausea, you just feel queasy. It’s not everyone. It’s probably one to two percent of people experience this, but generally after the first few days that passes. There are some people who have noticed headaches when they start the medication as well. Again, usually those headaches are resolved by over-the-counter medications, like Tylenol or Motrin. So that’s the most common thing that happens, it doesn’t happen to everyone but I want you to be aware of it because I don’t just want you to stop because usually if you can power through and take it consistently, that goes away.


Gilead has 2 medications available for HIV preexposure prophylaxis. Discussed are the similarities and differences between the two medications and a review of the available literature. Enjoy!

Firmagon is a type of hormone therapy used in the treatment of advanced prostate cancer.
The active ingredient in Firmagon, degarelix, is a type of medicine called a gonadotrophin releasing hormone antagonist. It acts on the pituitary gland in the brain.
