“The risks are completely different depending upon a variety of issues including the history of prior stroke, heart failure, high blood pressure, diabetes, vascular disease, age, and gender,” says Lee Memorial Health System cardiologist Dr. Richard Chazal.
With the help of a risk calculator, doctors assess each patient. Much research and development is being aimed towards AFib, but the simple truth is, that one of the most basic, time-tested treatments remains the best first line approach.
“In 2014, the American College of Cardiology and the American Heart Association released guidelines on treatment of patients with atrial fibrillation. In most cases, patients with minimal symptoms are recommended to be treated with control of the heart rate and also anticoagulant blood thinners to reduce the likelihood of having a stroke,” says Dr. Chazal.
At the same time advances are being made in surgical treatments for AFib, R&D is put to use in developing new drugs. These medications are easier to take than in years past. The hope is more people who need them will find drug therapy easier to swallow.

“Their advantage is a reduction in the likelihood of interaction with other medications, a reduction in the likelihood of interaction with certain foods or beverages that could be problematic with warfarin or Coumadin, and importantly, the lack of need for regular blood test to monitor the control of the anticoagulant,” says Dr. Chazal.

A drug recommended to use to treat and prevent only life-threatening ventricular arrhythmias due to potentially serious side effects (generally not recommended for less severe arrhythmias). It is taken by mouth or intravenously.

It is a non-selective beta-adrenergic receptor blocker with class II (beta-blocker) and III (potassium channel blocker) antiarrhythmic properties. It is a "pure" potassium channel blockers which produces little or no other cardiac ion channel electrophysiologic effect, as well as dofetilide and possibly ibutilide.

It can cause life-threatening ventricular tachycardia associated with QT interval prolongation. To minimize the risk of drug-induced arrhythmia, give it by oral administration in a facility that can provide cardiac resuscitation and continuous electrocardiographic monitoring.
Do not use it if baseline corrected QT is 450 msec or greater. If the QT interval prolongs to 500 msec or greater, reduce the dose, lengthen the dosing interval, or discontinue the drug.

It is nearly completely absorbed after oral administration and undergoes essentially no first-pass hepatic metabolism, resulting in its absolute bioavailability at 90-100%.
It is primarily eliminated by renal excretion, with approximately 80-90% of a dose being excreted unchanged in the urine, and a small amount is excreted unchanged in the feces.
Calculate creatinine clearance to determine appropriate dosing.

Carisoprodol is a skeletal muscle relaxant that is most commonly known as the brand name Soma. The lesser known brand name is Vanadom. This drug has benefits in treating discomfort experienced from acute painful skeletal muscle conditions over a 2-3 week duration of therapy. The benefits beyond a short term duration (2-3 weeks) has not been proven. There are two strengths in a tablet form being 250 mg and 350 mg. The medication is dose qid with tid in addition to at bedtime. Carisoprodol forms the metabolite meprobamate which has anxiolytic and sedative effects. Since CYP2C19 function is crucial to the metabolism to meprobamate it is important to be aware of drugs that may affect CYP2C19 function along with other factors such as race and gender. Asian and African Americans are of note since there is higher prevalence of poor CYP2C19 metabolizer function. Also females generally have a higher exposure to carisoprodol but not to the metabolized form of meprobamate. When monitoring patients it is important to be aware of signs of misuse and abuse along with assessing their mental status along with their level or relief regarding pain and muscle spasms.
