POTS is characterised by orthostatic intolerance. This means that patients feel significantly worse when they adopt an upright posture compared to when they are lying down. To try and find effective treatments, it is vital that we try and understand the physiology of what happens in the body when normal healthy people stand up.

When we stand up, blood in our vessels tends to get sucked down into our legs because of the effect of gravity. This means that there is less blood getting to our brain and ordinarily we would all pass out and fall because of a shortage of blood to the brain. This does not happen because two reflex mechanisms come into action which serve to maintain the blood flow to our brains. The first is regulated by stretch receptors in our blood vessels in the legs which try and constrict the best they can to prevent blood from pooling in the legs. Secondly the heart rate increases to try and maintain adequate blood flow to the brain. In some patients, the leg vessels don’t constrict as well and therefore they are not able to contribute to increasing the blood flow to the brain and therefore the body has to compensate for this by increasing the heart rate even further. As the heart rate goes up excessively the heart doesn’t have as much time to fill with blood and therefore pumps out less blood than it should which exacerbates the situation.

If we can in some way reduce the stretching of blood vessels especially in the lower limbs when we adopt an upright posture, it means that there is relatively more blood going round and less blood pooling which can improve symptoms.

Midodrine works by increasing the tone of our peripheral blood vessels and therefore reduces the likelihood of pooling of blood. It doesn’t really directly affect the heart as such but if there is more blood getting to the heart then that means that every heart beat is more effective and that can have the effect of improving orthostatic tolerance. Interestingly though this can also have an unwanted effect on the bladder. Because of the effect of the agent on tone, the bladder does not stretch as much and therefore bladder emptying is delayed.

Benzodiazepines are commonly used to treat anxiety disorders and sleep disorders. They are thought to exert their effects in the brain by acting at receptors for the neurotransmitter gamma-aminobutyric acid, or GABA. In this video, I cover the the mechanism of action for benzodiazepines.

Welcome to 2 minute neuroscience, where I explain neuroscience topics in 2 minutes or less. In this installment I will discuss benzodiazepines.

Benzodiazepines are a class of drugs named for their chemical structure that are commonly used to treat anxiety disorders and sleep-related disorders. They include well-known drugs like valium, xanax, and klonopin. There are dozens of drugs in the benzodiazepine class, but the mechanism by which they all exert their effects is thought to be similar.

The sedating and anxiety-reducing effects of benzodiazepines are believed to be attributable to the drugs’ actions at receptors for the neurotransmitter gamma-aminobutyic acid, or GABA. In particular, benzodiazepines act at a subtype of GABA receptors called the GABAa receptor; GABAa receptors that also bind benzodiazepines are sometimes called benzodiazepine receptors.

When benzodiazepines bind, or attach, to the GABA receptor, they bind at a location separate from where GABA itself binds, and exert an influence over GABA binding. This type of action is called an allosteric effect, and in the case of benzodiazepines it results in increased action at the GABA receptor. There is not complete consensus on exactly how benzodiazepine binding affects activity at the GABA receptor but there is evidence to suggest that it increases the likelihood that GABA binding will activate the receptor and/or increases the effect that GABA has when it binds to the receptor.

That effect is to open an ion channel and allow the passage of negatively charged chloride ions into the neuron. This influx of negatively charged ions pushes the membrane potential further from zero, or hyperpolarizes it, and makes it less likely the neuron will fire an action potential. This type of neural inhibition is the basis for the effects of benzodiazepines, for by inhibiting the activity of neurons that make up networks involved with anxiety and arousal, the drugs are able to produce calming effects.

Are you worried that your medication is causing you to lose hair? Do you want to know what to do about it? This article will discuss everything you want to know about metoprolol-induced hair loss and how to reverse it.

Alopecia is a rather rare (yet rather real) side effect for some people taking metoprolol. Metoprolol is the generic form of the brand name drugs Lopressor, Metoprolol Succinate, Metoprolol Tartrate and Toprol XL.

Metoprolol is a beta-blocker used to treat hypertension (high blood pressure), angina (chest pain) and other heart-related conditions.

Because hair loss is such an uncommon side effect of metoprolol, your doctor may not make mention of it. Some doctors may outright tell you that the medication is not to blame for your thinning hair.

However, there have been studies suggesting that certain drugs, including beta-blockers, may induce alopecia. WebMD and The Mayo Clinic list hair thinning as a rare side effect of metoprolol.

The American Hair Loss Association also mentions metoprolol as a drug that may cause hair loss.

According to eHealthMe – a website that presents a statistical analysis of drug information that are reported to the FDA – less than 1% of people with side effects from metoprolol experience hair loss.

Though this information cannot be construed as concrete fact, it does seem to show a connection (though minute) between hair fall and metoprolol.

According to their data, thinning and alopecia due to metoprolol seems to be most common among women ages 60+.

It’s important that you research your medication to have an understanding of the possible side effects, even the rare ones.

You know your body better than anyone. If you feel your medication is causing you to lose hair, be firm with your doctor about looking further into it.

How Does Metoprolol Cause Hair Loss?
There are two types of hair loss brought on by medications: anagen effluvium and telogen effluvium.

Anagen effluvium is commonly associated with chemotherapy or other such drugs. Telogen effluvium is the most common type of non-chemotherapy drug-induced alopecia (i.e. your friend, metoprolol).

Drug-induced telogen effluvium causes a premature interruption of hair growth, causing the hair follicles to go into their resting phase (telogen) and fall out too early.
