*Additional Proton Pump information: The proton pump that we talked about in this video is more specifically a Hydrogen and Potassium Proton Pump. It works by using ATP as energy, to essentially push Hydrogen into the stomach which binds to chloride to form hydrochloric acid. And during the process, it pulls Potassium out of the stomach. This is why it is often called the Hydrogen Potassium ATPase proton pump. Even this is a simplified explanation of how the H+/K+ ATPase pump works!

- Lansoprazole belongs to a category of drugs called proton pump inhibitors. It suppresses the secretion of gastric acid by combining with an enzyme on the gastric parietal cells called H+/K+-ATPase (proton pump), which is the final pathway for acid secretion. And this decreases hydrogen ion transport into gastric lumen.

- Lansoprazole is available as 15 and 30 mg delayed-release capsules. And also available as enteric coated granules for oral suspension, and delayed release orally disintegrating tablets.

- It is used for duodenal ulcers, gastric ulcers erosive esophagitis and gastro-oesophageal reflux disease.
- It is also used for the treatment of Helicobacter pylori in combination with amoxicillin and clarithromycin.

A Lamotrigina é um anticonvulsivante da classe das feniltriazinas, com menos efeitos colaterais que outros fármacos. Ela age bloqueando canais de sódio e cálcio, reduzindo a liberação de glutamato. Isso ajuda a controlar a hiperatividade neuronal e prevenir crises convulsivas.

A Lamotrigina é eficaz na epilepsia e no transtorno bipolar, principalmente na prevenção de episódios depressivos. Também é usada off-label para TEPT, dor neuropática central e dermatotilomania, com evidências promissoras. Seu efeito se deve à modulação do glutamato, ajudando na estabilização do humor e controle de sintomas compulsivos.

A Lamotrigina pode causar reações cutâneas em 6 a 10% dos pacientes, desde erupções leves até condições graves, como Síndrome de Stevens-Johnson e Necrose Epidérmica Tóxica, principalmente em crianças e ao usar Valproato. Também pode interagir com outros medicamentos, como anticonvulsivantes e contraceptivos orais, exigindo ajustes.

A Lamotrigina é um medicamento com múltiplos mecanismos de ação, influenciando vários processos químicos. Ela é um bloqueador de canais de cálcio, inibindo o glutamato, que é um neurotransmissor, e de alguma forma gerando uma estabilização nas redes elétricas cerebrais. É quase como um "curto circuito" no cérebro, que é protegido através desse medicamento.

Apesar de não ter sido inventada para o transtorno bipolar, a Lamotrigina é muito utilizada, especificamente no transtorno bipolar tipo II. Esta classificação está relacionada a um transtorno com mais quadros depressivos.

Videoconsejo sanitario sobre Lamotrigina, para qué sirve este medicamento, cuándo y cómo debemos tomarlo. Videoconsejo impartido por el farmacéutico Juan Iniesta, elaborado por el Consejo General de Colegios Farmacéuticos y producido por Medicina TV.

Lamotrigina es un fármaco antiepiléptico que actúa bloqueando los canales de sodio dependientes de voltaje presentes en las neuronas. También inhibe la liberación de glutamato, el principal neurotransmisor con efecto excitatorio del sistema nervioso central, que juega un papel clave en la generación de las crisis epilépticas.

En base a este mecanismo, el fármaco está indicado en el tratamiento de distintos tipos de crisis convulsivas, como las crisis de ausencia, las crisis parciales o las crisis generalizadas, en adultos y niños a partir de 2 años.

También está indicado en la prevención de los episodios depresivos en pacientes adultos con trastorno bipolar de tipo 1, en el cual se alternan episodios depresivos, maniacos y mixtos.

Los medicamentos con lamotrigina se encuentran disponibles en las farmacias comunitarias y únicamente se dispensan con la correspondiente receta médica.

Estos medicamentos se presentan en forma de comprimidos masticables o dispersables, que, por tanto, pueden masticarse, dispersarse en una pequeña cantidad de agua o tragarse enteros.

Los medicamentos disponibles contienen dosis de lamotrigina de 2, 5, 25, 50, 100 o 200 mg por comprimido.

La dosis a emplear y la posología que paute el médico dependerá de la edad, el peso y de la presencia o ausencia de tratamientos concomitantes, ya se trate de otros antiepilépticos o de cualquier otro fármaco que pueda afectar al metabolismo de lamotrigina.

Por ejemplo, valproato, que es también un antiepiléptico puede aumentar la concentración de lamotrigina en el organismo; mientras que carbamazepina o el antiobiótico rifampicina, pueden hacer que ésta disminuya.

No obstante, el tratamiento suele comenzar con dosis bajas, que se incrementan progresivamente hasta encontrar la dosis eficaz más baja.

En caso de que sea necesario dejar de administrar el fármaco, la retirada también se realizará de forma gradual, pues una retirada brusca podría dar lugar a la aparición de crisis convulsivas.

Ante un embarazo, si clínicamente se considera necesario el tratamiento con lamotrigina, se deberá usar la menor dosis terapéutica posible.

Como cualquier fármaco, lamotrigina puede producir algunos efectos adversos. Entre los más frecuentes se encuentran dolor de cabeza, erupción cutánea, cansancio, somnolencia y molestias gastrointestinales.

Generic name: Lamotrigine.
Brand names: Lamictal.
Available as chewable and soluble tablets.

How to use it?
The chewable tablets can be chewed. And the solvable tablets should be solved in a glass of water.
In addition, you can take these tablets at any time, however it is recommended to take them at fixed times. This will make it less likely that you forget a dose.

When to use it?
Lamotrigine blocks sodium channels in your brain, stabilizing the neuronal membranes and thus preventing the release of glutamate.
- Epilepsia
- Bipolar disorder

What is the correct dosage?
The precise dose will depend on your specific indication of use, the side effect you might experience and the severity of your symptoms.Therefore always use the precise amount and frequency your doctor prescribed you.

- When treating epilepsy in Adults the initial oral dose is 25 mg/day. This can be slowly increased up to a maintenance dose of 100-200 mg/day. The maximum dose would be 500mg/day.
- When treating bipolair disorder in Adults the initial oral dose is 25mg 1x/day, which you doctor will slowly increase up to a maintenance dose of 100-400 mg/day.

What are common the side-effects?
- Very common (10%): rash and headache.
- Common (1-10%): irritation, drowsiness, insomnia, dizziness, tremor, nausea, vomiting, diarrhoea, dry mouth, pain and fatigue.
- Uncommon (0.1-1%): double vision, blurred vision and hair loss.
- Rare (0.01-0.1%): Stevens-Johnson syndrome, meningitis and conjunctivitis.
- Very rare (0.01%): Blood abnormalities, hypersensitivity reactions, rashes and several other severe side effects.

Safety:
First of all be careful to combine lamotrigine with alcohol. As this might increase it’s side effects.
In addition, it can be dangerous to drive while using this drug. As it can cause you to become drowsy, sleepy and/or dizzy. If you experiencing any of these side effects then please do not drive.
Furthermore, lamotrigine can have interactions with other drugs your are taking. always discuss this with your prescribing doctor.

Pregnancy and Lactation:
Research shows that use of lamotrigine could be used safely. However, there are some exceptions. Therefore always discuss this with your personal doctor.

Bipolar disorder is a serious mental illness that involves recurring episodes of depression and either hypomania or mania. It normally requires medication to treat the acute episodes as well as keep them from coming back once they resolve. But there are people who go periods of time without medication.

However, if you go too long without meds while you are having an episode, the period of time you spend in those episodes without treatment makes future episodes harder to treat. It’s called the kindling effect.

A therapy that can help bipolar disorder is interpersonal and social rhythm therapy. This therapy has been shown to reduce the time of episodes and delaying the onset of future episodes. This can be useful if you are trying to stay off medication in your interepisode period. Interpersonal therapy focuses on problems in your relationships and social rhythm therapy helps you establish regular daily routines.

Mechanism of Action of lamotrigine:
The precise mechanism(s) by which lamotrigine exerts its anticonvulsant action are unknown. In
animal models designed to detect anticonvulsant activity, lamotrigine was effective in preventing
seizure spread in the maximum electroshock (MES) and pentylenetetrazol (scMet) tests, and
prevented seizures in the visually and electrically evoked after-discharge (EEAD) tests for
antiepileptic activity. Lamotrigine also displayed inhibitory properties in the kindling model in
rats both during kindling development and in the fully kindled state. The relevance of these
models to human epilepsy, however, is not known.
One proposed mechanism of action of lamotrigine, the relevance of which remains to be
established in humans, involves an effect on sodium channels. In vitro pharmacological studies
suggest that lamotrigine inhibits voltage-sensitive sodium channels, thereby stabilizing neuronal
membranes and consequently modulating presynaptic transmitter release of excitatory amino
acids (e.g., glutamate and asp)

Typically, people with bipolar disorder require a combination of medications that you take at the same time. Why? Because the medications treat different aspects of the disorder. The main four phases that we target in treating bipolar disorder are Acute depression, Acute mania, mixed states and the maintenance phase.

And acute means it’s the active state that you're in and it's relatively recent. maintenance phase is the in between episodes when things have stabilized.

Not all of the medications work for every phase. So you could be put on a medication because you're manic and then once that mania goes away you can have a depressive episode pop up because that original mania drug isn't doing a good enough job keeping your depression away.

Taking any medication long term can be scary, and psychiatric medications are no exception. I’ve been on Lamictal (aka Lamotrigine) for ten years, and this is my story. I talk in depth about long term side effects and whether it is still effective after all this time… which is typically what I get asked about when it comes to my treatment since it’s been a decade now.

But Lamictal is amazing and it saved my life, and it’s saved a lot of people’s lives. And while long term side effects can cause one to worry, it’s important to remember that it’s sometimes that specific medication that gave you the ability to get a long term side effect in the first place.

So, put it this way, I’m a very grateful person.
