Hydromorphone drug interactions are mostly additive effects. Drugs that cause sedation or constipation can have additive effects on hydromorphone.
Be extremely careful with hydromorphone dosage forms. There are numerous different concentrations and strengths. I discuss this in this episode.

This chart for opioid dose conversions (thank you GrepMed) is extremely helpful - but it can still often be clunky to use when you are making multiple dosing conversions. In this video I show a simpler way to quickly convert between oxycodone and dilaudid. Keep in mind that not all institutions agree on these opioid equivalency charts and so it may vary a little based on your institution. Hopefully this simple little trick helps when you need to make that quick conversion to figure out how much relative oxycodone you're giving someone when you've ordered that 0.5mg dose of IV dilaudid!

Heroin, fentanyl, carfentanil are all deadly drugs when the dose exceeds your body’s ability to metabolize them.
Anyone who uses these opioids knows the risks and they point out that won’t stop them from using. The need for the drug overrides caution. Withdrawal is painful and extremely hard to achieve if you are living on the streets, scrounging for food and trying to scrape together the 25 to 35 dollars a day you need to support your habit.
In BC, hydromorphone is available to a limited number of chronic heroin users. The SALOME and NAOMI trials provided evidence that treatment is effective in stabilizing users.
The BC Centre for Substance Use recently issued guidelines that are a continuum of care that include counselling, detox, withdrawal management services and the administration of injectable and oral treatment through medication.
Colin Ross is fortunate. He has been an addict for about 20 years. Now he’s receiving hydromorphone two times a day – a treatment he says allows him to stabilize and consider the future. We sat down with Colin for a Conversation That Matters about the opioid crisis, the hydromorphone IV treatment program, mental health, housing and the potential he hopes to realize.

WHAT ARE TOPICAL STEROIDS:
Topical steroids work by reducing inflammation in the skin. They are used for various skin conditions including eczema. (Steroid medicines that reduce inflammation are sometimes called corticosteroids. They are very different to the anabolic steroids which are used by some bodybuilders and athletes.)
There are many types and brands of topical steroid. However, they are generally grouped into four categories depending on their strength - mild, moderately potent, potent and very potent. There are various brands and types in each category. For example, hydrocortisone cream 1% is a commonly used steroid cream and is classed as a mild topical steroid. The greater the strength (potency), the more effect it has on reducing inflammation but the greater the risk of side-effects with continued use.
Creams are usually best to treat moist or weeping areas of skin. Ointments are usually best to treat areas of skin which are dry or thickened. Lotions may be useful to treat hairy areas such as the scalp.
HOW TO APPLY TOPICAL STEROIDS:
Always follow your healthcare professionals instructions on how much to apply and how often.
Most people only need to use the medication once or twice a day for a week or two, although occasionally your doctor may suggest using it less frequently over a longer period of time.
The medication should only be applied to affected areas of skin. Gently smooth it into your skin in the direction the hair grows.
If you're using both topical corticosteroids and emollients, you should apply the emollient first. Then wait about 15 minutes before applying the topical corticosteroid.
FINGER TIP UNITS:
The amount of topical steroid that you should apply is commonly measured by fingertip units (FTUs). One FTU is the amount of topical steroid that is squeezed out from a standard tube along an adult's fingertip. (This assumes the tube has a standard 5 mm nozzle.) A fingertip is from the very end of the finger to the first crease in the finger.
One FTU is enough to treat an area of skin twice the size of the flat of an adult's hand with the fingers together.
Two FTUs are about the same as 1 g of topical steroid. For example, say you treat an area of skin the size of eight adult hands. You will need four FTUs for each dose. (This is 2 g per dose. If the dose is once a day, then a 30 g tube should last for about 15 days of treatment.)

SIDE EFFECTS:
Short courses of topical steroids (fewer than four weeks) are usually safe and usually cause no problems. Problems may develop if topical steroids are used for long periods, or if short courses of stronger steroids are repeated often. The main concern is if strong steroids are used on a long-term basis. Side-effects from mild topical steroids are uncommon.
Side-effects from topical steroids can either be local or systemic. Local means just affecting that bit of skin and systemic means affecting the whole person.

Welcome back to How To Medicate and welcome to this video where we will cover everything you need to know about hydrocortisone. This video is part of a video series on your skin. We will cover all kinds of skin related problems and drugs. So for more skin related videos check out this playlist:

Generic name: Hydrocortisone.
Brand names: Acecort, Ala-cor and Plenadren.
Available as an ointment, cream, tablet, capsule and injection.
How to use it?
Tablets or capsules: Swallow them whole with half a glass of water. Don’t break it and don’t chew on it. Take it before or after you ate something and do so at fixed times.
Ointment or cream: don’t apply it too thick, nor too thin. Wash your hands after you aplied it.
Duration of treatment?
The duration of the treatment depends on the indication for use: it can range from 2-3 weeks up to some cases where patients need to use hydrocortisone for life.
It is important that you do not interrupt the treatment and finish it completely.
When to use it?
Ointment and cream are used for skin-related problems like: Eczema, severe itching, inflammation of the skin and allergic skin reactions.
Tablets, capsules and injections are used as treatment in the following cases: problems with the production of corticoid hormones, severe increases of COPD or asthma or severe allergic reactions
What is the correct dosage?
Always use the dosage and frequency prescribed by your own doctor.
What are the side-effects?
Tablets and capsules: Very common (more than 10%): dizziness, headache, diarrhea, fatigue. Uncommon (0.1–1%): abdominal pain, nausea, itching, rash.
Ointment and cream: Thinning of the skin, allergic reaction, whitening of your skin and various other skin problems.
Safety:
It is safe to use with alcohol (in moderation), eat any type of food and drive your car while using this drug. However, hydrocortisone can have interactions with other drugs, which can be dangerous. Therefore always ask your prescribing physician if hydrocortisone is safe for you.
Furthermore if you are using this drugs for a longer time, than it can have additional risks when you quit suddenly or are exposed to very stresfull situations. Discuss this with your prescribing doctor.
Pregnancy and Lactation:
Hydrocortison can be dangerous when pregnant or breast feeding. So if you are pregnant or breastfeeding discuss this very clearly with your doctor.

What are corticosteroid creams?
Corticosteroid creams or CS are the mainstay of treatment. They are safe, provided you follow the instructions given to you by your dermatologist. There are different types of steroids based upon their concentration, potency & vehicle.
CS suppresses skin inflammation & this reduces the amount of damage to your pigment cells. One of the side effects of corticosteroids is excess hair production or hypertrichosis. This is a wanted outcome as pigment is derived from the cells that hide within the hair follicles.

Are steroids safe?
Yes, however you will need to adhere to the directions. I choose an appropriate steroid preparation based upon the site of vitiligo. For acral patches, the concentration of corticosteroids is higher compared to facial vitiligo.
Side effects of steroid ointments & creams include- thinning of skin, acne eruptions, perioral dermatitis, bruising & systemic absorption. These side effects can be mitigated by following the instructions on the prescription.

Corticosteroids reduce inflammation & can promote hair growth as a side effect (hypertrichosis). This increased hair growth can add more pigment cells in patches of vitiligo. Coupled with pigment stimulation light, re-pigmentation can be accelerated.

Q&A Hydrocortisone side effects and how to improve adrenal function http://drchristian.com/blog
Dr. Alan Christianson here. This is going to be a very tactical podcast about improving your cortisol metabolism. In my new book, THE ADRENAL RESET DIET, I call this being “crashed.” I talk about different levels of adrenal distress, and “crashed” is the lowest level. At this level, people can be horribly fatigued, depressed, have aches and pains and just not be functioning well. It can also badly affect sleep, weaken the immune system and probably create a long-term risk. It’s a big deal and very important to correct.
My patient’s previous doctors felt it would be helpful for her cortisol to be higher, so they gave her hydrocortisone pills. Ironically, taking cortisol was not a path back to health. Hydrocortisone is very similar to the cortisol your body makes, so it increased her cortisol levels. The problem was she put on 20 pounds in the first few weeks. This had never happened to her. Her weight had fluctuated no more than four pounds all of her adult life. She was in her mid-40's, and her weight had remained the same since high school–until she took the hydrocortisone. Besides the weight gain, she had severe acne, fluid retention, thinning hair and severe anxiety.
So, why didn't it help to take the cortisol? Well, here is what is going on: When you have an adrenal disease, the adrenal glands cannot do what the body wants them to do. The biggest adrenal disease is Addison's disease, which is an autoimmunity of the adrenals. The immune system attacks the adrenals and breaks them down to where they cannot produce enough hormone for your needs. There is a signal from the pituitary gland to the adrenals, telling them to make hormones. This signal is a hormone the pituitary gland releases called ACTH or adrenocorticotropic hormone. In cases of Addison's disease, there is no cortisol being made, and there is a very high level of ACTH. So, your brain is yelling at your adrenals to work, and they are not working.
When you have adrenal dysfunction, like my patient had, it is a different story. I address adrenal dysfunction in my new book, where I cover the levels of adrenal distress: crashed, wired or tired, or stressed. If we look at the ACTH level, it will not be elevated. There is low cortisol, but the body wants low cortisol. In my patient’s case, her system did not really want the cortisol from her own body. She was capable of making it, and she did make it at certain times, but her body did not want it because it was really run-down and exhausted. So, it took rest, relaxation, rejuvenation and restoring her cycles and daily rhythms for her body to want to make cortisol again. Pouring cortisol into her body (through the pills) was fighting against what the body was trying to achieve. That is why she had so many, troubling side effects.
If you are crashed, take a look at the chapter on crashed in the new book and think about botanical medicines. Think about timing of light. Think a lot about rest and all the factors that give rise to it. Your blood sugar is a critical one, too. So, manage all the factors that raise your resiliency, and know you can do better and your cortisol production can come back again. If you take cortisol, you are not giving your body what it wants. You are giving your body what it is trying to avoid. You will also make it harder for your body to make more cortisol because you will pull away from your adrenals that much further.
