Indications of Levothyroxine: Levothyroxine is a thyroid supplement used to treat hypothyroidism. It can also treat certain types of thyroid cancer or goiters.
 
Mechanism of action of Levothyroxine: Levothyroxine is a synthetic form of T4. In hypothyroidism the body has low levels of T4, so this man made synthetic form of T4 will restore it to normal levels which restores function all over the body including metabolism, gluconeogenesis, protein synthesis, cell growth and development of the brain and central nervous system.
 
Therapeutic effect of Levothyroxine: Levothyroxine restores normal hormone balance by replacing the depleted thyroid hormone T4.

Tirosint is a T4 only thyroid medication but with some twists.

Many patients report significant improvement in hypothyroid symptoms when switching from T4 only medications like Levothyroxine and Synthroid over to Tirosint.

But how can that be? Especially when Tirosint is a T4 only medication?

This occurs for several reasons:

1) Inactive ingredients -

+ Levothyroxine and Synthroid contain MANY inactive ingredients (complete list in the video) which your body may be reacting to and limiting absorption. These inactive ingredients may be causing negative side effects that mimic hypothyroidism or limit your ability to reach the optimal dose.

Tirosint has 3 inactive ingredients and 1 active ingredient making it the "cleanest" thyroid medication (T4 only).

2) Decreased absorption -

+ Due to these inactive ingredients the absorption of Levothyroxine and Synthroid may be inferior to that of Tirosint. The fewer inactive ingredients means the more thyroid hormone that will be absorbed which means the better you symptoms will be. This is also very important for hypothyroid patients who have GI related conditions such as SIBO/SIFO, GERD and low stomach acid. Tirosint is more easily absorbed and utilized than other forms of thyroid hormone due to these reasons.

Who should consider using Tirosint?

- Patients with GI related issues
- Patients who don't tolerate Synthroid or Levothyroxine
- Patients with multiple sensitivities or food allergies
- Patients with Leaky gut or increased intestinal permeability
- Patients who don't tolerate T3 containing medications like NDT and/or liothyronine/cytomel
- Patients who are extremely sensitive to medications and supplements
- Patients who have experienced some improvement on Levothyroxine or Synthroid but still have room for improvement

Synthroid

Synthroid is a synthetic thyroid hormone—T4. T4 has to be converted into T3 to be active. Another name for T4 is levothyroid or levothyroxine. 

• It takes 4 weeks to start making changes. 
• If you're not converting T4 to T3, Synthroid is not going to work.

Armour Thyroid

This is a natural extract from a pig's thyroid. 

• It contains T4, T3, and calcitonin (an important thyroid hormone). 
• It takes 2 weeks to start making changes.

Some symptoms of a hypothyroid:

• Tired
• Cold
• Hair loss
• Mental sluggishness
• Weight gain 

*I believe most hypothyroid problems are secondary to something else. 

Other natural thyroid extracts: 
• Nature Throid
• NP Thyroid 

If you have a negative reaction to T4, T3, or calcitonin, and you have extreme fatigue or flu-like symptoms—you may have Hashimoto's. If you have Hashimoto's and you take thyroid extracts, it will make you very tired. Or, you may also be having a reaction because of some of the inactive ingredients in the product.

In this video, I go over the complete list of Levothyroxine side effects ranging from weight gain to hair loss and everything in between.

Yes, Levothyroxine can cause hair loss and it can also cause weight GAIN. How can that be if levothyroxine is supposed to boost your metabolism?

In this video I explain how the metabolism of thyroid hormone in your body from T4 to reverse T3 may help explain why many people actually gain weight on this particular medication.

I also go over the recommendations I give to patients if this happens to them.

I also go over how to determine which thyroid medication is right for YOUR Body and how to dose your levothyroxine.

When patients have reactions to levothyroxine it's from 1 of 3 reasons typically:

1) they are reacting to a filler ingredient or a dye within the medication itself.

2) they are converting or metabolizing the thyroid hormone into by products that don't promote proper activity

3) they are getting too much thyroid hormone for THEIR body (this one is less common)

The side effects when patients react to fillers or dyes include reactions like facial swelling, edema, hives, itching sensation, etc.

The side effects from improper metabolism include reactions like weight gain, hair loss, slower than normal metabolism, cold hands/extremities, etc.

The side effects from TOO much medication include hot flashes, tachycardia or rapid heart rate, palpitations, hair loss, intolerance to heat.

The treatment for each of these reactions is quite different and I explain what to do depending on how you are reacting.

And there are many people out there right now, perhaps you, who are taking either too much or too little. The side effects associated with taking too much or too little are almost the complete opposite.

If you've been taking levothyroxine or Synthroid and you are NOT feeling better then this video is for you.

While it is more common for people to not take ENOUGH levothyroxine (meaning their dose is too small) there are still plenty of people out there who are taking too much levothyroxine.

When you take too much levothyroxine you will start to experience some of the symptoms of hyperthyroidism as well as a few added symptoms associated with the medication itself.

Signs your dose of levothyroxine may be too high include:

#1. Irritability
#2. Hair loss
#3. Anxiety
#4. Insomnia
#5. Muscle weakness
#6. Fertility issues
#7. Rapid heartrate
#8. Palpitations
#9. Diarrhea or loose stools
#10. Heat intolerance

2. Too much estrogen can be dangerous to the cardiovascular system
• Estrogen (estradiol) is typically at higher levels when someone has a heart attack, and when men have atherosclerosis. 

3. Estrogen dominance can cause an enlarged prostate
• Estrogen is an anabolic hormone. It grows things. 

What causes high estrogen in men? It could potentially be because of:

• Soy products 
• Endocrine disrupters 
• Dairy 
• Too much aromatase (This could be the #1 reason, and it’s likely because of belly fat)

What to do:
• DIM 
• Stinging nettle root

Why I am rethinking oral progesterone for HRT | Felice Gersh, MD Side Effects 3 Views • 5 months ago

I wanted to better understand the relationship between progesterone and sleep. On the one hand, many women feel like they can't do cyclic hormones because they need nightly progesterone to sleep. And yet, in a reproductive aged woman, high progesterone as seen towards the end of the luteal phase right before menstruation begins is associated with higher rates of insomnia and generally worse sleep quality. 

What I learned is making me rethink oral progesterone. A very high percentage of oral progesterone, when it is metabolized through the liver, gets converted to allopregnanolone, which is both a sedative and an antidepressant. In reproductive aged women, some progesterone gets converted to allopregnanolone, but not nearly at the rates that oral progesterone does.

This means (a) women are getting much lower doses of actual progesterone than we think and (b) oral progesterone is really functioning more like a drug than a physiologic hormone. Maybe this is helpful for women who would benefit from a sedative-antidepressant. But I don't think we should be drugging women without full understanding or consent. And, women aren't getting adequate progesterone. 

I'm not ready to change my prescribing just yet. My patients taking cyclic oral progesterone are doing really well. But I do want to be transparent that this is something I am looking into and considering.

Vivelle-Dot is also known by the generic name: Estradiol.

Vivelle-DOT comes in 0.025mg, 0.0375mg, 0.05mg, 0.075mg, and 0.1mg per 24 hours transdermal patches.
Vivelle-DOT is most commonly used for the purpose of adding estrogen to the body when the ovaries have been taken out or do not work properly and also to prevent or lower the signs of menopause. It may be given to you for other reasons, talk with your doctor or pharmacist. No matter what your doctor has you taking Vivelle-DOT for it, this patch should be applied to clean, dry skin below the waistline two times weekly. Change the location of the patch when applying a new one and only wear one patch at a time.
If you miss a dose of Vivelle-DOT:
• Put on a missed patch as soon as you think about it after taking of the old one

The best way to store Vivelle-DOT:
• Store at room temperature.
• Store in a dry place. Do not store in a bathroom or refrigerator
• After you take off a skin patch, be sure to fold the sticky sides of the patch to each other.
• Keep all drugs in a safe place. Keep all drugs out of the reach of children and pets.
• Check with your pharmacist about how to throw out unused drugs.

Usual doses of Vivelle-DOT range from 0.025mg/24 hours to 0.1mg/24 hours.

Some common side effects with Vivelle-DOT include headache, pain, depression, anxiety, dizziness, and breast tenderness.

Do not use Vivelle-DOT patches if:
• You have or have a history of deep vein thrombosis or pulmonary embolism
• You have a history of a stroke or heart attack
• You are pregnant
• You have breast cancer
Vivelle-DOT patches come with the US Black Box warnings of:
• Endometrial cancer
o There is an increased risk of developing endometrial cancer if a woman who has a uterus uses Vivelle-DOT patches
• Cardiovascular disease
o Estrogen therapy, such as Vivelle-DOT patches, should not be used alone for the prevention of cardiovascular disease in postmenopausal women. If used alone, it can lead to increased risk of stroke and deep vein thrombosis (DVT).
• Breast cancer
o There is an increased risk of breast cancer when using estrogen plus progestin therapy.
• Dementia
o There is an increased risk of developing dementia if taking estrogen therapy alone in post-menopausal women.
When using Vivelle-DOT patches, remember it is important to tell your doctor pharmacist if you have:
• Signs of an allergic reaction, like rash; hives; itching; blistered, or peeling skin with or without fever
• Chest pain or pressure
• Shortness of breath
• Coughing up blood
• Very bad headache
• Weakness on 1 side of the body, trouble speaking or thinking, change in balance, or blurred eyesight
• Back pain
• A lump in the breast, breast soreness
• Breast pain
• Very bad skin reaction
