La duloxetina es un fármaco antidepresivo, que estimula la transmisión nerviosa. Así pues, se emplea en adultos para el tratamiento de la depresión, de los trastornos de ansiedad generalizada y en el tratamiento del dolor neuropático diabético, descrito como quemazón, dolor punzante, como pinchazos, escozor, o como un calambre eléctrico.

Actualmente las presentaciones comercializadas son de 30, 60, 90 y 120 mg en forma de cápsulas o comprimidos. La dosis debe individualizarse en función de la respuesta de cada paciente. La dosis inicial es de 30 o 60 mg diarios, y en aquellos casos en los que el médico lo considere necesario, la dosis podría incrementarse hasta 120 mg al día.

Durante el tratamiento de la depresión es muy importante que avises a tu médico si notas que los síntomas de la depresión empeoran. Igualmente, si tienes un familiar en tratamiento con duloxetina u otro antidepresivo, avisa a tu médico o farmacéutico si notas que tiene cambios de humor o del comportamiento, ya que esto podría ser un indicativo de un empeoramiento de la enfermedad.

Ten en cuenta que los efectos de este medicamento aparecen gradualmente, en un periodo de varias semanas. Si notas que los síntomas no mejoran con la dosis que te ha indicado tu médico, comunícaselo, pero no modifiques la dosis por tu cuenta, pues puedes sufrir una sobredosis. Por norma general, en la mayoría de las personas con depresión o ansiedad, el medicamento comienza a actuar a las dos semanas tras haber comenzado el tratamiento, pero pueden pasar entre 2 y 4 semanas hasta que comiences a sentirte mejor.

También es importante que no suspendas el tratamiento con duloxetina y otros antidepresivos de forma brusca ya que se podría desencadenar un efecto rebote, apareciendo una sintomatología de depresión más intensa. Consulta con tu médico cómo debes suspender el fármaco.

Tampoco se debe consumir alcohol durante el tratamiento con duloxetina porque puede empeorar los síntomas o los efectos adversos.

Cymbalta or Duloxetine is a medication that is used to treat mainly Depression and Anxiety but also is used to treat Fibromyalgia, Diabetic and other types of Neuropathy, Joint pain and even Hot Flashes related to menopause. Cymbalta (Duloxetine) is in the family of medications called Serotonin Norepinephrine Reuptake Inhibitors (SSRI). They influence the amount of Serotonin and Norepinephrine that is available to help with mood and other functions of the body.

With depression you tend to have symptoms like difficulty concentrating, remembering things or making decisions. Fatigue, feelings of guilt, worthlessness, helplessness or hopelessness. People experience sleeping problems, mood swings, a loss of interest in things that you once enjoyed, appetite changes, generalized pain, persistent sad or empty feelings and even suicidal thoughts or attempts.

Those who are going through anxiety you may have symptoms such as sudden and unexplained onset of a rapid heart rate, sweating, trembling, shortness of breath, pain in your chest, fear of dying, numbness and tingling throughout your body or just a generalized feeling of anxiousness all the time that you just can’t explain.

If you are experiencing these symptoms you need to seek help from a professional. Go see your doctor and get an accurate diagnosis to figure out a good treatment plan for you. Remember depression and anxiety are real medical conditions and should be treated just as diabetes or high blood pressure need to be treated. You shouldn’t feel any different treating any one of these medical conditions.

We'll give a good overview in the video of the common side effects but its important to remember that most side effects if any are transient and can be minimized by working with your doctor. We will discuss more concerning side effects such as serotonin syndrome as well.

Before you start taking Duloxetine, Tell your doctor if you have any allergies, if you have had any medical conditions this includes, glaucoma, high blood pressure, heart problems, kidney problems, history of seizures, bipolar disorder, diabetes. If you drink alcohol, tell your doctor how much you drink... more in video

If you are going to have surgery, tell the surgeon or anesthetist that you are taking this medicine

DO NOT take Duloxetine if you are taking another medicine for depression called a monoamine oxidase inhibitor (MAOI) or have been taking a MAOI within the last 14 days. Usually, if you are swapping from a different class of antidepressants,  there is a wash out period before you start on a new antidepressant. more in video...

Dependent on your condition, the doctor will prescribe the appropriate dose for you and adjust the dose along the way. Take Duloxetine at the same time every day with a glass of water, it doesn't matter if you take it with or without food. more in video


Side Effects
Contact your doctor immediately if you experience the following:

- you feel sleepy
- you feel nausea and vomiting
- have tremors, uncontrollable movements
- have a fit (seizure)
- euphoric - where you have excessive enthusiasm or excitement, or you feel restless.

While you are on duloxetine, make sure you keep all your doctor's appointment, the doctor would need to monitor your  blood pressure • heart rhythm changes •  thyroid and liver function

One very important thing to take note of is anti-depressants acts as a crutch to help you overcome things a little easier. While you are on this medication, it's crucial that you work towards self-help,  work towards changing the pattern you think, changing the way you communicate with yourself, so when you are off it, you are able to stand on your own.  Listen to motivational speakers on topics of depressions, how to better yourself in life. Talk to your doctor about psychotherapy, go for counseling sessions, meditation etc.

PARA QUÉ SIRVE

Dulcolaxo contiene bisacodilo, un fármaco para tratar el estreñimiento ocasional para adultos y niños mayores de 10 años. Solo debe usarse para aliviar el estreñimiento que se produce en momentos puntuales.

NO DEBES TOMAR DULCOLAXO (o consultar antes al médico) SI:
Sufres estreñimiento crónico
Tienes dolor abdominal de origen desconocido
Padeces alguna afección en el aparato digestivo o

CÓMO TOMAR ESTE MEDICAMENTO
En general se recomienda tomar un comprimido de Dulcolaxo (aunque se podrían llegar a tomar dos si con uno no se logra el efecto deseado).
Toma la pastilla entera, sin masticar y con un gran vaso de agua.

¿CUÁNDO ME HARÁ EFECTO?

Si lo tomas antes de acostarte, que es lo más recomendable, irás al baño pasadas unas 10 horas.
Si lo tomas por la mañana en ayunas, calcula que tardarás en ir al baño unas 6 horas.

PRECAUCIONES A TENER EN CUENTA CON DULCOLAXO
1-Los laxantes pueden generar dependencia, Dulcolaxo también.
Úsalo solo durante periodos cortos de tiempo.
Si no vas al baño a las 12 horas de tomar 2 comprimidos de Dulcolaxo o los síntomas persisten después de 7 días de tratamiento debes acudir al médico

2- Interacciones
Evita comer regaliz mientras estés en tratamiento con Dulcolaxo
La leche y los medicamentos para la acidez del estómago (como almax, omeprazol o ranitidina) deberás tomarlos o 2 horas antes o dos horas después de tomar Dulcolaxo.
Informa siempre a tu médico o farmacéutico si estás tomando otros medicamentos, también los que se adquieren sin receta y los remedios naturales.

EFECTOS SECUNDARIOS DE DULCOLAXO

Las reacciones adversas que se presentan con más frecuencia son:
-Dolor abdominal
-Diarrea
Si notas cualquier otra reacción adversa, aunque no aparezca en el prospecto, debes consultar con tu médico o farmacéutico.

Uses of Dulcolax
Dulcolax is used to treat constipation.
It may also be used to clean out the intestines before a bowel examination/surgery.
Dulcolax is known as a stimulant laxative.
It works by increasing the movement of the intestines, helping the stool to come out.
How to use Dulcolax
Take this medication by mouth as directed by your doctor.
If you are self-treating, follow all directions on the product package.
If you have any questions, ask your doctor or pharmacist.
Swallow this medication whole.
Do not crush, chew, or break the tablet or take it within 1 hour of antacids, milk, or milk products.
Doing so can destroy the coating on the tablet and may increase the risk of stomach upset and nausea.
Dosage is based on your age, medical condition, and response to treatment. Do not increase your dose or take this drug more often than directed.

Acetaminophen doxylamine dextromethorphan phenylephrine is available under the following different brand names: Alka-Seltzer Plus Night Cold Formula, Tylenol Cold Multi-Symptom Nighttime Liquid, Alka-Seltzer Plus Night Severe Cold, Cough and Flu, and Alka-Seltzer Plus Night Sinus Congestion, Allergy and Cough.


WHAT ARE THE DOSAGES OF ACETAMINOPHEN DOXYLAMINE DEXTROMETHORPHAN PHENYLEPHRINE?

Dosages of Acetaminophen Doxylamine Dextromethorphan Phenylephrine:

Capsule:

325mg.

Dosage Considerations – Should be Given as Follows:

WHAT ARE WARNINGS AND PRECAUTIONS FOR ACETAMINOPHEN DOXYLAMINE DEXTROMETHORPHAN PHENYLEPHRINE?

Warnings:

This medication contains:
acetaminophen doxylamine dextromethorphan phenylephrine. Do not take Alka-Seltzer Plus Night Cold Formula, Tylenol Cold Multi-Symptom Nighttime Liquid, Alka-Seltzer Plus Night Severe Cold, Cough and Flu, or Alka-Seltzer Plus Night Sinus Congestion, Allergy and Cough if you are allergic to acetaminophen doxylamine dextromethorphan phenylephrine or any ingredients contained in this drug.
Keep out of reach of children. In case of overdose, get medical help or contact a Poison Control Center immediately.

Contraindications:

Hypersensitivity.
Asthma.
Narrow-angle glaucoma.
Symptomatic prostate hypertrophy.
Bladder-neck obstruction.
Stenosing peptic ulcer.
G-6-PD deficiency.
Severe hepatic impairment.

Cautions:

Acetaminophen hepatotoxicity is possible in chronic alcoholics following various dose levels; severe or recurrent pain or high or continued fever may indicate a serious illness.
Acetaminophen contained in many over-the-counter (OTC) products and combined use with these products may result in toxicity due to cumulative doses exceeding recommended maximum dose.
Do not take dextromethorphan for persistent or chronic cough associated with smoking, asthma, or emphysema, or if it is accompanied by excessive phlegm unless directed by a healthcare provider.
Dextromethorphan may slow respiration rate.
Doxylamine may exacerbate angle-closure glaucoma, hyperthyroidism, peptic ulcer, or urinary tract obstruction; dry mouth may occur.

Pregnancy and Lactation:

Use acetaminophen doxylamine dextromethorphan phenylephrine with caution during pregnancy if the benefits outweigh the risks. Animal studies show risk and human studies are not available or neither animal nor human studies were done.
Acetaminophen doxylamine dextromethorphan phenylephrine is excreted in breast milk. Use caution while breastfeeding.

In the podcast this week, I talk about doxylamine pharmacology. Doxylamine is a first-generation antihistamine; it is commonly an active ingredient in night-time medications like Unisom, Nyquil, and Mucinex. The pharmacology of doxylamine is similar to other first-generation antihistamines, it competitively inhibits the binding of histamine at H1 receptors. Its main uses are as sleep aides, in cough-and-cold medications, but doxylamine has also been given with pyridoxine to treat nausea and vomiting during pregnancy.






Doxylamine’s adverse reactions are related to its anticholinergic properties, they include dry eyes, dry mouth, increased fall risk, sedation, urinary retention, constipation, and confusion. Contraindications include concurrent use with a monoamine oxidase inhibitor, known hypersensitivities, concomitant alcohol use, and if the patient has the following conditions: elevated intraocular pressure, narrow-angle glaucoma, asthma, peptic ulcer disease, urinary bladder neck obstruction, or gastric outlet obstruction. It is also a Beer’s list drug due to its anticholinergic effects. The normal dose in adults is 25 mg. In cases of overdosage, the most common manifestation is exacerbations of its anticholinergic effects. The major complications of an overdose include arrhythmia, respiratory failure, seizures, hyperthermia, rhabdomyolysis, and coma. 






When you know a patient is taking doxylamine, it’s important to be cognizant of their occupation, as well as what other conditions they may have. For example, doxylamine should be used with caution in patients that drive heavy machinery due to its sedating properties. You might be able to tell if a patient’s experiencing an adverse reaction exacerbation if they begin having worsening dementia symptoms or increased urinary retention. Other indications include the use of artificial tears, or saliva, or increased complaints of constipation. To monitor for doxylamine, it’s important to monitor the patient’s tolerability. The onset of doxylamine is relatively quick as well, with a peak concentration within 2-4 hours.






For drug-drug interactions, CYP interactions aren’t as concerning as usual. The main interaction to consider when a patient is taking doxylamine is additive anticholinergic effects. Sedative effects can increase when benzodiazepines, skeletal muscle relaxants, opioids, or antihistamines are concurrently taken. Doxylamine can also counteract the usefulness of dementia or BPH medications due to its anticholinergic properties. There is also a risk of increased anticholinergic burden when taken with skeletal muscle relaxants or tricyclic antidepressants.
