Your Medication Albuterol

Albuterol is also known as the brand names: ProAir HFA, ProAir RespiClick, Ventolin HFA, Proventil HFA, and AccuNeb.

Albuterol is commonly referred to as your rescue inhaler.

Albuterol is most commonly used to help open the airways in lung diseases where spasm may cause breathing problems and also to help prevent exercise-induced breathing problems. Albuterol is used for quick relief in patients who are having breathing problems. It is not used to treat or fix disease, but to help control symptoms. Albuterol is most commonly used on an as needed basis. Do not use more than instructed by your doctor.

Albuterol comes in a 90 mcg/actuation aerosol powder breath activated inhaler, 90mcg/actuation aerosol solution, and a 0.63mg/3ml nebulizer solution for inhalation.

Some common side effects with albuterol include excitement, nervousness, upset stomach, and throat irritation.
Some less common side effects include headache, chest pain, shakiness, drowsiness, and fatigue.
Do not take albuterol if:
• You have a severe hypersensitivity to milk proteins (especially with ProAir RespiClick)
While taking albuterol, remember it is important to tell your doctor or pharmacist if you have:
• Signs of an allergic reaction, such as rash, hives, itching, or peeling skin with or without fever
• Signs of low potassium levels, like muscle pains or weakness, muscle cramps, or a heartbeat that does not feel normal

If prescribed an albuterol inhaler, it is very important that you use your inhaler correctly. Instructions are included in the box containing your inhaler. If you need more assistance with the proper way to use your inhaler, your pharmacist can help you.
If while taking this medication you experience trouble breathing, breathing that is worse, wheezing, or coughing, get medical help right away.
If you miss a dose of albuterol:
• If you use this drug on a regular basis, use a missed dose as soon as you think about it.
• If it is close to the time for your next dose, skip the missed dose and go back to your normal time.
• Do not use 2 doses at the same time or extra doses.
• Many times, this drug is used on an as needed basis. Do not use more often than told by the doctor.
The best way to store albuterol:
• Store at room temperature
• Protect from light
• Store in a dry place. Do not store in a bathroom or refrigerator.
• Keep all drugs in a safe place. Keep all drugs out of the reach of children and pets.

Mechanism of Action of Albuterol: Binds to beta 2 adrenergic receptors which work in the lungs causing them to relax or expand, also known as bronchodilation. This smooth muscle relaxation of the airways helps to improve air exchange.

Side effects of Albuterol: Nervousness, tremor, headache, and hyperactivity because of the beta 2 receptor binding. Chest pain, arrhythmias, palpitations, tachycardia, and hypertension can occur because even though albuterol binds mainly to beta 2 receptor sites working on the lungs, it also has some effect on beta 1 receptor sites in the heart.

Assessment and Considerations for Albuterol: Monitor vital signs, especially heart rate, respiratory rate, and blood pressure. If there is overuse it may lead to tolerance, bronchospasm, or hypokalemia.

Albuterol is a Short-Acting Beta Agonist medication that is given to open up the airways of the lungs to relieve bronchoconstriction. It's properties promote the relaxation of smooth muscle tissue in the airways of the lungs so that they can open up to take in more oxygen. The medication is often used to treat asthma attacks and acute episodes of Chronic Obstructive Pulmonary Disease (COPD). Although it has it's benefits, albuterol definitely comes with certain side effects as well.

Description: Albendazole, a potent anthelmintic of the benzimidazole carbamate class. It serves as a broad-spectrum treatment for various intestinal worm infestations and hydatid disease.

Indications:
Ascariasis, Trichuriasis, Strongyloidiasis, Hookworm infestations, Enterobiasis (pinworm, threadworm), Capillariasis, Cysticercosis, Cutaneous larva migrans, Hydatid disease, Surgery as an adjunct therapy (pre- or post-operatively)
Dosage and Administration:
• For ascariasis, hookworm infestations, trichuriasis, and cutaneous larva migrans in adults and children aged 2 years or older: a single dose of 400 mg.
• In strongyloidiasis: 400 mg daily for 3 consecutive days, with possible repetition after 3 weeks if needed.
• In enterobiasis: A single dose of 100 mg for children aged 2 years or more, repeated after 7 days. Adults receive a 400 mg dose, also repeated after 7 days.
• In hydatid disease: Adults are prescribed 400 mg twice daily, typically in 28-day cycles with a 2-week interval between cycles. The number of cycles may range from 1 to 12.


Side Effects:
• Common side effects during treatment include gastrointestinal disturbances, headache, and dizziness.
• Mild symptoms usually resolve without treatment.
• Rare side effects include rash, fever, and alopecia.

Contraindications:
• Albendazole is contraindicated during pregnancy.

Precautions:
• Reports of elevated hepatic enzyme levels and reversible reduction in total white cell count.
• More common during the treatment of E. multilocularis.

Drug Interactions:
• Albendazole induces liver enzymes of the cytochrome P-450 system, potentially interacting with:
• Theophylline
• Anticonvulsants
• Anticoagulants
• Oral contraceptives
• Oral hypoglycemics
• Caution is advised when introducing Albendazole in patients receiving these compounds.

Overdosage:
• No reported cases of overdosage.
• Gastric lavage within the first two to three hours after ingestion is recommended.
• No specific antidote; treatment involves symptomatic care and general support as needed.

Albendazole is a broad-spectrum anthelmintic belonging to the benzimidazole class.
It is effective against a variety of parasitic worm infections, including nematodes, cestodes, and some protozoa.
Albendazole is indicated for the treatment of various infections, including the following.
Nematode infections such as ascariasis, hookworm, trichuriasis, and strongyloidiasis.
Cestode infections such as neurocysticercosis and hydatid disease.
Mixed helminth infections, particularly in endemic regions.
Protozoal infections like giardiasis.
And cutaneous larva migrans.
Albendazole works by binding to beta-tubulin, inhibiting microtubule polymerization in parasitic cells.
This disrupts vital cellular processes such as glucose uptake, leading to depletion of energy stores and eventual death of the parasite.
It exhibits selective toxicity as it has a higher affinity for parasitic tubulin compared to mammalian tubulin.
Albendazole is administered orally as tablets or suspension.
It has poor oral bioavailability. However, the absorption can be enhanced if taken with a fatty meal.
Albendazole is rapidly converted in the liver to its active metabolite, albendazole sulfoxide, which is responsible for most of its therapeutic effects.
The drug and its metabolites are excreted mainly in urine and, to a lesser extent, in bile.
Half-life is 8-12 hours for albendazole sulfoxide.
Major Adverse Effects of albendazole include the following.
Gastrointestinal effects such as abdominal pain, nausea, vomiting, and diarrhea.
Hepatic effects such as elevation of liver enzymes and, rarely, hepatotoxicity.
Hematologic effects such as bone marrow suppression, leading to neutropenia or pancytopenia.
Neurologic effects such as headache, dizziness, or seizures, especially in neurocysticercosis due to inflammatory reactions caused by dying parasites.
And allergic reactions like skin rash, urticaria, or rarely, anaphylaxis.
Contraindications for the use of albendazole include the following.
Known allergy to albendazole or other benzimidazoles.
Pregnancy.
Contraindicated during the first trimester due to potential teratogenic effects.
Women of childbearing age should use contraception during treatment and for one month afterward.
Liver impairment.
And bone marrow suppression.

In this video, I explain the differences between the well-known non-steroidal anti-inflammatory drugs: Ibuprofen (Advil, Motrin) and Naproxen (Aleve). These are both over-the-counter analgesic medications that are very similar especially when just seeing them on the shelf in the drug store, but there are some key differences to know about when you are taking either of them. In this video, I start off by giving a general overview of NSAIDs (non-steroidal anti-inflammatory drugs), then I break down the key differences between ibuprofen and naproxen. As a nurse practitioner, I am commonly asked about these two drugs and so I hope this video helps you understand the differences between ibuprofen and naproxen as well.

- In this video we'll briefly discuss what is Acyclovir, how it works, what it's used for, the dose and side effects, contraindications and precautions.

- Acyclovir is an antiviral active against herpes simplex virus and varicella zoster virus. It inhibits viral DNA replication by interfering with viral DNA polymerase.

- It is available as 200 mg, 400 mg and 800 mg tablets, and as 40 mg/ml oral suspension. Also available for topical use and injection.

- Acyclovir uses:

- Treatment of recurrent or extensive oral and oesophageal herpes in immunocompromised patients.
- Treatment of herpetic kerato-uveitis.
- Treatment of genital herpes.
- Secondary prophylaxis of herpes in patients with frequent and/or severe recurrences.
- Treatment of severe forms of zoster: necrotic or extensive forms, facial or ophthalmic zoster.

Dosage of Acyclovir:

- Treatment of recurrent or extensive oral and oesophageal herpes in immunocompromised patients, treatment of herpetic kerato-uveitis
Child under 2 years: 200 mg 5 times per day for 7 days
Child over 2 years and adult: 400 mg 5 times per day for 7 days
- Treatment of genital herpes
Child over 2 years and adult: 400 mg 3 times per day for 7 days; in immunocompromised patients, continue treatment until clinical resolution
- Secondary prophylaxis of herpes in patients with frequent and/or severe recurrences
Child under 2 years: 200 mg 2 times per day
Child over 2 years and adult: 400 mg 2 times per day
- Treatment of severe forms of zoster
Adult: 800 mg 5 times per day for 7 days

And for topical use, for both adults and children: Apply to lesions every 3 hr, 6 times/day.

- Contraindications:

- Do not administer to patients with hypersensitivity to acyclovir.

- Acyclovir side effects:

- Acyclovir ( Zovirax ) may cause: headache, skin rash, allergic reactions, gastrointestinal disturbances, raised transaminases, neurologic disorders in patients with renal impairment and elderly patients; rarely, haematological disorders.: Reactions range from mild to life threatening.

- Precautions:

- Reduce dosage in patients with renal impairment.
- Drink a lot of liquid during treatment.
- Genital herpes: Sexual intercourse must be avoided when lesions are present. Use of acyclovir does not prevent transmission.
- Pregnancy: no contra-indication
- Breast-feeding: no contra-indication

Valtrex or the generic valacyclovir treats infection due to herpes simplex type 1 and type 2 which translates to cold sores, fever blisters and genital herpes. It also offers benefits for the skin rash of shingles or herpes zoster. Timing is critically important in both conditions.

Beginning therapy for cold sores or genital herpes during the prodromal symptoms – itching, burning, tingling, numbness – prior to appearance of the lesion offers maximal benefit. Delaying onset until a day or so after the eruption guarantees lack of effect. Similarly for shingles, the window of opportunity requires instituting therapy within 72 hours of onset. Additionally Valtrex does not appear to alter the course of post-herpetic neuralgia, the lingering pain that persists for months after an attack of shingles.

Valacyclovir must be converted in the body to its active form. Removal of the amino acid valine leaves acyclovir to perform the antiviral function. Blood levels of acyclovir from valacyclovir average several times greater than those resulting from taking Zovirax or acyclovir itself.

Dosing for recurrent herpes simplex often involves one 500 mg pill twice daily for 3 days. An alternative regimen is 1000 mg immediately and again at 12 hours. Either seem acceptable. With shingles a higher dose of 1000 mg three times daily for 10 days seems optimal.

For those with frequent recurrences of herpes simplex, a suppressive dose generally reduces the frequency of attacks. A daily dose of 500 or 1000 mg for 6 months often allows a reset. Discontinuing the drug may demonstrate a long symptom free interval. If necessary the valacyclovir may be resumed with the knowledge that with age the frequency of attacks diminishes.

Unfortunately valacyclovir only reduces the frequency of clinical disease. It neither eliminates the virus from its resting place in the nerve nor does it guarantee freedom from attacks while on therapy. Reduces, yes; eliminates, no. The same applies to asymptomatic shedding of the virus and the likelihood of passage to an uninfected partner. Reduces, yes; eliminates, no.

Valacyclovir rarely causes any adverse reaction and even seems safe for pregnant women where the benefits outweigh the negligible risks. Resistance of the virus to the drug appears clinically irrelevant. Valacyclovir should not be present in the circulation when a person receives the chickenpox vaccine.

What is Acyclovir?
Acyclovir (or Aciclovir) is a human-made purine nucleoside analog which suppresses the activity of all two types of herpes simplex virus (HSV-1 and 2), and varicella-zoster virus (VZV). The primary purpose is to inhibit the multiplication of the virus. It decreases the production of the virus's DNA.

How does it work?
Its affinity for thymidine kinase (TK) encoded by HSV and VZV, determines the high selectivity of acyclovir. With the help of viral TK, it is transformed into a nucleotide analog; acyclovir monophosphate which is then converted by host cell kinases to acyclovir triphosphate. ACP-TP prevents further viral DNA synthesis without affecting the natural cellular processes. There are three primary mechanisms of suppression: integration into DNA chain and termination of it, competitive suppression, and deactivation of the DNA polymerase. A high affinity towards the viral enzyme, thymidine kinase (TK) by Acyclovir makes the drug more effective on herpes simplex.

Introduction: Acyclovir, an antiviral agent primarily targeting Herpes Simplex Virus (HSV) infections, presents predominantly mild adverse effects such as malaise (10%), gastrointestinal symptoms, and elevated liver function tests (1-10%). However, more severe complications, notably associated with intravenous administration, encompass rare occurrences of acute renal failure, neurological toxicity, and sporadic cases of bradycardia. A review of literature revealed 2 instances linking acyclovir to bradycardia, with no documented reports on junctional rhythm. A phase IV clinical study examining FDA data identified 1,424 individuals encountering side effects from Acyclovir, with only 6 individuals (0.42%) reporting bradycardia, primarily observed in males aged 60 and above using the medication for less than a month. Here we report a rare case of junctional rhythm caused by acyclovir.
